Translationeel validatieonderzoek om KFO179-1-biomarkerscores te onderzoeken voor de voorspelling en prognose van geavanceerde primaire resectabele endeldarmkankerstadia UICC-II-IV, met een op 5-fluorouracil gebaseerde standaard radiochemotherapie gevolgd door totale mesorectale excisie. (TransValid-A)
Translationele validatiestudie om KFO179-1-biomarkerscores te onderzoeken voor de voorspelling en prognose van geavanceerde primaire resectabele endeldarmkankerstadia UICC II-IV, met een op 5-FU gebaseerde standaard radiochemotherapie gevolgd door totale mesorectale excisie.
Het doel van de TransValid-KFO179/GRCSG-Trial-A is de validatie van potentiële biomarkers. Dit zijn voorspellende (voorspelling van de waarschijnlijkheid van respons op een bepaalde therapie) / prognostische (voorspelling van langetermijnuitkomsten) op microarray gebaseerde genexpressiesignaturen en immunohistochemisch geëvalueerde biomarkers. De evaluatie is gedaan binnen de KFO179 (www.kfo179.de) - de validatie is geïmplementeerd in deze studie.
Daarom zal tumormateriaal van patiënten die standaard radiochemotherapie ondergaan worden geanalyseerd uit voorbehandelingsbiopten en restweefsel van het resectiepreparaat na de operatie. Deze validatie en de bevestiging van het biomateriaal zullen in alle deelnemende centra in de klinische routine worden opgenomen als model voor de behandeling van solide tumoren. De verkregen biomarkers met een voorspellende en prognostische kracht zullen gebruikt worden om een algoritme te ontwikkelen om patiënten met een hoog risico op lokale en verre kankerrecidief te voorspellen.

Inclusiecriteria:
- Van 18 tot en met 85 jaar
- Histologisch bevestigde gevorderde primaire endeldarmkanker gelokaliseerd tot 12 cm boven de anocutane lijn (bepaald met een starre rectoscoop), geclassificeerd als T3/T4- of N+-carcinomen of met aanwijzingen voor synchrone, maar reseceerbare metastasen op afstand (lever- of longmetastasen)
- Geen specifieke tumorbehandeling behalve colostoma vanwege tumorstenose met ileus
- Wereldgezondheidsorganisatie (WHO)/Eastern Cooperative Oncology Group (ECOG)-status ≤2
- Adequate beenmergfunctie (WBC >3,0x10^9/L, neutrofielen >1,5x10^9/L, trombocyten >100x10^9/L, hemoglobine ≥10 g/dl)
- Adequate leverfunctie (bilirubine ≤ 2,0 mg/dl, SGOT, SGPT, AP, gamma-GT < drie komma vijf keer de bovengrens van het normale bereik
- serumcreatinine < 1,5 mg/dl
- Schriftelijke en ondertekende geïnformeerde toestemming waaruit blijkt dat u het onderzoekskarakter en het onderzoeksprotocol begrijpt.
Uitsluitingscriteria:
- Zwangere of zogende vrouwen
- Mannen en vrouwen die geen zeer effectieve anticonceptiemethoden willen of kunnen gebruiken (volgens institutionele standaard) tijdens de behandeling en gedurende 6 maanden (mannelijk of vrouwelijk) na het einde van de behandeling
- Langdurig misbruik van drugs, medicijnen of alcohol
- Eerdere chemotherapie (tot 2 jaar voor de diagnose van endeldarmkanker)
- Eerdere bestraling van het bekkengebied
- Gelijktijdige therapie met andere geneesmiddelen tegen kanker
- Deelname aan een klinische studie in de periode 30 dagen voorafgaand aan opname
- Patiënten (man en vrouw) die geen behandeling en nazorg volgens het onderzoeksprotocol kunnen of willen accepteren
Patiënten (man en vrouw) met ongecontroleerde, ernstige lichamelijke of geestelijke aandoeningen, bijv.: instabiele hartziekte ondanks medische behandeling, myocardinfarct in de laatste 3 maanden voorafgaand aan deelname aan de studie
- neurologische of psychiatrische disfunctie waaronder dementie of convulsies
- Gedissemineerde infectie of sepsis
- Gedissemineerde intravasculaire coagulopathie
- Symptomatische neuropathie (NCI CTC ≥2)
- Patiënten met secundaire maligniteiten behalve basaalcelcarcinoom van de huid of carcinoom in situ van de cervix, die met succes zijn behandeld. (De opname van patiënten met andere tumoren die met succes zijn behandeld en die in de afgelopen 3-5 jaar geen recidief hebben gehad, moet worden besproken vóór registratie in het onderzoek)

Ander: Bloed- en weefselmonsters tijdens therapie
Verzameling van bloed- en weefselmonsters tijdens preoperatieve multimodale behandeling (radiochemotherapie (RCTx) gevolgd door totale mesorectale excisie (TME) en chemotherapie (CT)) bij endeldarmkanker.

Bloed-/biopsiemonsters (tumor en slijmvlies) worden pretherapeutisch genomen. Tijdens RCTx worden op 11 tijdstippen bloedmonsters (serum en plasma) afgenomen (bijv. voor analyse van 5-FU-metabolisme, genomische DNA-extractie). Na pathohistologische opwerking van het TME-specimen zal het met formaline gefixeerde materiaal worden overgebracht naar de centrale biobank van KFO179. Tijdens de 5-jarige follow-up zullen bloedmonsters (plasma en serum) worden afgenomen. Behandeling (gebaseerd op CAO/ARO/AIO-94- en CAO/ARO/AIO-04-fase III-onderzoeken van de GRCSG): Preoperatieve RCT: 28x1.8 Gy (totaal: 50,4 Gy, 5 fracties per week op d1-d38) gecombineerd met 5-FU (1000 mg/m2/d) als 120 h civ gedurende 1e en 5e week. TME-chirurgie wordt 6 weken na RCTx uitgevoerd. 4 tot 8 weken na de operatie krijgen patiënten ofwel 4 cycli 5-FU (500 mg 5-FU/m2 iv, d1-5, herhaling d 29-33) of 3 cycli (6 enkele appl.) van het FOLFOX-regime [(400 mg FA/m2, 2 uur civ, d 1; 100 mg oxaliplatine/m2, 2 uur civ in 500 ml glucose 5%, d 1; 2400 mg 5-FU/m2 als 46 uur civ) op d1+d15 , d30+d45 en d60+d75]. |

De werkzaamheid van de pretherapeutisch bepaalde responsvoorspelling met behulp van een betrouwbaar en robuust panel van biomarkers (genexpressiesignatuur) wordt beoordeeld door verschillende klinisch-pathologische parameters na preoperatieve RCTx- en TME-chirurgie die respons en toxiciteit aangeven (ypN-status, pCR, tumorregressie-grading , R-status, toxiciteit).
