Inbrengbare, door een hartmonitor geleide vroege interventie om atriumfibrillatiebelasting na katheterablatie te verminderen (ICM REDUCE-AF)
Om prospectief de werkzaamheid te onderzoeken van een door een implanteerbare hartmonitor geleide behandeling van atriumfibrilleren (AF) bij het verminderen van de daaropvolgende AF-last bij patiënten met aanhoudend of paroxismaal AF die atriale katheterablatie (CA) ondergaan.

In deze studie zullen 120 proefpersonen van de inschrijvingssite van de Universiteit van Rochester gerandomiseerd worden (1:1) naar conventionele AF-behandeling vs.
Abbott ICM-geleide AF-behandeling na ablatie voor aanhoudende AF.
De proefpersonen worden gedurende 15 maanden gevolgd, inclusief een blankingperiode van 3 maanden na AF-ablatie.
De proefpersonenpopulatie omvat proefpersonen met paroxismaal atriumfibrilleren (volgens de definitie van de HRS-richtlijnen van 2019: een episode van AF die spontaan of met interventie in minder dan zeven dagen eindigt) of aanhoudend atriumfibrilleren (aanhoudende AF-episode die langer dan 7 dagen duurt, maar minder dan 1 jaar), volgens de huidige richtlijnindicaties voor persisterende AF-ablatie en Abbott ICM-implantatie.
De Abbott ICM bevat een mobiele app (myMerlin) die vroege detectie van AF-recidief mogelijk maakt door middel van door de patiënt getriggerde externe transmissies en correlatie met symptomen op basis van subklinische AF (SCAF) en door de patiënt getriggerde externe transmissies.
Toekomstige door de FDA goedgekeurde ICM-apparaten van Abbott die dezelfde functionaliteit gebruiken, kunnen ook in dit onderzoek worden gebruikt.
Dit is een fase 4-onderzoek en we vergelijken twee managementstrategieën die momenteel in de klinische praktijk worden gebruikt.

Deelname Criteria
Onderzoekers zoeken naar mensen die aan een bepaalde beschrijving voldoen, de zogenaamde geschiktheidscriteria. Enkele voorbeelden van deze criteria zijn iemands algemene gezondheidstoestand of eerdere behandelingen.

Inclusiecriteria:
- Geschiedenis van paroxismaal atriumfibrilleren (volgens de definitie van de HRS-richtlijnen van 2019: een episode van AF die spontaan of met interventie in minder dan zeven dagen eindigt), volgens de huidige richtlijnindicaties voor paroxismaal AF CA (Klasse I/II); of patiënten met een voorgeschiedenis van aanhoudend boezemfibrilleren (volgens de definitie van de HRS-richtlijnen van 2019: aanhoudende AF-episode die langer dan 7 dagen, maar minder dan 1 jaar duurt), volgens de huidige richtlijn indicaties voor aanhoudende AF CA (Klasse I/IIIa) op enig moment in de verleden.
- 18 jaar of ouder op het moment van toestemming
- CA uit te voeren voor AF als standaardzorg binnen 2 kalendermaanden na toestemming en vóór randomisatiedatum
- Abbott ICM-apparaat geïndiceerd voor het monitoren van symptomen na CA als standaardzorg en geplaatst binnen 2 kalendermaanden na toestemming en vóór de randomisatiedatum
- Patiënten met een bestaande CIED (ICD/CRTD/PPM/ICM) zijn toegestaan als er een atriale lead is naar track AF.
Uitsluitingscriteria:
- Onvermogen of onwil om CA te ondergaan (bijv. aanwezigheid van intracardiale trombus, contra-indicatie voor antistolling of andere contra-indicatie voor CA)
- Onvermogen of onwil om het Abbott ICM-apparaat in te brengen
- Onvermogen om enige AAD-therapie te tolereren
- Permanent atriumfibrilleren dat meer dan 3 jaar aanhoudt voorafgaand aan de datum van toestemming
- NYHA klasse IV congestief hartfalen
- Levensverwachting <1 jaar na toestemmingsdatum voor elke medische aandoening
- Zwangerschap of borstvoeding
- Onwil om te voldoen aan alle post-procedurele vervolgvereisten en om geïnformeerde toestemming te ondertekenen
- Deelname aan ander interventioneel onderzoek (waarnemingsregisters zijn toegestaan).

Implanteerbaar apparaat (ICM/CIED) dat zorgt voor nauwkeurige dagelijkse overdracht van elektrische cardiale gegevens voor aritmiedetectie.
Patiënten zonder een bestaande CIED die liever niet-invasieve LT-ECG-monitoring gebruiken, zullen de mogelijkheid krijgen om de Carnation Ambulatory Monitor ("CAM™"; Bardy Diagnostics) te gebruiken.
De CAM™ is door een patch goedgekeurd voor LT-ECG-monitoring, aangebracht op de huid boven de borst gedurende een periode van 7 dagen elke 2 maanden voor elektrische cardiale gegevens voor aritmiedetectie.

Het primaire eindpunt van het onderzoek is de totale AF-last tussen 3 en 15 maanden na de index CA-procedure (d.w.z. meer dan 1 jaar na blanking), zoals gedetecteerd door een ICM die vóór of op het moment van de CA-procedure zal worden ingebracht , bestaande CIED- of niet-invasieve LT-ECG-monitor bij alle proefpersonen.
Herhaling van AF wordt gedefinieerd als elke episode van AF die langer dan 30 seconden duurt.
AF-last wordt gedefinieerd als de gemiddelde hoeveelheid tijd die in AF wordt doorgebracht gedurende de vooraf gespecificeerde tijdsperiode (exclusief korte AF-episodes van ≤30 seconden).

De vragenlijst heeft 20 vragen op een 7-punts Likertschaal die de symptomen, dagelijkse activiteiten en behandelingsproblemen beoordelen.
Scores variëren van 20-140, waarbij hogere scores wijzen op slechtere resultaten.
