(2R,6R)-Hydroxynorketamine voor de behandeling van neuropathische pijn (HNK)
(2R,6R)-Hydroxynorketamine een nieuw therapeutisch analgeticum voor de behandeling van neuropathische pijn: een gerandomiseerde dubbelblinde cross-over-studie.
Het doel van deze gerandomiseerde dubbelblinde drieweg (1:1:1) cross-over klinische studie is het evalueren van de effectiviteit en duur van analgesie van een enkele infusie van (2R,6R)-HNK 0,5 mg/kg vergeleken met ketamine 0,5 mg / kg en zoutoplossing met een interval van 5 weken tussen behandelingen op pijn, pijnkwaliteiten, fysiek functioneren, pijninterferentie, slaapstoornissen en kwaliteit van leven bij proefpersonen met neuropathische pijn van de extremiteiten.
De vragen die dit onderzoek zal beantwoorden zijn:
- Wat is de analgetische werkzaamheid van (2R,6R)-HNK op pijnintensiteit en pijnkwaliteiten bij patiënten met chronische (>3 maanden) neuropathische pijn (NP).
- Wat is de effectieve duur van een enkele infusie van (2R,6R)-HNK bij patiënten met NP.
- Zal (2R,6R)-HNK pijngerelateerde effecten verminderen, waaronder interferentie met dagelijkse activiteiten van het leven, slaapstoornissen en de door patiënten gerapporteerde pijnkwaliteiten veranderen.
Deelnemers ontvangen elk van de drie onderzoeksgeneesmiddelen in willekeurige volgorde met tussenpozen van 5 weken gedurende een periode van 15 weken. Het medicijn wordt toegediend als een infuus van 45 minuten.
Deelnemers zullen kwantitatieve sensorische en pijnevaluaties voltooien en de door de patiënt gerapporteerde pijnuitkomsten voltooien voorafgaand aan het ontvangen van het eerste onderzoeksgeneesmiddel en op 7, 14 en 21 en 35 dagen na toediening van het onderzoeksgeneesmiddel.

Beschrijving
Inclusiecriteria:
- Volwassen patiënten (18 tot 80 jaar) met een vastgestelde diagnose van chronische (> 3 maanden) NP van de extremiteiten.
- Aanwezigheid van NP zoals bepaald bij screening met behulp van de 10-item Neuropathic Pain Questionnaire (DN4), met een score van ≥4 vereist voor opname in de studie.
- Mogelijkheid om voldoende Engels te lezen en te schrijven om studiegerelateerde procedures te voltooien.
- Een body mass index (BMI) (gewicht [kg]/lengte [m ]) tussen 18 en 35 kg/m (inclusief) en een gewicht tussen 50 kg en 120 kg (110 - 264 pond).
- De bloeddruk van de patiënt ligt ongeveer 5 minuten in rugligging tussen 90 en 145 mmHg systolisch en niet hoger dan 90 mmHg diastolisch bij aanvang.
- Een 12-afleidingen ECG zonder klinisch significante afwijking zoals beoordeeld door de onderzoeker en QTc-interval ≤ 450 milliseconden bij baseline.
- Polsslag in rust tussen 45 en 100 slagen per minuut.
- Klinische laboratoriumbevindingen en leverfunctietesten binnen het normale bereik, of indien buiten het normale bereik, naar de mening van de PI als niet klinisch significant beschouwd.
- Ga ermee akkoord om schriftelijke geïnformeerde toestemming te geven en te voldoen aan de regels met betrekking tot de consumptie van alcohol, cafeïnehoudende dranken en tabaks-/nicotineproducten tijdens het onderzoek.
- Patiënten kunnen geplande of zo nodig medicijnen gebruiken voor hun chronische neuropathische pijn en stemmen ermee in om de geplande medicijnen gedurende de onderzoeksperiode te blijven gebruiken.
- Als het onderwerp pijnverlichting ervaart, kunnen ze ervoor kiezen om niet de benodigde medicijnen in te nemen.
Uitsluitingscriteria:
- Proefpersonen met vermoedelijke verhoogde intracraniale of intraoculaire druk.
- Proefpersonen die eerder ketamine hebben gekregen voor de behandeling van een chronische pijndiagnose.
- Eerdere of huidige deelname aan een klinische studie met een onderzoeksgeneesmiddel, apparaat of biologisch geneesmiddel binnen 30 dagen.
- Proefpersonen met ernstige medische aandoeningen, waaronder (maar niet beperkt tot) hepatische, cardiovasculaire, long-, nier-, hematologische, endocriene, gastro-intestinale, immunologische, dermatologische, neurologische, oncologische of psychiatrische aandoeningen die naar de mening van de PI de veiligheid van de onderwerp of de validiteit van de studieresultaten.
- Klinisch significante acute ziekte in de 2 weken voorafgaand aan de dosering.
- Onvermogen om effectief te communiceren met onderzoekspersoneel.
- Proefpersonen met een bekende leverziekte.
- Wijdverbreide pijn of een diagnose van fibromyalgie.
- Huidige diagnose van geestesziekte.
- Zwangerschap.
- Allergie voor ketamine of een studiemedicijn.
- Consumptie van dranken of voedsel dat alcohol, grapefruit, maanzaad, spruitjes, granaatappel, broccoli, gegrild vlees bevat binnen 2 dagen voorafgaand aan de toediening van het geneesmiddel.
- Gebruik van tabak of nicotinebevattende producten binnen 4 weken voorafgaand aan de toediening van het geneesmiddel.
- Slechte perifere veneuze toegang.
- Proefpersonen mogen naar het oordeel van de PI niet deelnemen aan het onderzoek.
