BK-virus na niertransplantatie: nieuwe praktijk versus traditionele aanpak

Deze studie zal een gecombineerde retrospectieve kaartreview en prospectieve observationele studie zijn. Dit zal een project in één centrum zijn dat zal plaatsvinden in het Loma Linda University Transplant Institute. Alle volwassen nierontvangers van wie is vastgesteld dat ze BK-viremie hebben met een BK PCR-virale belasting van meer dan 500 kopieën na transplantatie en die voldoen aan de inclusiecriteria maar geen uitsluitingscriteria, worden ingeschreven en geobserveerd volgens het onderzoeksprotocol.
Alle ontvangers van een niertransplantatie worden routinematig beoordeeld op BK-virusnefropathie en viremie na de transplantatie als onderdeel van hun standaardzorg. Patiënten met BK-viremie of BK-virusnefropathie zullen hun immunosuppressieve medicatie moeten aanpassen op basis van de huidige richtlijnen en aanbevelingen, waaronder vermindering van immunosuppressie, behandeling met medicijnen met antivirale werking (Cidofovir, Ciprofloxacine, Leflunomide, IVIG) of overstappen van een Tacrolimus (Prograft) -gebaseerd regime naar een op ciclosporine gebaseerd regime. Retrospectief zal de onderzoeker gegevens verzamelen over patiënten bij wie de diagnose BK-viremie of BK-virusnefropathie is gesteld en die in de afgelopen 66 maanden (van 1/1/2010 tot 30/06/2015) een dergelijke behandeling hebben gehad. In de toekomst zal de onderzoeker zich inschrijven en gegevens verzamelen over patiënten die zullen worden behandeld voor BK-viremie of BK-virusnefropathie.
Als standaardzorg zullen alle ontvangers van een niertransplantatie routinematig wekelijks of tweewekelijks worden gezien in de post-transplantatiekliniek gedurende maximaal 3 maanden, daarna elke 3 maanden gedurende maximaal een jaar en daarna jaarlijks. Alle ontvangers van een niertransplantaat zullen worden gecontroleerd op BK-viremie door middel van bloed-PCR-testen op maandelijkse basis op 1, 2, 3, 6 en 12 maanden na de transplantatie. Ook zullen alle patiënten na niertransplantatie worden getest op BK-viremie als er een acute stijging van hun creatinine is.

Inclusiecriteria:
- Leeftijd ≥ 18 jaar
Ontvanger van nier of gecombineerde nier en pancreas
- BK-viremie bevestigd met bloed-PCR-test met virale belasting van meer dan 500
- BK-virusnefropathie bevestigd door nierbiopsie
Uitsluitingscriteria:
- Ontvanger van gecombineerde orgaantransplantatie behalve nier en pancreas
- Leeftijd patiënt < 18 jaar
- Als de patiënt niet instemt met een nierbiopsie
- Momenteel in behandeling voor acute afstoting
- Patiënten met een HIV-, Hep C- of Hep B-infectie
- Patiënten die andere immunosuppressie gebruiken naast ons standaardregime, waaronder Prograf, cellcept/Myfortic en prednison
