Een aantal activiteiten in het dagelijks leven wordt gestaakt na een beroerte. Deze beëindigde activiteiten omvatten meestal elementaire zelfzorg, instrumentele activiteiten van het dagelijks leven, vrije tijd en werk. Deze veranderingen in participatie dragen bij aan een verminderde kwaliteit van leven en tevredenheid met het leven. De Cognitive Orientation to daily Occupational Performance (CO-OP)-benadering is een gedragsprobleemoplossende interventie die zich richt op het leren van cliënten hoe cognitieve strategieën kunnen worden toegepast om het leren te verbeteren en prestatiebelemmeringen te overwinnen.
CO-OP gebruikt zinvolle, door de klant gekozen activiteiten om het leren van nieuwe vaardigheden en neurale reorganisatie te stimuleren. In overeenstemming met deze principes van neuroplasticiteit, is transcraniële gelijkstroomstimulatie (tDCS) een niet-invasieve hersenstimulatiemethode voor het moduleren van corticale prikkelbaarheid door toepassing van lage stromen op gerichte gebieden op de hoofdhuid.
Er zijn bekende positieve effecten van CO-OP om de activiteitsprestaties te verbeteren en voor tDCS om de resultaten op het niveau van stoornissen na een beroerte te verbeteren. De onderzoekers veronderstellen dat de bekende effecten van CO-OP kunnen worden versterkt door het gebruik van tDCS om bekende neurale hubs van uitvoerende netwerken te veranderen die het oplossen van problemen ondersteunen. Kortom, (1) CO-OP is effectief in het verbeteren van activiteitsprestaties, en (2) de effecten van CO-OP kunnen worden versterkt door nieuwe methoden die het mogelijk maken om de hersenen in een verbeterde neuroplastische toestand te brengen, zoals tDCS.

Beschrijving
Inclusiecriteria:
- >6 maanden na ischemische beroerte
- ontvangt momenteel geen therapiediensten
- minimaal vier zelfbenoemde functionele doelen
Uitsluitingscriteria:
- ernstige depressieve symptomen (>20 op Patient Health Questionnaire)
- symptomen van dementie (<24 op Montreal Cognitive Assessment)
- eventuele aanvullende neurologische aandoeningen
- matige tot ernstige afasie (NIH Stroke Scale afasieschaal groter dan of gelijk aan 2)
- eventuele tDCS-contra-indicatie

Experimenteel: CO-OP en tDCS-groep
Elke sessie bestaat uit 20 minuten anodische tDCS toegepast op de linker dorsolaterale prefrontale cortex (DLPFC) bij 1,5 mA.
Dit wordt gevolgd door 45 minuten van de toegewezen CO-OP.

CO-OP richt zich op het leren van een wereldwijde probleemoplossende strategie, Goal-Plan-Do-Check (GPDC), binnen de uitvoering van door de deelnemer gekozen doelen.
Het gebruik van een breed toepasbare strategie en zinvolle activiteit zorgt ervoor dat de interventie opvallend is voor de deelnemer en vergroot de kans op overdracht.
Uniek is dat therapeuten die CO-OP toepassen, begeleide ontdekkingsmethoden gebruiken om deelnemers te ondersteunen bij het analyseren van hun eigen uitvoering van een bepaalde taak en het genereren van mogelijke oplossingen om de prestaties te verbeteren.
Deelnemers raken uitgerust met deze vaardigheden door herhaalde toepassing van het Goal-Plan-Do-Check-proces. De interventie richt zich op het leren van GPDC met geleidelijke terugtrekking van begeleide ontdekkingsmethoden.
Elke sessie begint met 20 minuten anodische tDCS toegepast op de ipsilesionale dorsolaterale prefrontale cortex (DLPFC) bij 1,5 mA.
De kathode wordt over het contralesionale supraorbitale gebied geplaatst.
Gelijkstroom wordt aangebracht via twee met zoutoplossing doordrenkte sponzen die op de hoofdhuid worden geplaatst.
Het tDCS-apparaat bevat software voor dubbelblinde toediening.
