Gedetailleerde beschrijving
Het Center for Infectious Disease Research in Zambia (CIDRZ) zal in samenwerking met het Zambian National TB Program (NTP) en het ministerie van Volksgezondheid (MOH) Xpert-testen in de buurt van het zorgpunt bieden bij een grootschalige antiretrovirale behandeling ( ART) kliniek in Lusaka, Zambia. In totaal zullen 892 studiedeelnemers worden ingeschreven in twee parallelle cohorten: een "Xpert"- en een "standaardzorg"-cohort (SOC) gedurende een periode van 7 maanden. Het Xpert-cohort zal deelnemers inschrijven bij het gezondheidscentrum dat Xpert implementeert. Het SOC-cohort zal deelnemers inschrijven uit één gezondheidscentrum dat de zorgstandaard aanbiedt (sputumuitstrijkmicroscopie plus klinische evaluatie). Er zal een prospectieve cohortstudie met parallelle groepen worden uitgevoerd door deelnemers in één cohort van elke locatie gedurende 210 dagen te volgen vanaf het moment van indiening van het sputum, zodat er voldoende persoonlijke tijd is om alle relevante uitkomsten te observeren, inclusief het voltooien van een standaardkuur van ATT als evenals uitval van patiënten en verlies aan follow-up.
Er zal een systematische beoordelingsenquête van de faciliteiten worden uitgevoerd met behulp van een aangepaste versie van de WHO Service Availability and Readiness Assessment-tool. Sleutelinformanten worden geïnterviewd.

Beschrijving
Inclusiecriteria:
- In staat om minimaal 2 sputummonsters spontaan op te spuwen, in overeenstemming met de NTP-richtlijnen
- Van plan bent om gedurende ten minste 7 maanden zorg te blijven ontvangen op de respectievelijke onderzoekslocaties
- Bereid om locator-informatie te verstrekken en contact per telefoon of huisbezoek toe te staan in geval van verlies om op te volgen na het starten van ATT
Uitsluitingscriteria:
- TBC-behandeling ontvangen binnen 60 dagen voorafgaand aan inschrijving en/of bij wie in de afgelopen 6 maanden tbc is vastgesteld
- Kan niet spontaan sputum ophoesten
- Zijn al ingeschreven in een ander onderzoek en kunnen de implementatie van dit onderzoeksprotocol verstoren
- Een sputummonster verstrekt dat resulteert in een besmet tbc-kweekresultaat
Een sleutelinformant komt in aanmerking voor opname in het SARA-onderzoeksonderdeel van het onderzoek als hij of zij aan de volgende criteria voldoet:
- 18 jaar of ouder
- medewerker van de kliniek in Chilenje Health Center of Chelstone Health Center
- eerder verbonden zijn geweest aan, of anderszins bekend zijn met, afdelingen die hiv-, tbc- en/of laboratoriumgezondheidsdiensten aanbieden
Uitsluitingscriteria:
-onwillig of niet in staat om mondelinge geïnformeerde toestemming in het Engels te geven
