카테터 절제 후 심방 세동 부담을 줄이기 위한 삽입형 심장 모니터 안내 조기 개입(ICM REDUCE-AF)
심방 카테터 절제술(CA)을 받는 지속성 또는 발작성 심방세동 환자의 후속 심방세동 부담을 줄이는 삽입형 심장 모니터 유도 심방세동(AF) 관리의 효능을 전향적으로 조사합니다.

상세 설명
이 연구에서는 University of Rochester 등록 사이트의 120명의 피험자가 기존의 AF 관리 대 AF 관리에 무작위(1:1) 배정됩니다.
영구 AF에 대한 절제 후 Abbott ICM 안내 AF 관리.
피험자는 AF 절제 후 3개월의 공백 기간을 포함하여 15개월 동안 추적될 것입니다.
연구 피험자 모집단에는 발작성 심방세동(2019 HRS 가이드라인 정의에 따라: 자발적으로 종료되거나 7일 미만의 개입으로 종료되는 심방세동 에피소드) 또는 지속성 심방세동(지속적인 심방세동이 7일 이상 지속되지만 1년 미만), 지속적인 AF 절제 및 Abbott ICM 이식에 대한 현재 지침 적응증에 따라.
Abbott ICM은 무증상 AF(SCAF) 및 환자 트리거 원격 전송에 기반한 증상과 환자 트리거 원격 전송 및 상관 관계를 통해 AF 재발을 조기에 감지할 수 있는 모바일 앱(myMerlin)을 통합합니다.
동일한 기능을 사용하는 향후 FDA 승인 Abbott ICM 장치도 이 연구에서 활용될 수 있습니다.

- 발작성 심방 세동의 병력(2019 HRS 가이드라인 정의에 따름: 자발적으로 종료되거나 7일 미만의 개입으로 종료되는 AF 에피소드), 발작성 AF CA(클래스 I/II)에 대한 현재 가이드라인 적응증에 따라; 또는 지속성 심방세동 병력이 있는 환자(2019 HRS 지침 정의에 따름: 지속성 AF 에피소드가 7일 이상 1년 미만 지속됨), 지속성 AF CA(클래스 I/IIIa)에 대한 현재 지침 적응증에 따라 과거.
- 동의 시점에 만 18세 이상
- 동의 후 2개월 이내 및 무작위화 날짜 이전에 표준 치료로 AF에 대해 CA를 수행해야 함
- CA 후 표준 치료로 증상을 모니터링하기 위해 표시된 Abbott ICM 장치는 동의 후 2개월 이내에 무작위 배정 날짜 이전에 삽입되었습니다.
- 기존 CIED(ICD/CRTD/PPM/ICM)가 있는 환자는 AF를 추적하기 위한 심방 리드가 있는 경우 허용됩니다.
제외 기준:
- CA를 받을 수 없거나 원하지 않는 경우(예: 심장 내 혈전의 존재, 항응고제에 대한 금기 또는 CA에 대한 기타 금기)
- Abbott ICM 장치 삽입을 할 수 없거나 원하지 않는 경우
- AAD 요법을 견딜 수 없음
- 동의일로부터 3년 이상 지속되는 영구적 심방세동
- NYHA 클래스 IV 울혈성 심부전
- 건강 상태에 대한 동의일로부터 1년 미만의 기대 수명
- 임신 또는 수유
- 모든 절차 후 후속 요구 사항을 준수하고 정보에 입각한 동의서에 서명하지 않으려 함
- 기타 중재적 연구 연구 참여(관찰 등록이 허용됨).
