Spijsverterings-ENDoscopie na hartstilstand buiten het ziekenhuis (ENTRACT)
Ischemie/reperfusie-fenomeen na hartstilstand leidde tot orgaanbeschadiging en -falen. Van de verschillende organen kan letsel aan het maag-darmkanaal bijdragen aan shock na een hartstilstand.
De ischemische verwonding van de gastro-intestinale (GI) tractus wordt gesuggereerd door afwijkingen in spijsverteringsbiomarkers en door de frequente endotoxemie na CA. Directe mucosale schade is echter niet duidelijk aangetoond na OHCA. De werkelijke incidentie van ischemische laesies van het maagdarmkanaal en hun mogelijke betrokkenheid bij de post-CA-shock is daarom onbekend.
We stellen een origineel klinisch onderzoeksprogramma voor dat gericht is op het rigoureus bepalen van de incidentie van bovenste GI-laesies na OHCA en het analyseren van hun bijdrage aan de ernst van post-CA-shock door middel van een prospectieve, interventionele, multicentrische studie

De screening van de patiënten vindt 2 tot 4 dagen na opname op de Intensive Care (IC) plaats. Procedure en examens op de dag van opname (dag 0):
- verzameling spijsverteringssymptomen
- het uitvoeren van de œsophago-gastro-duodenoscopie en het opstellen van een formeel rapport met de eventuele laesies en hun ischemische aard.
- bij afwezigheid van contra-indicatie en na definitieve beslissing van de gastroscopist, systematische biopsieën van fundus-, antraal- en duodenumslijmvlies; contra-indicatie voor biopten zijn de aanwezigheid van een vasculaire of hemorragische laesie en de stollingsstoornissen.
- Bloedmonster voor H. pylori-serologie en serumbevriezing. Urinemonster voor bevriezing van urine
Procedure en onderzoeken vanaf dag 1 tot ontslag uit het ziekenhuis:
- Dagelijkse verzameling van spijsverteringssymptomen en behoeften aan vasopressorondersteuning
- Sepsis-gerelateerde Organ Failure Assessment (SOFA)-score op dag 2 en 5
- Cerebral Performance Category (CPC) scorebepaling bij ontslag uit het ziekenhuis Bij spijsverteringssymptomen na dag 0 en in overeenstemming met goede klinische praktijken, zal een tweede gastroscopie en/of een abdominale computertomografiescan (CT-scan) en/of een colonoscopie worden uitgevoerd. uitgevoerd naar goeddunken van de arts.

Beschrijving
Inclusiecriteria:
- Volwassen patiënt die minder dan 5 dagen in het ziekenhuis is opgenomen op een deelnemende Intensive Unit Care na een met succes gereanimeerde hartstilstand buiten het ziekenhuis
- Patiënt nog steeds mechanisch beademd
- Onderkoelingsperiode voorbij, lichaamstemperatuur > 36°C.
- Schriftelijke toestemming van een naaste
Uitsluitingscriteria:
- Hartstilstand in het ziekenhuis
- Patiënten geëxtubeerd vóór gastroscopie
- Contra-indicatie gastroscopie: verdenking van spijsverteringsperforatie, ernstige bloedingsdiathese ondanks transfusie van stollingsmiddelen of verdenking van de ziekte van Creutzfeldt-Jacob
- Patiënten met hartklepprothese of eerdere endocarditis
- Zwangerschap, vrouwen die borstvoeding geven In geval van ernstige stollingsstoornissen (bloedplaatjesgetal < 30 G/L, International Normalised Ratio (INR) > 2) of heparinebehandeling of gecombineerde bloedplaatjesremmingsbehandeling is opname van de patiënt mogelijk, maar geen spijsverteringsbiopten toegestaan.
