Veiligheid en werkzaamheid van Saracatinib bij proefpersonen met lymfangioleiomyomatose (SLAM-2)

Tubereuze sclerose-complex (TSC) is een autosomaal dominante aandoening die wordt veroorzaakt door mutaties in tubereuze sclerose-complex 1 (TSC1) of TSC2-tumorsuppressorgenen. TSC wordt gekenmerkt door tumoren in een breed scala van weefsels, epileptische aanvallen, mentale retardatie, autisme en orgaanfalen. Lymfangioleiomyomatose (LAM), de belangrijkste pulmonale manifestatie bij vrouwen met TSC, is een progressieve longziekte die wordt gekenmerkt door infiltratie van atypische gladde spiercellen (TSC-/-LAM-cellen) en vorming van parenchymale cysten. Sporadische LAM kan zich ontwikkelen bij vrouwen die niet voldoen aan de criteria voor de diagnose van TSC, vanwege somatische mutaties in het TSC2-gen.
Het langetermijndoel van dit onderzoek is het ontwikkelen van nieuwe therapeutische strategieën voor patiënten met LAM. Het waargenomen gedrag van LAM-cellen met betrekking tot hun infiltratieve groeipatroon, metastatisch potentieel en veranderde celdifferentiatie doet denken aan cellen die een epitheel-mesenchymale overgang (EMT) ondergaan. Src-kinases zijn belangrijke regulatoren van cellulaire proliferatie, motiliteit, invasiviteit en EMT. Recente resultaten hebben aangetoond dat autofagie de afbraak van actief Src bevordert. Daardoor kan verminderde autofagie als gevolg van mTOR-activering waarvan bekend is dat deze voorkomt in LAM-cellen, een belangrijke rol spelen bij de accumulatie van actief Src in deze cellen. Src onderdrukt de transcriptie van E-cadherine door zijn transcriptionele repressoren opwaarts te reguleren. De voorlopige gegevens onthullen een toename van actieve Src in longweefsels van patiënten met LAM en in gekweekte TSC2-/--cellen. Verder is E-cadherine in TSC2-/--cellen aanzienlijk gereduceerd en lokaliseert het niet naar het plasmamembraan, zoals in wildtype cellen.
De focus van deze studie is om te onderzoeken of Src-remming een potentiële therapeutische strategie in LAM vertegenwoordigt. Er wordt voorgesteld dat Src-activering in TSC2-/--cellen resulteert in de vermindering van E-cadherine, verlies van cel-celadhesie en verhoging van het oncogene en metastatische potentieel van deze cellen. De verhoogde Src-activiteit in TSC2-/--cellen wordt waarschijnlijk veroorzaakt door remming van autofagie geassocieerd met hyperactivering van mTOR. Daarom kan het gebruik van Src-remmers leiden tot een vermindering van de tumorgroei en het voorkomen van verspreiding van TSC2-/--cellen. In deze studie zullen de onderzoekers de veiligheid en werkzaamheid van Src-remming bij proefpersonen met LAM evalueren.
Een aantal remmers van Src wordt nu klinisch onderzocht bij patiënten met een reeks verschillende tumoren. Dasatinib, een orale adenosinetrifosfaat (ATP)-competitieve Src-remmer, is nu goedgekeurd voor klinisch gebruik bij patiënten met chronische myeloïde leukemie of acute lymfoblastische leukemie. Dasatinib heeft echter een bredere 'off target'-remmende activiteit dan saracatinib, waaronder krachtige activiteit tegen Abl, ephrin-receptorkinasen, van bloedplaatjes afgeleide groeifactorreceptor en c-KIT (type receptortyrosinekinase en een type tumormarker. Ook wel CD117 en stamcelfactorreceptor genoemd.) Daarentegen heeft saracatinib een >10-voudige voorkeur voor Src boven Abl-kinases en heeft het zeer weinig activiteit tegen andere tyrosine- en serine/threonine-kinases. Saracatinib is al in meerdere klinische onderzoeken gekarakteriseerd op het gebied van veiligheid en farmacokinetiek.
De onderzoekers hebben een fase 1b-onderzoek uitgevoerd om de veiligheid van verschillende doses Saracatinib bij LAM-proefpersonen te testen. Het doel van de fase 1b-studie was om de optimale dosis te bepalen in termen van veiligheid en verdraagbaarheid in LAM-populatie. Drie escalerende doses saracatinib; 50, 125 en 175 mg werden onderzocht. Saracatinib werd eenmaal daags oraal toegediend. Over het algemeen verdroegen proefpersonen Saracatinib goed. De onderzoekers kozen de dosis van 125 mg om verder te testen op zowel veiligheid als werkzaamheid in deze fase 2a-studie.

