Ongelijke placenta-deling bij monochoriale diamniotische tweelingen: een observatiestudie om voorspelling en uitkomst te onderzoeken: de TWINSHARE-studie

In alle cohorten wordt tijdens de zwangerschap serieel echografie uitgevoerd. Tijdens de zwangerschap wordt eenmaal MRI aangeboden. Na de bevalling vindt er een placenta-onderzoek plaats.
Bij niet-geselecteerde MCDA-tweelingen zullen de onderzoekers de relatie beoordelen tussen ongelijke verdeling van de placenta en het risico op foetaal of neonataal verlies. Ook gaan de onderzoekers na of metingen van de navelstrengader of MRI kunnen worden gebruikt om het delen van de placenta te voorspellen.
In gevallen gecompliceerd door het twin-to-twin transfusiesyndroom (TTTS), zullen de onderzoekers kijken naar het verband tussen ongelijke placenta-deling en intra-uteriene foetale dood na laserchirurgie.
In gevallen gecompliceerd door selectieve intra-uteriene groeirestrictie (sIUGR), zal de relatie tussen ongelijke verdeling van de placenta en het risico op foetaal of neonataal verlies en geboorte vóór 34 weken worden beoordeeld.
Bovendien zullen gegevens over de neurologische uitkomst op de leeftijd van 2 jaar in alle cohorten worden verzameld.

Inclusiecriteria:
MCDA-cohort
- Doorlopende monochoriale diamniotische tweelingzwangerschap tussen 11 en 14 weken, zoals bepaald door de kruinstaartlengte van de grotere tweeling bij spontane concepties en door de datum van inseminatie of embryonale leeftijd bij vervanging bij zwangerschappen als gevolg van subfertiliteitsbehandeling
- Vrouwen van 18 jaar of ouder die toestemming kunnen geven
- Schriftelijke geïnformeerde toestemming om deel te nemen aan deze studie, formulieren worden goedgekeurd door de ethische commissies
TTTS-cohort
- Monochoriale diamniotische tweelingzwangerschap, met zwangerschapsduur gedefinieerd in het eerste trimester zoals hierboven vermeld
- Gecompliceerd door TTS volgens de definitie die wordt gebruikt in de Quintero-stadiëring van TTS (DVP van donor < 2 cm en DVP van receptor > 8 cm vóór 20 weken of > 10 cm na 20 weken
- Laserchirurgie ondergaan voor TTS
- Vrouwen van 18 jaar of ouder die toestemming kunnen geven
- Schriftelijke geïnformeerde toestemming om deel te nemen aan deze studie, formulieren worden goedgekeurd door de ethische commissies
sIUGR-cohort
- Monochoriale diamniotische tweelingzwangerschap, met zwangerschapsduur gedefinieerd in het eerste trimester zoals hierboven vermeld
- Gecompliceerd door sIUGR gedefinieerd als een discordantie in EFW van ≥ 20%, waarbij het percentage discordantie wordt berekend als (EFW grotere tweeling - EFW kleinere tweeling)/EFW grotere tweeling x 100.
- Vrouwen van 18 jaar of ouder die toestemming kunnen geven
- Schriftelijke geïnformeerde toestemming om deel te nemen aan deze studie, formulieren worden goedgekeurd door de ethische commissies
Uitsluitingscriteria:
- Leeftijd < 18 jaar
- Onvermogen om in te stemmen
