Hyperglykemie in het ziekenhuis komt vaak voor en is in verband gebracht met verhoogde ziekenhuiscomplicaties, verblijfsduur en mortaliteit. Het is aangetoond dat het verbeteren van de glykemische controle de verblijfsduur, multi-orgaanfalen, systemische infecties en mortaliteit op korte en lange termijn verbetert. Klinische richtlijnen van professionele organisaties bevelen het gebruik van subcutane (SQ) insuline aan als voorkeurstherapie voor glykemische controle bij algemene medische en chirurgische patiënten met T2D. Deze benadering is echter arbeidsintensief en vereist meerdere dagelijkse insuline-injecties, is kostbaar en gaat gepaard met een aanzienlijk risico op iatrogene hypoglykemie.
Meer dan 75% van de patiënten met T2D wordt behandeld met orale antidiabetica (OAD's), maar vanwege het gebrek aan veiligheids- en werkzaamheidsgegevens uit gerandomiseerde gecontroleerde studies bevelen klinische richtlijnen aan om OAD's te stoppen tijdens ziekenhuisopname. De huidige klinische richtlijnen hebben aanleiding gegeven tot bezorgdheid over het gebruik van OAD's, waaronder het risico op hypoglykemie met sulfonylureumderivaten, vochtretentie en verergering van hartfalen met thiazolidinedionen, en het risico op metformine-geassocieerde lactaatacidose bij patiënten met een ernstige nierfunctiestoornis. Verschillende observationele studies hebben echter gemeld dat het gebruik van OAD's resulteert in vergelijkbare glykemische controle zonder verhoogd risico op complicaties in vergelijking met insulineregimes. Een recent observationeel onderzoek met 17.325 gehospitaliseerde patiënten met T2D toonde aan dat patiënten die met OAD's werden behandeld, een vergelijkbare glykemische controle hadden zonder verschillen in complicaties en geen toename van het aantal hypoglykemie in vergelijking met degenen die met insuline werden behandeld.
Deze studie zal beoordelen of voortzetting van orale antidiabetica thuis tijdens ziekenhuisopname kan worden gebruikt als een veilig en effectief alternatief voor insulinetherapie bij de behandeling van diabetes bij ziekenhuispatiënten met T2D. Voor een subset van deelnemers (50 patiënten per groep) zal een CGM-apparaat worden geplaatst voor de duur van het onderzoek om parameters van glykemische controle en hypoglykemie te beoordelen.

Beschrijving
Inclusiecriteria:
- Mannen of vrouwen, leeftijd 18-80 jaar opgenomen in een algemene geneeskunde en chirurgie
- Bekende geschiedenis van T2D die OAD's ontving, hetzij als monotherapie of in combinatietherapie
- Opname BG < 250 mg/dl of randomisatie BG < 250 mg/dl en geen basale insuline
- Patiënten die OAD's krijgen in combinatie met GLP-1-receptoragonisten (GLP-1RA) met een HbA1c <7,5% in de afgelopen drie maanden
Uitsluitingscriteria:
- Geen bekende voorgeschiedenis van diabetes
- Laboratoriumbewijs van diabetische ketoacidose
- Proefpersonen met een voorgeschiedenis van diabetes type 1 (aangeraden door BMI < 25 waarvoor insulinetherapie nodig is of met een voorgeschiedenis van diabetische ketoacidose of ketonurie)
- Voldoen aan eventuele uitsluitingscriteria op basis van specifieke contra-indicaties voor hun orale thuistherapie
- Acute kritieke ziekte of hartchirurgie die naar verwachting opname op een intensive care-afdeling vereist
- Gastro-intestinale obstructie, adynamische ileus of gastro-intestinale afzuiging vereist
- Medische of chirurgische patiënten zullen naar verwachting >24-48 uur NPO worden gehouden na opname of na voltooiing van de chirurgische ingreep
- Verminderde nierfunctie (eGFR <30 ml/min)
- Huidige behandeling met orale of injecteerbare corticosteroïden
- Geestelijke toestand waardoor de proefpersoon de aard en reikwijdte van het onderzoek niet kan begrijpen
- Vrouwelijke proefpersonen die zwanger zijn of borstvoeding geven op het moment van deelname aan het onderzoek
- Nieuwe of recente aanvang (binnen twee weken) van infectie met coronavirusziekte 2019 (COVID-19) op het moment van opname

Deelnemers worden overgezet naar het voorkeursmiddel binnen de categorie medicatie die ze thuis gebruiken.

De OAD wordt gehouden als de deelnemer op strikte nil per os (NPO) is geplaatst en na inschrijving geen orale medicatie kan innemen.

The Dexcom CGM is a small sensor that inserts just under the skin to continuously monitor glucose levels. Results are transmitted to the wearer's smartphone every five minutes.