Beschrijving
Inclusiecriteria:
- Vrouwelijke patiënten. Opgemerkt moet worden dat LAM bijna uitsluitend bij vrouwen voorkomt.
- 18 tot 65 jaar.
- Alle patiënten moeten een diagnose van LAM hebben, zoals gedefinieerd door compatibele cystische veranderingen op thoraxcomputertomografie (CT) en een van de volgende:
- Open long-, transbronchiale of thoracale naaldbiopsie consistent met LAM
- Open of naaldbiopsiebevindingen consistent met LAM
- Klinische bevindingen van tubereuze scleromacomplex (TSC), renaal angiomyolipoom, cystische abdominale lymfangiomen of voorgeschiedenis van chyluseffusie in de borst of buik
- Serum vasculaire endotheliale groeifactor D (VEGF-D) > 800 pg/ml
- Proefpersonen moeten een recente vermindering van het geforceerde expiratoire volume op 1 seconde (FEV1) van > 50 ml/jaar hebben gehad, zoals blijkt uit ten minste twee longfunctietesten (PFT), gemeten met een tussenpoos van ten minste 6 maanden in de laatste 24 maanden voorafgaand aan inschrijving studie.
Uitsluitingscriteria:
- Huidige infectie.
- Grote operatie in de afgelopen 2 maanden
- Gevorderde hematologische, nier-, lever-, niet-LAM-longziekte of metabole ziekten; of BMI van >35
- Het gebruik van een ander onderzoeksgeneesmiddel binnen 30 dagen
- Het gebruik van mTOR-remmers (mammalian target of rapamycin) binnen 30 dagen
- Eerdere longtransplantatie.
- Onvermogen om geplande kliniekbezoeken bij te wonen
- Onvermogen om geïnformeerde toestemming te geven
- Onvermogen om longfunctietesten uit te voeren
- Geschiedenis van maligniteit in de afgelopen twee jaar, anders dan plaveiselcel- of basaalcelkanker of status na succesvolle excisie of behandeling.
- Moeders die borstvoeding geven
- Huidige of geplande zwangerschap.
- Geen adequate anticonceptie gebruiken (bij vrouwen in de vruchtbare leeftijd).
- Significante klinische verandering in gezondheid in de afgelopen 30 dagen

Experimenteel: Saracatinib
Saracatinib wordt oraal toegediend in een dosis van 125 milligram eenmaal daags gedurende 9 maanden.
Saracatinib wordt geleverd als een roze tablet.

Proefpersonen krijgen bij elk bezoek voldoende tabletten voor 90 dagen +/- 14 dagen.
De proefpersoon zal om de 90 dagen een bezoek brengen voor de verantwoordelijkheid van het geneesmiddel, evenals veiligheids- en werkzaamheidstesten, waaronder longfunctietesten, laboratoriumtesten om lever- en nierprofiel op te nemen, urine-zwangerschapstesten bij elk bezoek, vitale functies, lichamelijk onderzoek - alle medisch significante veranderingen van baseline bezoek zal worden geregistreerd, alle bijwerkingen zullen worden gecontroleerd totdat ze zijn opgelost.
