Étude de validation translationnelle pour examiner les scores des biomarqueurs KFO179-1 pour la prédiction et le pronostic des stades avancés du cancer rectal primaire résécable UICC-II-IV, avec une radiochimiothérapie standard à base de 5-fluorouracile suivie d'une excision mésorectale totale. (TransValid-A)
Étude de validation translationnelle pour examiner les scores des biomarqueurs KFO179-1 pour la prédiction et le pronostic des stades avancés du cancer rectal primaire résécable UICC II-IV, avec une radiochimiothérapie standard à base de 5-FU suivie d'une excision mésorectale totale.
L'objectif du TransValid-KFO179/GRCSG-Trial-A est la validation de biomarqueurs potentiels. Il s'agit de signatures d'expression génique basées sur des puces à ADN prédictives (prédiction de la probabilité de réponse à une certaine thérapie) / pronostiques (prédiction des résultats à long terme) et de biomarqueurs évalués par immunohistochimie. L'évaluation a été effectuée au sein du KFO179 (www.kfo179.de) - la validation est mise en œuvre dans cet essai.
Par conséquent, le matériel tumoral des patients subissant une radiochimiothérapie standard sera analysé à partir des biopsies de prétraitement et du tissu résiduel de l'échantillon de résection après la chirurgie. Cette validation et l'assertion du biomatériau seront intégrées dans la routine clinique de tous les centres participants en tant que modèle pour le traitement des tumeurs solides. Les biomarqueurs obtenus avec un pouvoir prédictif et pronostique seront utilisés pour développer un algorithme permettant de prédire les patients à haut risque de récidive locale et à distance du cancer.

Critère d'intégration:
- De 18 à 85 ans inclus
- Cancer rectal primitif avancé histologiquement confirmé localisé jusqu'à 12 cm au-dessus de la ligne anocutanée (déterminé avec un rectoscope rigide), classé comme carcinome T3/T4 ou N+ ou avec preuve de métastases à distance synchrones mais résécables (métastases hépatiques ou pulmonaires)
- Pas de traitement tumoral spécifique sauf colostomie due à une sténose tumorale avec iléus
- Statut Organisation mondiale de la santé (OMS)/Eastern Cooperative Oncology Group (ECOG) ≤2
- Fonction adéquate de la moelle osseuse (GB > 3,0 x 10 ^ 9/L, neutrophiles > 1,5 x 10 ^ 9/L, thrombocytes > 100 x 10 ^ 9/L, hémoglobine ≥ 10 g/dl)
- Fonction hépatique adéquate (bilirubine ≤ 2,0 mg/dl, SGOT, SGPT, AP, gamma-GT < trois virgule cinq fois le niveau supérieur de la plage normale
- créatinine sérique < 1,5 mg/dl
- Consentement éclairé écrit et signé indiquant la compréhension de la nature expérimentale et du protocole d'étude.
Critère d'exclusion:
- Femmes enceintes ou allaitantes
- Hommes et femmes refusant ou incapables d'utiliser des méthodes de contraception hautement efficaces (selon les normes institutionnelles) pendant le traitement et pendant 6 mois (homme ou femme) après la fin du traitement
- Abus prolongé de drogues, de médicaments ou d'alcool
- Chimiothérapie antérieure (jusqu'à 2 ans avant le diagnostic de cancer du rectum)
- Radiothérapie antérieure de la région pelvienne
- Traitement simultané avec d'autres médicaments anticancéreux
- Participation à un essai clinique dans la période de 30 jours précédant l'inclusion
- Patients (homme et femme) qui ne sont pas capables ou désireux d'accepter un traitement et des soins de suivi selon le protocole de l'essai
Patients (homme et femme) atteints de maladies physiques ou mentales graves non contrôlées, par exemple : maladie cardiaque instable malgré un traitement médical, infarctus du myocarde au cours des 3 derniers mois précédant le début de la participation à l'essai
- dysfonctionnement neurologique ou psychiatrique, y compris démence ou trouble convulsif
- Infection disséminée ou septicémie
- Coagulopathie intravasculaire disséminée
- Neuropathie symptomatique (NCI CTC ≥2)
- Patients atteints de tumeurs malignes secondaires, à l'exception du carcinome basocellulaire de la peau ou du carcinome in situ du col de l'utérus, qui ont été traités avec succès. (L'inclusion de patients avec d'autres tumeurs qui ont été traitées avec succès et sans récidive au cours des 3 à 5 dernières années doit être discutée avant l'inscription à l'essai)
- Diarrhée chronique (>grade 1 selon NCI CTCAE)
- Réaction allergique aux dérivés du platine ou aux médicaments à l'étude
- Traitement simultané avec la sorivudine et analogues
- Déficit connu en dihydropyrimidine déshydrogénase

Autre: Échantillons de sang et de tissus pendant le traitement
Prélèvement d'échantillons sanguins et tissulaires lors d'un traitement multimodal préopératoire (Radiochimiothérapie (RCTx) suivie d'une exérèse mésorectale totale (ETM) et d'une Chimiothérapie (CT)) dans le cancer du rectum.

Des échantillons de sang/biopsie (tumeur et muqueuse) sont prélevés avant le traitement. Des échantillons de sang (sérum et plasma) sont prélevés à 11 points dans le temps pendant le RCTx (par exemple, pour l'analyse du métabolisme du 5-FU, l'extraction de l'ADN génomique). Après bilan pathohistologique de l'échantillon TME, le matériel fixé au formol sera transféré à la biobanque centrale de KFO179. Des échantillons de sang (plasma et sérum) seront prélevés au cours du suivi de 5 ans. Traitement (basé sur les essais CAO/ARO/AIO-94- et CAO/ARO/AIO-04-phase-III du GRCSG) : ECR préopératoire : 28 x 1,8 Gy (total : 50,4 Gy, 5 fractions par semaine à j1-j38) associé au 5-FU (1000 mg/m2/j) en 120 h civ pendant la 1ère et la 5ème semaine. La chirurgie TME est réalisée 6 semaines après RCTx. 4 à 8 semaines après la chirurgie, les patients reçoivent soit 4 cycles de 5-FU (500 mg 5-FU/m2 iv, j1-5, répéter j 29-33) soit 3 cycles (6 appl. simples) de régime FOLFOX [(400 mg FA/m2, 2 h civ, j 1 ; 100 mg oxaliplatine/m2, 2 h civ dans 500 ml Glucose 5 %, j 1 ; 2400 mg 5-FU/m2 en 46 h civ) à j1+j15 , d30+d45 et d60+d75]. |

Nombre de patients présentant une rémission complète (pCR) confirmée histopathologiquement (ypT0N0 R0) après RCTx préopératoire liée à la signature d'expression génique préthérapeutique prédictive de la réponse tumorale à RCTx.

L'efficacité de la prédiction de la réponse déterminée de manière préthérapeutique à l'aide d'un panel fiable et robuste de biomarqueurs (signature d'expression génique) est évaluée par plusieurs paramètres clinicopathologiques après la chirurgie RCTx et TME préopératoire qui indiquent la réponse et la toxicité (statut ypN, pCR, régression tumorale , statut R, toxicité). Le moment de mesure de la signature d'expression génique est le moment du diagnostic (biopsie avant le traitement); plusieurs biomarqueurs immunohistochimiques (Thymidylatesynthase, Survivin, HER-2) seront déterminés dans la biopsie de prétraitement ainsi qu'au moment de la résection dans le tissu tumoral résiduel. |
