Gedetailleerde beschrijving
Beleid inzake rookvrij wonen in woningen met meerdere woningen zijn veelbelovende en steeds vaker voorkomende interventies om roken en passief roken te verminderen. Weinig onderzoek heeft factoren geïdentificeerd die de naleving van het rookvrije huisvestingsbeleid in meergezinswoningen met een laag inkomen verbeteren en overeenkomstige oplossingen getest.
De voorgestelde gerandomiseerde gecontroleerde studie (RCT) pakt de belangrijkste hiaten in kennis aan en profiteert van belangrijke wetenschappelijke kansen door: 1) gebruik te maken van het federale mandaat om roken te verbieden in een volkshuisvestingssysteem dat meer dan voldoende groot is om een adequaat aangedreven RCT uit te voeren; 2) ons begrip van de naleving van het rookvrijbeleid uitbreiden tot buiten de beleidsimplementatie door twee nieuwe behandelingen te testen: a) stoppen met roken in de woning en b) goedkeuring door bewoners, terwijl 3) bevolkings- en locatiespecifieke tabaksgerelateerde verschillen aanpakken.
De onderzoekers veronderstellen dat de relocatie/stoppen plus bewoner-goedkeuringsinterventie significant grotere reducties in persoonlijk roken en passief roken zal opleveren, in vergelijking met op zichzelf staande interventies en de standaardbenadering.

Beschrijving
Criteria voor inclusie van gebouwen:
- NYCHA-gebouwen in Manhattan en de Bronx (gelijkmatig gestratificeerd per stadsdeel) met meer dan 50 eenheden, die geen ingrijpende renovatie ondergaan.
Uitsluitingscriteria voor gebouwen:
- Gebouwen in een wijk die niet in Manhattan of de Bronx ligt
- Gebouwen kleiner dan 50 eenheden
- Gebouwen die ingrijpende renovaties ondergaan
- Gebouwen die deel uitmaken of zullen uitmaken van Rental Assistance Demonstration (RAD) of Permanent Affordability Commitment Together (PACT)
- Gebouwen met gemengde financiën
- Gebouwen die exclusief voor ouderen zijn
- Gebouwen die particulier beheerd worden
- Gebouwen waar andere onderzoeken aan de gang zijn
Criteria voor deelname aan deelnemers:
- Rekruteer NYCHA-bewoners via deurkloppingen en onderscheppingen in de lobby totdat we ons beoogde aantal per groep hebben bereikt (4 rokers, 4 niet-rokers in elk gebouw).
- Bewoners worden gescreend op rookstatus. Er worden zowel rokers als niet-rokers geworven, maar er wordt slechts één deelnemer - roker of niet-roker - per appartement geworven.
- Roken wordt gedefinieerd als degenen die melden dat ze in de afgelopen maand ten minste 5 dagen een sigaret of een ander brandbaar tabaksproduct (bijv. kleine sigaar, cigarillo) hebben gebruikt. Niet-rokers zijn onder meer nooit-rokers en ex-rokers die 12 maanden geleden of eerder zijn gestopt.
- Aanvullende subsidiabiliteitscriteria zijn onder meer personen ouder dan 18 jaar die minimaal 5 dagen per week en 9 maanden per jaar in het gebouw wonen en niet van plan zijn om in de komende 2 jaar te verhuizen.
Criteria voor uitsluiting van deelnemers:
- Personen met ernstige lichamelijke of geestelijke medische aandoeningen (d.w.z. cognitieve handicap) of andere factoren die de deelname aan of het vermogen om geïnformeerde toestemming te geven aan het onderzoek bij baseline of tijdens follow-ups zouden kunnen beperken.
- Personen die deelnamen aan de eerdere focusgroepen.
- Personen die alleen niet-tabaksproducten roken (bijvoorbeeld marihuana).

Experimenteel: Naleving door reductie (verhuizing en reductie persoonlijk roken) en stoppen
Rokers zullen door het onderzoeksteam worden doorverwezen naar peer educators van een gemeenschapsorganisatie die is opgeleid om peer-to-peer gezondheidsvoorlichting te geven, inclusief ondersteuning bij het stoppen met roken.
De peer-educator coördineert ondersteuning bij het stoppen met roken, waaronder het dienen als contactpersoon tussen de deelnemer en het onderzoeksteam, het verstrekken van informatie over het rookvrij-beleid en mogelijkheden voor verhuizing, en het verbinden van de deelnemer met toegang tot tabaksvervangende therapie en/of ondersteuning door een arts, indien dit passend wordt geacht .

Experimenteel: Naleving door middel van goedkeuring door bewoners
De toegewezen gebouwen zullen het doelwit zijn van een reeks van 2 in-residence-programma's waarbij gemeenschapsfora en creatieve kunsten betrokken zijn om bewoners te overtuigen van rookvrije leefomgevingen.
Deze arm is gebaseerd op betrokkenheid van bewoners en probeert de sociale en fysieke dimensies van de woonomgeving te beïnvloeden om naleving te bereiken.
De sessies zullen: 1) bewoners informeren over de risico's van roken en passief roken; 2) redenen identificeren om een rookvrij huis te hebben, 3) bewoners vragen om een toezegging op papier en/of virtueel te ondertekenen; 4) plaats op deuren en/of social media-pagina's een rookvrij bord met een originele hashtag (#Smokefree[gebouwadres]); en 5) bewoners verwijzen naar de Smoke-free NYCHA-website voor informatie over het beleid en bestaande middelen om te stoppen.

Experimenteel: Naleving door verlaging/stopzetting plus instemming van de ingezetene (gecombineerd)
De gecombineerde interventie zal worden uitgevoerd in de gebouwen die zijn toegewezen aan deze RCT-arm, die in-residence-programma's zal bieden op basis van de bewoner-endossementbehandeling en de verhuis-/stopbehandeling met roken.
Beide zullen gelijktijdig plaatsvinden, waarbij de ene gericht is op alle bewoners van het gebouw (aanbeveling van de bewoners) en de andere op rokers (verhuizing/stoppen met roken) met als doel zowel persoonlijk roken als passief roken te verminderen.

Geen tussenkomst: Standaard NYCHA-benadering (controle)
Gebouwen en studiedeelnemers die aan deze arm zijn toegewezen, zullen worden gerekruteerd en gedurende een periode van 12 maanden worden gevolgd om de resultaten te beoordelen.
Er zullen geen aanvullende programma's of diensten worden geleverd aan de gebouwen of bewoners die aan deze arm zijn toegewezen, buiten de standaardprogramma's die NYCHA kan bieden ter ondersteuning van het rookvrije mandaat.
Buitendienstmedewerkers documenteren alle beleidsgerelateerde bewegwijzering, activiteiten of informatie waaraan deze deelnemers worden blootgesteld.
