Studie van alveolair botbehoud met behulp van fotobiomodulatie gecombineerd met enten met scaffold-biomateriaal
Evaluatie van alveolair botbehoud na molaire extractie met behulp van fotobiomodulatie gecombineerd met enten met scaffold-biomateriaal: klinische, gerandomiseerde, drievoudig blinde studie.

Klinisch, gerandomiseerd, drievoudig blind, drievoudig blind onderzoek (de patiënt zal niet weten of hij de laserbehandeling heeft ondergaan of niet is geënt met het biomateriaal, en de operator die de laser zal toepassen zal ook blind zijn voor het apparaat met of zonder laserbestraling.
Er zullen 60 deelnemers worden gerekruteerd om de tandheelkundige kliniek van de Universidade Nove de Julho bij te wonen, in de cursussen chirurgie, endodontie en kindertandheelkunde, evenals in een privékantoor, met een indicatie voor het verwijderen van de eerste en tweede boven- en onderkiezen . Deze aanwerving zal enkel gebeuren na goedkeuring door de ethische commissie.
4.3 Steekproefgrootte: De steekproefomvang werd bepaald op basis van de primaire uitkomst van het onderzoek: alveolaire conservering met scaffold-biomateriaal. Gebaseerd op gegevens van Rosero et al. (Fotobiomodulatie als adjuvante therapie voor alveolaire conservering: een voorstudie bij mensen) en Araújo et al. (Ridveranderingen na implantatie van nieuwe extractiedozen bij de mens. Een gerandomiseerde klinische studie) Onze aanvankelijke schatting van de steekproefomvang was 9 proefpersonen per groep voor een significant niveau van 0,05 en een geschatte testkracht van 80% voor referenties 104 en 105. Om rekening te houden met de mogelijke niet-parametrische verdeling van de gegevens, zouden 15% meer proefpersonen aan elke groep moeten worden toegevoegd. Er zal nog eens 25% worden toegevoegd om rekening te houden met mogelijke uitval, wat resulteert in 13 deelnemers per groep en voor een grotere veiligheid en betrouwbaarheid werden in elke groep 15 patiënten vastgesteld. Voor het uitvoeren van de berekeningen is G*Power 3.1.9.6 gebruikt.
Materialen gebruikt in het onderzoek:
Het voor dit onderzoek gekozen biomateriaal is het Plenum® OSShp en Plenum® Guide (Brazilië) allograft. Het zijn volledig synthetische bottransplantaten en resorbeerbare regenererende membranen (vrij van dierlijke en menselijke oorsprong).
Plenum® OSShp is een poreuze synthetische bifasische biokeramiek (hydroxyapatiet: β - tricalciumfosfaat, HA: β-TCP - 70:30%), en de korrels zijn 500 - 1000 µm.
Plenum® Guide is een op polydioxanon (PDO) gebaseerd synthetisch membraanmateriaal waarvan de oppervlaktemorfologie de extracellulaire matrix nauw nabootst.
Randomisatie:
Alle deelnemers worden uitgenodigd om deel te nemen aan het onderzoek en worden na ondertekening van de geïnformeerde toestemming gerandomiseerd in blokken van 4; 60 deelnemers worden willekeurig verdeeld in 4 groepen n=15 Exo (extractie), Exo+Laser (extractie en behandeling met 808 nm laser), Exo+Biomat (extractie met biomateriaaltransplantaat), Exo+Biomat+Laser (extractie en enting van biomateriaal en 808 nm laserbehandeling). Deelnemers worden willekeurig toegewezen aan de hand van een computergegenereerde lijst (Excel). 4.7 KLINISCHE PROCEDURE: Op het preoperatieve moment wordt de patiënt onderworpen aan een CT-scan (Figuur 12) en een intra-orale scan (Figuur 13) voor de initiële meting van de verticale en horizontale botafmetingen van de alveolus en tussen de aangrenzende tanden van de tand die moet worden getrokken op zijn plaats in de alveolus en tandboog, en chirurgische planning, zoals aanbevolen door de beste huidige tandartspraktijken, en zal de autorisatietermijn voor beeldgebruik ondertekenen ( BIJLAGE 4). Pre-operatieve begeleiding en aflevering van recepten volgens het vastgestelde medicatieprotocol zullen ook worden uitgevoerd. Alle onderzoekspatiënten krijgen dezelfde postoperatieve begeleidingszorg (BIJLAGE 3) en hetzelfde medicatieprotocol dat wordt toegepast voor alle tandverwijderingsoperaties met of zonder transplantatie met biomaterialen. De tand wordt getrokken met behulp van een odontosectietechniek om de wortels met zo min mogelijk trauma te verwijderen. Het zal niet nodig zijn om de zachte weefselflap op te tillen vanwege de eigenschappen van het membraan dat zal worden gebruikt, waardoor weefseltrauma wordt verminderd en het herstel van de operatieplaats wordt bevorderd. Na de extractie wordt het biomateriaal toegevoegd en in de koker ondergebracht met instrumenten die dit inbrengen mogelijk maken en afgedekt door een membraan dat zal dienen als een mechanische barrière, waardoor de invaginatie van het tandvleesweefsel en de vorming van verhoornd weefsel wordt voorkomen om de operatiekamer af te sluiten. wond om de osseo-integratie van het botbiomateriaal niet in gevaar te brengen. geënt.
De Exo+Laser- en Exo+Biomat+Laser-groepen zullen, naast het geneesmiddelenprotocol en de richtlijnen, een laserbehandeling krijgen. De Exo- en Exo+Biomat-groepen zullen een soortgelijk laserapparaat gebruiken, maar dat zal geen straling uitzenden.
In de postoperatieve periode na 3 en 6 maanden zullen nieuwe intraorale digitale scanning- en digitale computertomografie-onderzoeken worden uitgevoerd om beelden te maken en volumetrische analyses uit te voeren met specifieke software.

