Het doel van deze klinische proef is om de veiligheid en werkzaamheid van het LAmbre Plus-apparaat aan te tonen bij patiënten met niet-valvulaire atriale fibrillatie in vergelijking met de in de handel verkrijgbare transkatheter LAAO-apparaten. De belangrijkste vragen die het wil beantwoorden zijn:
Algehele veiligheid van het apparaat die wordt beoordeeld over een periode van 12 maanden. Algemene werkzaamheid, nadat alle proefpersonen de periode van 18 maanden hebben bereikt.
Deelnemers worden willekeurig toegewezen aan het LAmbre-apparaat of andere door de FDA goedgekeurde LAAO-apparaten.

Beschrijving
Inclusiecriteria:
- De patiënt is een mannelijke of niet-zwangere vrouw van ≥18 jaar
- De patiënt heeft paroxysmaal, aanhoudend of permanent niet-valvulair atriumfibrilleren gedocumenteerd
- De patiënt heeft een CHADS2-score ≥ 2 of een CHA2DS2-VASc-score van ≥ 3 en wordt aanbevolen voor orale antistollingstherapie
- De arts acht de patiënt geschikt voor een kortdurende behandeling met warfarine, maar er is een passende reden om te zoeken naar een niet-farmacologisch alternatief voor orale antistolling.
- De patiënt wordt door een multidisciplinair hartteam, waarvan minimaal 1 onderzoeker (bijv. cardiologen) en 1 arts die niet betrokken was als onderdeel van het procedureteam en gebruik maakte van een gedeeld besluitvormingsproces, en deze vaststelling is gedocumenteerd in het medisch dossier van de patiënt
- De patiënt is bereid en in staat zich te houden aan de in het protocol gespecificeerde behandeling en vervolgevaluaties
- De patiënt (of zijn of haar wettelijke vertegenwoordiger) is op de hoogte gebracht van de aard van het onderzoek, gaat akkoord met de bepalingen ervan en heeft schriftelijke geïnformeerde toestemming gekregen, goedgekeurd door de betreffende Institutional Review Board (IRB) of Ethische Commissie (EC).
Uitsluitingscriteria:
- Zwangere of zogende patiënten en patiënten die een zwangerschap plannen in de periode tot 1 jaar na de indexprocedure. Vrouwelijke patiënten die zwanger kunnen worden, moeten binnen 7 dagen voorafgaand aan de indexprocedure een negatieve zwangerschapstest laten uitvoeren per standaardtest op de locatie.
- Patiënten met atriale fibrillatie die wordt gedefinieerd door een eenmalig voorval, of die van voorbijgaande aard of omkeerbaar is (bijvoorbeeld secundair aan CABG, een interventionele procedure, pneumonie of hyperthyreoïdie)
- Patiënten die langdurige antistolling nodig hebben vanwege een andere aandoening dan atriumfibrilleren
- Patiënten met een indicatie voor chronische P2Y12-bloedplaatjesremmingstherapie
- Patiënten die niet geschikt zijn voor warfarine op korte termijn (inclusief als gevolg van bloedingsdiathese of coagulopathie of absolute contra-indicatie warfarine) of die een transfusie zullen weigeren
- Patiënten met reumatische mitralisklepziekte, bekende ernstige aortastenose waarvoor chirurgische of percutane klepvervanging nodig is, of bestaande mechanische klepprothese
- Actieve infectie met bacteriëmie
- Bekende overgevoeligheid of contra-indicatie voor aspirine, clopidogrel, heparine/bivalirudine, materiaal of component van het hulpmiddel (nitinol, nikkel, titanium, PET) en/of contrastgevoeligheid die niet voldoende vooraf kan worden behandeld
- Anatomische omstandigheden die de uitvoering van de LAA-occlusieprocedure zouden verhinderen (bijv. eerder atriumseptumdefect [ASS] of patiënt foramen ovale [PFO] chirurgische reparatie of geïmplanteerd sluitingsapparaat, of uitgewist linker atriumhartoor)
- Recente (binnen 30 dagen vóór de procedure) of geplande (binnen 60 dagen na de procedure) cardiale of niet-cardiale interventionele of chirurgische procedure (bijv. cardioversie, ablatie, percutane coronaire interventie, cataractchirurgie, enz.)
- Recente (binnen 90 dagen vóór de procedure) beroerte, voorbijgaande ischemische aanval of myocardinfarct
- Ernstig hartfalen (New York Heart Association Klasse IV)
- Bekende asymptomatische halsslagaderziekte met >70% diameterstenose OF symptomatische halsslagaderziekte (>50% diameterstenose met ipsilaterale beroerte of TIA). Proefpersonen met een eerdere carotis-endarteriëctomie of plaatsing van een carotisstent kunnen worden ingeschreven, op voorwaarde dat de bekende diameterstenose <50% bedraagt.
- Een hart- of andere orgaantransplantatie in het verleden of in behandeling, of op de wachtlijst staan voor een orgaantransplantatie
- Bekende andere medische ziekte of bekende geschiedenis van middelenmisbruik die kan leiden tot niet-naleving van het protocol, de interpretatie van gegevens kan verstoren of die verband houdt met een levensverwachting van minder dan 2 jaar
- Huidige deelname aan een ander onderzoek naar een geneesmiddel of apparaat. Echocardiografische uitsluitingscriteria
- De anatomie van het linker atriumhartoor is niet geschikt voor het LAmbre Plus LAAO-apparaat of het controleapparaat volgens de IFU van de fabrikant (de anatomie en maatvoering moeten bijvoorbeeld geschikt zijn om beide apparaten in de proef te laten deelnemen, of de maximale LAAO-lengte < maximale ostiumbreedte per TEE)
- LVEF <30%
- Intracardiale trombus of dicht spontaan echocontrast, zoals gevisualiseerd door TEE binnen 2 dagen vóór implantatie
- Matige of grote pericardiale effusie >10 mm of symptomatische pericardiale effusie, tekenen of symptomen van acute of chronische pericarditis, of tekenen van tamponadefysiologie. In het geval van een behandelbare pericardiale effusie kan de patiënt op een later tijdstip een implantatie ondergaan nadat deze adequaat is behandeld.
- Atriumseptumdefect dat sluiting rechtvaardigt.
- Aanwezigheid van een patent foramen ovale (PFO) met hoog risico, gedefinieerd als een atriumseptumaneurysma (excursie >15 mm of lengte >15 mm) of grote shunt (vroeg, binnen 3 slagen en/of aanzienlijke doorgang van bellen)
- Matige of ernstige mitralisklepstenose (mitraalklepoppervlak <1,5 cm2)
- Complex atheroma met mobiele plaque van de dalende aorta en/of aortaboog (graad 4 of hoger)
- Bewijs van een harttumor

De controleapparaten voor het onderzoek zullen in de handel verkrijgbare transkatheter LAAO-apparaten zijn.
Momenteel zijn er drie door de FDA goedgekeurde apparaten (WATCHMAN FLX en WATCHMAN FLX Pro van Boston Scientific en Amplatzer Amulet van Abbott Laboratories) die allemaal kunnen worden gebruikt bij proefpersonen die aan de controlegroep zijn toegewezen.
