Sommige onderzoeken hebben aangetoond dat factoren die ventriculaire tachycardie (VT) of ventrikelfibrillatie (VF) recidieven en bijgevolg ICD-interventies voorspellen, de leeftijd van de patiënt, hartfalen van de New York Heart Association-klasse, linkerventrikelejectiefractie (LVEF), de aanwezigheid van atriumfibrilleren ( AF), de breedte van het QRS-complex en de status van de nierfunctie. De nauwkeurigheid van deze parameters is echter beperkt en er wordt gezocht naar andere variabelen die kwetsbare patiënten kunnen identificeren. Dit kan belangrijke klinische implicaties hebben omdat dergelijke patiënten agressievere anti-aritmica kunnen krijgen of profylactische ablatie van aritmiesubstraat kunnen ondergaan om het optreden van aritmieën en ICD-ontladingen te voorkomen. Sommige onderzoeken suggereerden inderdaad dat het de moeite waard zou kunnen zijn om profylactische ablatie van VT uit te voeren bij patiënten met een eerder geïmplanteerde ICD, maar deze aanpak is nog niet in de dagelijkse klinische praktijk ingevoerd.
Een van de methoden van risicostratificatie voor plotselinge hartdood die al vele jaren wordt gebruikt, is geprogrammeerde ventriculaire stimulatie (PVS). Dit is een invasieve studie die de waarschijnlijkheid beoordeelt van het optreden van VT of VF geïnduceerd door stimulatie. Deze test is ook gebruikt om in aanmerking te komen voor profylactische ICD-implantatie bij patiënten met LVEF 31-40% en niet-aanhoudende VT in ambulant ECG. Het primaire nadeel van PVS is de invasiviteit. Moderne ICD's zijn relatief complexe apparaten met een aantal functies, waaronder de mogelijkheid om NIPS uit te voeren met een geïmplanteerde elektrode in het rechterventrikel. NIPS is direct beschikbaar, goedkoop, minimaal belastende ICD-batterij en minder storend voor de patiënt, omdat er geen invasieve procedure voor nodig is.
Sommige onderzoeken suggereerden dat geschikte interventies vaker voorkomen bij patiënten met ventriculaire aritmieën veroorzaakt door NIPS, maar de prognostische betekenis van NIPS is nog niet vastgesteld. Een van de eerste NIPS-gerelateerde onderzoeken toonde aan dat VF/VT-induceerbaarheid tijdens de test van het apparaat kan helpen bij het optimaliseren van de ICD-programmering, maar de voorspellende waarde van NIPS was op dat moment nog niet onderzocht. In een ander onderzoek, een van de eerste en weinige over de prognostische waarde van NIPS, is aangetoond dat inductie van monomorfe, vooral relatief langzame VT (cyclusduur > 280 ms) prognostisch was voor het opnieuw optreden van aritmieën. Deze studie was relatief klein, er was geen rekening gehouden met de verspreiding van etiologie en sommige resultaten waren verrassend, bijv. eerder myocardinfarct van de onderwand, in plaats van de voorwand, voorspelde een gewijzigde uitkomst, terwijl LVEF geen prognostische waarde had. Bovendien waren deze studies vele jaren geleden uitgevoerd, toen het gebruik van primaire dotterbehandeling bij acuut myocardinfarct laag was, en daarom zijn de klinische kenmerken van de patiëntengroepen uit die periode anders dan die van hedendaagse patiënten.

Beschrijving
Inclusiecriteria:
- patiënten met een ICD geïmplanteerd voor zowel primaire als secundaire preventie, ongeacht de etiologie, die worden gevolgd in de polikliniek van ons centrum en die niet voldoen aan de uitsluitingscriteria
Uitsluitingscriteria:
- gebrek aan toestemming voor NIPS
- gedecompenseerd hartfalen
- instabiele angina
- aanhoudend/langdurig AF zonder effectieve antistolling (risico op terugkeer van het sinusritme tijdens NIPS)
- trombus in de linker ventrikel
- passende apparaatinterventies gedurende de 40 dagen voorafgaand aan de geplande NIPS
- stimulatie-/waarnemingsproblemen