Inclusiecriteria:
- Alle geslachten, ouder dan 18 jaar, zonder comorbiditeiten, vertonen een bevredigende mondhygiëne en vertonen een aandoening van verlies van eerste of tweede blijvende kiezen
Uitsluitingscriteria:
- Gewoonlijk gebruik van pijnstillende en ontstekingsremmende medicijnen voor andere comorbiditeiten, bestaande allergieën voor het voorgestelde medicijnprotocol, bij neurologische/psychiatrische behandelingen, aanwezigheid van tanden met laesies die moeten worden behandeld in dezelfde hemiarch van de te verwijderen tand, huidige rookgewoonte, zwanger bent of borstvoeding geeft, met lichtgevoeligheidsstoornissen, met de aanwezigheid van neoplasie in de orale regio; en patiënten met operatieve complicaties bij wie de chirurgische tijd langer duurt dan 90 minuten

Chirurgische interventiefase

Het computertomografieonderzoek zal vóór de operatie worden uitgevoerd, met de tand vóór extractie en na 3 en 6 maanden na de operatie. De gekozen metingen voor de dimensionale meting van het alveolaire proces zullen betrekking hebben op de buccolinguale dikte van de alveolus.
Bij de orthoradiale snede worden de grenzen van de alveolus begrensd op basis van de tandtop tot de maximale hoogte van de botkammen.
In de meest centrale positie wordt de verticale meting uitgevoerd en worden er 3 buccolinguale sneden gemaakt, proportioneel aan elkaar volgens de gemeten verticale meting; en in elk apicaal, midden en cervicaal derde deel zullen metingen worden uitgevoerd met betrekking tot de horizontale afmetingen van de alveolus.
Bij het intra-orale scanonderzoek worden de mesio-distale metingen gemeten vanaf de mesiale botkam tot de distale botkam, en de buccaal-linguale metingen in het meest centrale deel van de tandkas.
