Denne studien vil være en kombinert retrospektiv kartgjennomgang og prospektiv observasjonsstudie. Dette vil være et enkeltsenterprosjekt som vil finne sted ved Loma Linda University Transplant Institute. Alle voksne nyremottakere som er fastslått å ha BK-viremi med en BK PCR-virusmengde på over 500 kopier etter transplantasjon og oppfyller inklusjonskriteriene, men ingen eksklusjonskriterier vil bli registrert og observert i henhold til studieprotokollen.
Alle nyretransplanterte blir rutinemessig vurdert for BK-virus nefropati og viremi etter transplantasjon som en del av deres standardbehandling. Pasienter med BK-viremi eller BK-virusnefropati vil få justeringer i sin immunsuppressive medisin basert på gjeldende retningslinjer og anbefalinger som inkluderer reduksjon i immunsuppresjon, behandling med medisiner med antiviral aktivitet (Cidofovir, Ciprofloxacin, Leflunomide, IVIG) eller bytte fra Tacrolimus (Prograf) -basert regime til et cyklosporin-basert regime. Retrospektivt vil etterforskeren samle inn data om pasienter som har blitt diagnostisert med BK-viremi eller BK-virusnefropati og har hatt en slik behandling de siste 66 månedene (fra 1.1.2010 til 30.06.2015). Prospektivt vil etterforskeren registrere og samle inn data om pasienter som skal behandles for BK-viremi eller BK-virusnefropati.
Som standardbehandling vil alle nyretransplantasjonsmottakere bli sett rutinemessig ukentlig ukentlig på klinikken etter transplantasjon i opptil 3 måneder, deretter hver 3. måned i opptil ett år og deretter årlig. Alle nyretransplanterte mottakere vil bli overvåket for BK-viremi ved å få testet blod-PCR på månedlig basis 1, 2, 3, 6 og 12 måneder etter transplantasjonen. Alle pasienter etter nyretransplantasjon vil også bli testet for BK-viremi hvis det er en akutt økning i kreatinin.

Inklusjonskriterier:
- Alder ≥ 18 år
Mottaker av nyre eller kombinert nyre og bukspyttkjertel
- BK-viremi bekreftet med blod PCR-test med virusmengde over 500
- BK-virus nefropati bekreftet ved nyrebiopsi
Ekskluderingskriterier:
- Mottaker av kombinert organtransplantasjon unntatt nyre og bukspyttkjertel
- Pasientalder < 18 år
- Hvis pasienten ikke samtykker til nyrebiopsi
- For tiden på behandling for akutt avstøtning
- Pasienter med HIV, Hep C eller Hep B infeksjon
- Pasienter som er på annen immunsuppresjon ved siden av vårt standardregime som inkluderer Prograf, cellcept/Myfortic og prednison
