Beschrijving
Inclusiecriteria:
Een depressieve episode volgens de criteria van het Diagnostic and Statistical Manual of Mental Disorders (DSM-5) tijdens het beloop van MDD met:
- HDRS-score > 17
- Leeftijd 18-65 jaar;
- Bij behandeling met TSD+LT
- Ondertekende geïnformeerde toestemming, in staat de nationale taal te begrijpen, spreken en schrijven
Uitsluitingscriteria:
- Voorgeschiedenis van bipolaire stoornis, schizofrenie, schizoaffectieve stoornis, psychose niet anders gespecificeerd; anorexia of boulimia nervosa;
- Gebruik van de volgende medicijnen: antipsychotica, anticonvulsiva, stemmingsstabilisatoren; stimulerende middelen
- Actieve infectie waarvoor antibioticatherapie nodig is;
- Immunosuppressieve patiënt of andere chronische ziekten
- Tekenen van een actieve infectie die behandeling vereist
- Regelmatig gebruik van ontstekingsremmende medicijnen bij een chronische ontstekings-/auto-immuunziekte. Verboden behandeling: corticosteroïden, niet-steroïde anti-inflammatoire geneesmiddelen, immunosuppressiva op basis van IV-Ig
- Aanhoudende koorts, infectie behandeld met antibiotica of ongecontroleerde diabetes type I of II;
- Bestaande kanker of een voorgeschiedenis van kanker in de afgelopen 5 jaar (behalve epidermoïde huidkanker of in-situ baarmoederhalskanker);
- Bekende HIV-infectie of klinisch manifest Acquired Immune Deficiency Syndrome (AIDS), de ziekte van Parkinson of Alzheimer, of enige andere ernstige aandoening die waarschijnlijk de uitvoering van het onderzoek zal verstoren;
- Misbruik van drugs of alcohol in de afgelopen 6 maanden. Andere uitsluitingscriteria met betrekking tot de MRI-procedure zijn onder meer:
- Aneurysma clip
- Geïmplanteerde neurale stimulator
- Geïmplanteerde pacemaker of auto-defibrillator
- Cochleair implantaat
- Oculair vreemd lichaam (bijvoorbeeld metaalspaanders)
- Elk geïmplanteerd apparaat (pompen, infuusapparaten, enz.)
- Verwondingen door granaatscherven

MDD-patiënten
60 patiënten met een depressieve episode in het beloop van een ernstige depressie (MDD) en behandeld met een chronobiologische interventie waaronder totale slaapdeprivatie (TSD) + lichttherapie (LT). Elke proefpersoon zal gedurende 6 maanden aan het onderzoek deelnemen, een MRI en een klinische evaluatie ondergaan die in totaal ongeveer 2 uur zal duren bij aanvang (V0), na een week chronobiologische behandeling en bij follow-up na 6 maanden. |

De respons op het totale slaaptekort zal worden gemeten (50% reductie op de Hamilton Depression Rating Scale (HDRS), min. score 0, max. 52; hogere scores, hogere ernst van de depressie) bij patiënten met C-reactieve proteïneniveaus <3 en >=3 mg/l .

Neurobiologische markers voor voorspelling en progressie van behandeling met antidepressiva

er zal een machine learning-algoritme worden geïmplementeerd om markers te identificeren voor het voorspellen van de respons op de behandeling (50% reductie in HDRS, min score 0 max 52; hogere scores hogere ernst van depressie) en ziekteprogressie (aantal recidieven en ernst van depressie na 6 maanden follow-up) gebaseerd op veranderingen in perifere ontstekingsmarkers en in hersenmetingen van neurale plasticiteit (bijv.
hersenniveaus van glutamaat en GABA; metingen van functionele connectiviteit in rusttoestand: amplitude van laagfrequente fluctuatie, regionale homogeniteit) en structurele integriteit (bijv.
hersenvolumes, microstructuur van witte stof - fractionele anisotropie).
