Proef van antimicrobiële terughoudendheid bij vermoedelijke longontsteking (TARPP)

Bij deze studie zullen 8 centra betrokken zijn om het effect te bepalen van het uitstellen van antimicrobiële initiatie totdat objectieve microbiologische gegevens zijn verkregen bij patiënten met vermoedelijk op de IC opgelopen pneumonie zonder septische shock. Het onderzoeksteam vergelijkt twee opeenvolgende periodes van 4 maanden in elke eenheid, een 'agressieve' antimicrobiële initiatieperiode en een 'conservatieve' antimicrobiële initiatieperiode.
Deze studie zal de volgende specifieke doelen dienen:
Specifiek doel 1: Het bepalen van de haalbaarheid van een grotere multicenter studie met dezelfde opzet en het beoordelen van de naleving van het protocol in meerdere centra.
Specifiek doel 2: Prospectief de mortaliteit in het ziekenhuis, ongeacht de oorzaak, bepalen voor alle patiënten met verdenking op pneumonie die werden behandeld volgens een agressief of conservatief antimicrobieel initiatieprotocol.
Specifiek doel 3: Prospectief bepalen van de initiatiepercentages van antimicrobiële stoffen, het totale aantal dagen van toediening van antimicrobiële stoffen, de duur van het verblijf in het ziekenhuis en op de intensive care en het aantal dagen zonder beademing voor patiënten die onder elk protocol worden behandeld.
Specifiek doel 4: Artsen die aan het onderzoek hebben deelgenomen, ondervragen om hun gevoelens over het onderzoek te beoordelen, waaronder de mate van comfort bij het agressief starten van antimicrobiële middelen, het niveau van comfort bij het achterhouden van antimicrobiële middelen tot definitief bewijs van infectie, waargenomen naleving van het protocol, waargenomen belang van het onderzoek en bereidheid om deel te nemen aan andere studies in zijn soort (uit te voeren na afsluiting van het klinische gedeelte van de studie).

Beschrijving
Inclusiecriteria:
- Geïntubeerde patiënten die zijn opgenomen op een chirurgische of trauma-intensive care-afdeling en die een geschikte kwantitatieve of semi-kwantitatieve endobronchiale sputumkweek hebben gehad, zijn ≥48 uur na hun IC-opname opgestuurd
- Primaire pathologie beheerd door chirurgisch specialisme
- Leeftijd ≥18 jaar.
Uitsluitingscriteria:
- Niet-geïntubeerde patiënten.
- Geïntubeerde patiënten die zich zorgen maken over een actieve infectie waarvan niet wordt vermoed dat het een longontsteking is (d.w.z. intra-abdominale infectie, huid- en weke delen infectie, urineweginfectie, etc.)
- Primaire ziekte niet chirurgisch of traumatisch van aard
- Primaire diagnose van brandwonden
- Gevangenisstatus
- Zwangere status of bevalling tijdens deze ziekenhuisopname.
- Op actieve immunosuppressiva (of innemen als thuismedicatie voor aankomst)

Ander: Agressieve armen
Als wordt vermoed dat een geïntubeerde patiënt tijdens de agressieve periode een op de intensive care verworven HAP/VAP heeft, moeten antimicrobiële middelen onmiddellijk worden gestart nadat kwantitatieve of semi-kwantitatieve endobronchiale kweken zijn verzonden, ongeacht de klinische status.
Hiertoe behoren ook patiënten die, zoals vastgesteld door de behandelend intensivist, in sepsis of septische shock verkeren.
Als de kweken en andere klinische gegevens na 72 uur niet wijzen op een longontsteking, moeten de antimicrobiële middelen worden stopgezet als er geen andere infectiebron is.

Ander: Conservatieve arm
Als wordt vermoed dat een patiënt tijdens de conservatieve periode een op de intensive care verworven HAP/VAP heeft, moeten kwantitatieve of semi-kwantitatieve endobronchiale kweken worden verzonden.
Als de patiënt in septische shock verkeert, aanhoudende hypotensie vereist waarvoor vasoactieve medicatie nodig is om de gemiddelde arteriële druk (MAP) ≥65 mm HG te handhaven of aanhoudende lactaatacidose (>2 mmol/l) ondanks adequate reanimatie), zullen er onmiddellijk antimicrobiële middelen worden gestart.
Als bij de patiënt een nieuwe orgaandisfunctie optreedt waarvan wordt aangenomen dat deze het gevolg is van een infectie (sepsis), zullen antimicrobiële middelen worden gestart, naar goeddunken van de behandelende intensivist.
Als er geen sprake is van septische shock of sepsis (intensivistische discretie), zullen er geen antimicrobiële middelen worden ingezet, tenzij objectief bewijs van longontsteking aanwezig is of een andere gedocumenteerde bron van infectie wordt geïdentificeerd die behandeling met antimicrobiële middelen verplicht stelt.
