Vergelijking van kwaliteit en kwantiteit van M-PRP Cellular Content Filgrastim vs. Pegfilgrastim (M-PRP)
Een prospectieve studie in één centrum om de kwaliteit en kwantiteit van de cellulaire inhoud van M-PRP te vergelijken die is geoogst na perifere mobilisatie van progenitorcellen met behulp van filgrastim versus pegfilgrastim
Het doel van deze prospectieve, observationele studie is het vergelijken van de kwaliteit en kwantiteit van de cellulaire inhoud van bloedplaatjesrijk plasma dat wordt geoogst na toediening van een van de twee celstimulerende eiwitten, filgrastim en pegfilgrastim. De belangrijkste vraag die het wil beantwoorden is:
• Zullen deelnemers een vergelijkbare cellulaire inhoud hebben als ze een 4-daagse behandeling met filgrastim vergelijken met een eendaagse behandeling met pegfilgrastim?
Deelnemers krijgen de volgende interventie toegediend:
- Er wordt 130 ml bloed afgenomen bij het eerste bezoek na toestemming en bij vervolgbezoeken na toediening van de behandeling (4 dagen voor filgrastim, 7 dagen voor pegfilgrastim)
- De helft van alle deelnemers krijgt eerst filgrastim, gevolgd door pegfilgrastim 8 weken na beëindiging van de behandeling met filgrastim. De andere helft krijgt de behandelingen in omgekeerde volgorde
Onderzoekers zullen de kwaliteit en kwantiteit van de celinhoud na elke behandelingstoediening vergelijken en verschillen in gegevens vergelijken, afhankelijk van de volgorde waarin de behandeling werd gegeven.

De voorgestelde studie is een prospectieve, gerandomiseerde gecontroleerde, single-center laboratoriumstudie met 10 gezonde vrijwilligers. Zodra de potentiële deelnemer de screening heeft doorstaan, heeft ingestemd met de onderzoeksprocedures, het medische interview heeft voltooid en bloedonderzoek in het laboratorium heeft ondergaan, ondergaat de proefpersoon twee opeenvolgende mobilisatie-evenementen. De planning van de mobilisatie-evenementen zal worden gevarieerd over de 10 deelnemers om sequencing-effecten van de mobilisatie-evenementen tegen te gaan. Aan 5 gezonde donoren wordt gedurende 4 dagen een standaard mobilisatieregime van filgrastim van 10 mcg/kg per dag toegediend. Dit wordt gevolgd door een standaardregime voor mobilisatie van pegfilgrastim, bestaande uit één injectie van 6 mg met een tussenpoos van 8 weken voor 5 van de deelnemers. De andere 5 gezonde donoren krijgen de omgekeerde volgorde van het farmaceutische middel, eerst pegfilgrastim gevolgd door filgrastim.
Op de eerste dag van het onderzoek zal een eerste bloedafname van 130 ml worden uitgevoerd, die zal worden gebruikt om standaard PRP voor laboratoriumtests te maken en proefpersonen zullen beginnen met een filgrastim- of pegfilgrastim-doseringsreeks. Na de gespecificeerde tijd (4 dagen voor filgrastim en 7 dagen voor pegfilgrastim) wordt een tweede 130 ml bloed afgenomen en verwerkt met het Arthrex Angel-systeem om M-PRP te maken voor laboratoriumtests. De standaard celinhoud van PRP en M-PRP zal in vitro worden bestudeerd en gekwantificeerd met celtelling, celkweek en eiwitanalyse. 8 weken na de tweede bloedafname komen de proefpersonen terug voor een derde bloedafname van 130 ml, gevolgd door toediening van een tweede mobiliserend middel (pegfilgrastim of filgrastim). Na de aangegeven tijd (4 dagen voor filgrastim en 7 dagen voor pegfilgrastim) komen de patiënten terug voor een vierde bloedafname van 130 ml. Het monster wordt verwerkt met het Arthrex Angel-systeem om M-PRP te maken voor laboratoriumtests. De cellulaire inhoud van het M-PRP-product zal in vitro worden bestudeerd en gekwantificeerd met celtelling, celkweek en eiwitanalyse. Daarna zal de cellulaire inhoud van het M-PRP-product worden vergeleken tussen filgrastim en pegfilgrastim-mobilisatiemiddelen.

Beschrijving
Inclusiecriteria:
- Gezonde 19-39 jaar en gewicht 50-100kg
- Proefpersoon stemt ermee in om 5 opeenvolgende dagen te komen voor behandeling met filgrastim en extra bloedafname, 8 weken later twee extra bezoeken voor behandeling met pegfilgrastim en 7 dagen later bloedafname. Deze volgorde van toediening wordt aan de helft van de deelnemers verstrekt, terwijl de andere helft dezelfde behandelingen in omgekeerde volgorde krijgt.
Uitsluitingscriteria:
- Vrouwelijk
- Gewicht < 50 kg of > 100 kg
- Eerdere allergische reactie op filgrastim, PEG, lidocaïne, latex, acryl of een ander injecteerbaar verdovend middel
- Geschiedenis van suikerziekte
- Abdominale tederheid bij palpatie
- Onduidelijke longvelden bij lichamelijk onderzoek
- Splenomegalie
- Significante cardiovasculaire, nier-, lever- of longziekte
- Aantal witte bloedcellen (WBC) meer dan 20.000/microliter (mcL) bij initiële screening van volledig bloedbeeld (CBC)
- Bloedaandoeningen, auto-immuunziekten, aandoeningen die immunosuppressie vereisen, kanker, een aanhoudende infectieziekte, sikkelcelziekte of andere bloedaandoeningen.
