Une étude de phase 2 sur l'innocuité, l'efficacité et la pharmacodynamique du RTA 408 dans le traitement de l'ataxie de Friedreich (MOXIe)
L'ataxie de Friedreich est une ataxie cérébelleuse autosomique récessive causée par des expansions répétées de triplets. La mutation causale est une expansion répétée du trinucléotide (GAA) dans le premier intron du gène de la frataxine, entraînant une altération de la transcription de la frataxine. Les conséquences pathologiques de la carence en frataxine comprennent une perturbation sévère de la biosynthèse des grappes fer-soufre, une surcharge mitochondriale en fer couplée à une dérégulation cellulaire du fer et une sensibilité accrue au stress oxydatif.
Une caractéristique de l'ataxie de Friedreich est l'altération des mécanismes de défense antioxydante, qui jouent un rôle majeur dans la progression de la maladie. Des études ont démontré que la signalisation du facteur 2 lié au facteur 2 lié à l'érythroïde nucléaire (Nrf2) est gravement altérée chez les patients atteints d'ataxie de Friedreich. Par conséquent, la capacité de l'omaveloxolone (RTA 408) à activer Nrf2 et à induire des gènes cibles antioxydants est supposée être thérapeutique chez les patients atteints d'ataxie de Friedreich.
Cette étude en 2 parties évaluera l'efficacité, l'innocuité et la pharmacodynamie de l'omaveloxolone (RTA 408) dans le traitement des patients atteints d'ataxie de Friedreich.
Partie 1 : La première partie de cette étude sera une étude randomisée, contrôlée par placebo, en double aveugle, à dose croissante pour évaluer l'innocuité de l'omaveloxolone (RTA 408) à différentes doses chez des patients atteints d'ataxie de Friedreich.
Partie 2 : La deuxième partie de cette étude est une étude randomisée, contrôlée par placebo, en double aveugle et en groupes parallèles visant à évaluer l'innocuité et l'efficacité de l'omaveloxolone (RTA 408) 150 mg chez les patients atteints d'ataxie de Friedreich. Les patients inscrits à la partie 2 seront randomisés 1: 1 pour recevoir de l'omaveloxolone (RTA 408) 150 mg ou un placebo.
Extension : L'extension évaluera l'innocuité et la tolérabilité à long terme de l'omaveloxolone (RTA 408) chez les patients qualifiés atteints d'ataxie de Friedreich après avoir terminé la partie 1 ou la partie 2. Les patients ne seront pas levés en aveugle pour étudier le traitement de la partie 1 ou de la partie 2 lors de étude d'extension. Les patients recevront de l'omaveloxolone en ouvert (RTA 408) à 150 mg une fois par jour.

Critère d'intégration:
- Avoir génétiquement confirmé l'ataxie de Friedreich
- Avoir un score FARS modifié ≥20 et ≤80
- Être un homme ou une femme et avoir ≥16 ans et ≤40 ans
- Ne pas modifier le régime d'exercice dans les 30 jours précédant le jour de l'étude 1 et être prêt à rester sur le même régime d'exercice pendant la période d'étude de 16 semaines
- Avoir la capacité de compléter des tests d'effort maximal
- Pouvoir avaler des gélules
Critère d'exclusion:
- Avoir un diabète non contrôlé (HbA1c> 11,0%)
- Avoir une valeur de peptide natriurétique de type B> 200 pg / ml
- Avoir des antécédents de cardiopathie gauche cliniquement significative et/ou de maladie cardiaque cliniquement significative
- Avoir une infection fongique, bactérienne et/ou virale active connue, y compris le virus de l'immunodéficience humaine ou le virus de l'hépatite (B ou C)
- Avoir connu ou suspecté un abus actif de drogues ou d'alcool
- Avoir des anomalies cliniquement significatives de l'hématologie clinique ou de la biochimie, y compris, mais sans s'y limiter, des élévations supérieures à 1,5 fois la limite supérieure de la normale de l'aspartate aminotransférase ou de l'alanine aminotransférase
- Avoir une valeur de test de laboratoire anormale ou une condition médicale préexistante grave qui, de l'avis de l'investigateur, mettrait le patient en danger par l'inscription à l'étude
Avoir pris l'un des médicaments suivants dans les 7 jours précédant le jour 1 de l'étude ou prévoir de prendre l'un de ces médicaments pendant la durée de la participation à l'étude :
- Substrats sensibles au cytochrome P450 2C8 ou 3A4 (p. ex. répaglinide, midazolam, sildénafil)
- Inhibiteurs ou inducteurs modérés ou puissants du cytochrome P450 3A4 (par exemple, carbamazépine, phénytoïne, ciprofloxacine, jus de pamplemousse)
- Substrats pour le transporteur de la glycoprotéine p (p. ex. ambrisentan, digoxine)
- Avoir participé à toute autre étude clinique interventionnelle dans les 30 jours précédant le jour de l'étude 1

Cycle ergometry using a recumbent stationary bicycle was used to conduct maximal exercise testing and workload was increased incrementally.
Peak work is defined as the workload at which participants reach maximal volition (defined as an inability to continue to exercise due to exhaustion).

The CGIC was assessed by completing the following statement "Compared to the participant's condition at the start of the trial, this participant's overall status is", where 1=very much improved, 2=much improved, 3=minimally improved, 4=no change, 5=minimally worse, 6=much worse, 7=very much worse.

Part 2: Change From Baseline in Performance on a 9-Hole Peg Test (9-HPT) at Week 48

The 9-HPT is a brief, standardized, quantitative test of upper extremity (arm and hand) function.
Both the dominant and nondominant hands were tested twice (2 consecutive trials of the dominant hand, followed immediately by 2 consecutive trials of the nondominant hand).
The participant picks up pegs 1 at a time (9 in total), using 1 hand only, and places them into holes on the board as quickly as possible, in any order until all holes are filled.
Then, without pausing, the participant removes the pegs 1 at a time and returns them as quickly as possible.
The times recorded for the two trials for each hand are averaged, and the reciprocal average value for each hand was used in analyses.
Longer test times reflect more impairment on the participant's upper extremity function, thus a negative change from baseline suggests an improvement.

The T25-FWT is a quantitative mobility and leg function performance test based on time (in seconds) to complete a 25-foot walk.
Participants were instructed to attempt two T25-FWT trials at each visit.
Both walk times were averaged, and the reciprocal average value was used in analyses.
Longer test times reflect more impairment on the participant's ability to walk, thus a negative change from baseline suggests an improvement.

The ADL scale examines the ability of participants to complete everyday tasks (e.g., holding a fork, dressing).
The ADL is a 9-question assessment, with the total score being the sum of 9 questions.
The ADL survey assesses 9 concepts: (1) speech; (2) swallowing; (3) cutting food and handling utensils; (4) dressing; (5) personal hygiene; (6) falling; (7) walking; (8) quality of sitting position; and (9) bladder function.
Each of these is rated on a 5-point scale where 0= normal and 4= severe disability/inability to carry out activity independently, for a total possible score of 0 to 36, with higher scores representing greater disability/dependency.
A negative change from baseline indicates improvement.
