Het doel / doel van deze studie is om de rol van probiotica bij de behandeling van NAFLD te evalueren door middel van fibroscan bij zowel nieuw gediagnosticeerde als bekende pediatrische patiënten met NAFLD. Dit wordt gedaan door tijdens elk bezoek een fibroscan uit te voeren en de ALT en het fecale microbioom van de patiënt met verschillende tussenpozen te vergelijken.
Het primaire eindpunt is het beoordelen van het effect van probiotische behandeling bij leversteatose en fibrose door beoordeling met fibroscan met behulp van CAP-score en TE-stadiëring. Hoe hoger de CAP-score (S1, S2, S3) hoe erger de steatose. TE-stadiëring (F0-F4), hoger de stadiëring duidt op verslechtering van fibrose.
Secundaire eindpunten zijn onder meer:
- De mate van afname van ALAT over een tijdsinterval van 3 maanden en gedurende het hele onderzoek vanaf de eerste diagnose.
- De mate van afname van leversteatose en BMI
Deze studie zal gericht zijn op pediatrische patiënten met NAFLD. Deelnemers zijn nieuwe en eerder gediagnosticeerde NAFLD-patiënten in de leeftijd van 5 jaar tot 18 jaar.
Dit is een studie in één centrum. Onderzoekers zullen patiënten inschrijven van de afdeling Pediatrische gastro-enterologie, leverziekte en voeding.
Deelnemers worden blind gerandomiseerd in een van de twee groepen: probioticum of placebo.
Onderzoekers verwachten dat de studie 1 jaar zal lopen van 15/09/2020 tot 14/09/2021. Onderzoekers zullen patiënten gedurende een periode van 6 maanden inschrijven. Na randomisatie in twee afzonderlijke groepen (probioticum versus placebo), zijn de onderzoekers van plan gegevens te verzamelen voor de daaropvolgende 6 maanden na inschrijving.

Inclusiecriteria:
- Deze studie zal gericht zijn op pediatrische patiënten met NAFLD. Deelnemers zijn nieuwe en eerder gediagnosticeerde NAFLD-patiënten in de leeftijd van 5 jaar tot 18 jaar. De diagnose moet worden gesteld aan de hand van een verhoogd ALAT-serum (tweemaal boven de geslachtsspecifieke bovengrens van normaal) en een echografie die consistent is met steatose
Uitsluitingscriteria:
Uitsluitingscriteria voor beide groepen
- Gelijktijdige onderliggende leverziekte zoals maar niet beperkt tot auto-immuunhepatitis
- Gelijktijdige infectieuze hepatitis
- Medicatiegebruik van steroïden, methotrexaat, metformine en therapeutische dosis Vit. E
- Recent antibioticagebruik in de afgelopen 4 weken
- Klinisch significant gewichtsverlies (minstens 5% gewichtsafname ten opzichte van het uitgangsniveau [18]) bij follow-up NAFLD-patiënten die een leefstijlaanpassingsinterventie ondergingen
- Cirrose (Fibroscan-score ≥14,0)>
