Afasie is een taalstoornis, voornamelijk veroorzaakt door hersenletsel aan de linker hersenhelft. Afasiebehandelingen omvatten spraak- en taaltherapie en farmacologische therapie, maar uit verschillende onderzoeken is gebleken dat deze behandelingen niet volledig effectief zijn bij patiënten met afasie, waardoor ze met resterende tekorten achterblijven die de kosten van beroerte-gerelateerde zorg aanzienlijk verhogen. Bovendien kan de hoeveelheid en frequentie van de geleverde spraak- en taaltherapie een cruciaal effect hebben op het herstel. Daarom is er behoefte aan nieuwe behandelingen of aanvullingen op bestaande behandelingen, zoals hersenstimulatie-interventies, die het potentieel hebben om grotere verbeteringen te laten zien bij patiënten met afasie. Eén van deze nieuwe benaderingen voor niet-invasieve hersenstimulatie is transcraniële gelijkstroomstimulatie (tDCS).
Deze studie zal de effecten van tDCS tijdens logopedie onderzoeken om verder te onderzoeken welke methode of methoden het beste is voor het herstel van de patiënt. Patiënten die aan het onderzoek deelnemen, zullen taaltesten ondergaan die een breed scala aan taalfuncties bestrijken. Functionele Magnetische Resonantie Beeldvorming (fMRI) zal voor en na logopedische interventiearmen worden uitgevoerd om de neurale processen te onderzoeken die worden beïnvloed door tDCS en logopedie.
Studieontwerp:
Patiënten worden willekeurig toegewezen aan een van de twee logopediegroepen en willekeurig toegewezen aan een van de twee stimulatiegroepen (actieve anodale stimulatie of schijntherapie). Patiënten krijgen een van de twee verschillende logopedische behandelingsinterventies om zich te concentreren op specifieke verwerkingstekorten. Deelnemers ondergaan neuropsychologische evaluatie en fMRI-beoordeling voordat ze gerichte anodale-tDCS of schijn-tDCS ontvangen voor 10 therapiesessies. Deelnemers worden vervolgens opnieuw getest met behulp van de gedragsbeoordelingsmaatregel en fMRI voordat ze oversteken om opnieuw de tDCS-interventie te ontvangen, samen met dezelfde logopedische arm. Ze zullen een gedragsbeoordeling en fMRI voltooien 3 maanden na behandeling 2 en een definitieve gedragsbeoordeling 6 maanden na behandeling 2.

Beschrijving
Inclusiecriteria:
- Patiënten moeten 18 jaar of ouder zijn.
Patiënten mogen niet ouder zijn dan 85 jaar.
Patiënten moeten een taalachterstand hebben als gevolg van linkszijdige focale neurologische schade (bijv. beroerte, tumor).
Patiënten moeten volwassenen zijn en vloeiend Engels spreken.
Patiënten moeten in aanmerking komen voor een MRI.
Naast het voldoen aan de inclusiecriteria, wordt er geen voorkeur gegeven op basis van ras, etniciteit of geslacht.
Uitsluitingscriteria:
- Geavanceerde neurodegeneratieve ziekte (d.w.z. Fase 3 van de ziekte van Alzheimer)
Neurologische ziekte (bijv. idiopathische epilepsie, epileptische aanvallen die niet goed worden behandeld, de ziekte van Parkinson, ALS),
Ernstige psychopathologie (bijv. schizofrenie, bipolaire stoornis, acute depressieve episode)
Vermoedelijke of gediagnosticeerde niet-corrigeerbare gehoor- of gezichtsproblemen, of ontwikkelingsstoornissen (d.w.z. verstandelijke beperking of leerstoornis).
Contra-indicaties voor MRI zoals claustrofobie, geïmplanteerde elektronische apparaten, MRI-incompatibel metaal in het lichaam, extreme obesitas, zwangerschap, onvermogen om plat te liggen, onvermogen om stimulusmateriaal te zien of te horen.
Jonger dan 18 jaar of ouder dan 85 jaar.
< 6 maanden na tumorresectie.

Experimenteel: HD-tDCS
Deelnemers worden gerandomiseerd om anodale HD-tDCS of schijn-tDCS te ontvangen.

High-Definition-tDCS wordt geleverd via een batterijaangedreven constante gelijkstroomstimulator (Soterix) met behulp van een 4x1 montage (1 centrale anodale elektrode en 4 kathodale elektroden) gerangschikt in een HD-kap.
Anodale of schijn-tDCS zal worden toegediend.
