Verbetering van de intergenerationele gezondheid in Nigeria: peripartum als cruciale levensfase voor de cardiovasculaire gezondheid (ENHANCE-CVH)
Het doel van de ENHANCE-CVH-studie is het bevorderen van de intergenerationele cardiovasculaire gezondheid (CVH) van vrouwen en kinderen in Abuja en het omliggende Federaal Hoofdstedelijk Territorium van Nigeria. Het onderzoeksteam is van plan dit te bereiken door een bestaande, op bewijs gebaseerde interventie – Parents as Teachers (PAT) curriculum + Healthy Eating Active Living Taught at Home (HEALTH) – aan te passen onder zwangere vrouwen met obesitas en hun kinderen in Nigeria. Het doel van de studie is om de effectiviteit en implementatie van de PAT+HEALTH-interventie onder leiding van gemeenschapsdocenten te evalueren via huisbezoeken om de volgende hoofdvragen te beantwoorden:
- Zullen de vrouwen die aan de interventie deelnemen een grotere gewichtsverandering ondergaan vanaf de uitgangssituatie tot de follow-up na 18 maanden vergeleken met de vrouwen die dat niet doen?
- Zullen de vrouwen en kinderen in de interventiegroep beter CVH-bevorderend levensstijlgedrag vertonen vergeleken met die in de controlegroep?
Deelnemers aan het onderzoek vullen basisonderzoeken in en laten klinische gegevens, laboratoriumgegevens en lichaamsafmetingen vastleggen. Deelnemers zullen deze procedures ook laten herhalen bij de geboorte en elke 6 maanden tot 24 maanden na de bevalling. Eenmaal gerandomiseerd naar de interventiegroep, voltooien de deelnemers 40 tweewekelijkse PAT+HEALTH-curriculumsessies, die elk ongeveer 60-90 minuten duren.

Beschrijving
Inclusiecriteria:
•≥18 jaar oud,
- Zwangere vrouwen jonger dan 14 weken zwangerschap,
- Singleton-dracht,
- Body mass index > 30 kg/m2
- Bereid om 2 jaar deel te nemen aan ENHANCE CVH,
- Van plan bent om prenatale zorg te ontvangen en te leveren in een van de deelnemende eerstelijnsgezondheidszorgcentra, centra of het University of Abuja Teaching Hospital, en
- In staat om geïnformeerde toestemming te geven voor deelname aan het onderzoek, met begrip voor zowel zij als haar kind.
Uitsluitingscriteria:
- Huidige deelname aan een afslankprogramma,
- Onvermogen om deel te nemen aan een wandelprogramma,
- Misbruik van actieve stoffen
- Behandeling met medicijnen waarvan bekend is dat ze het gewicht beïnvloeden (bijv. corticosteroïden, antipsychotica),
- Bent u van plan om binnen de komende twee jaar te verhuizen, of
- Niet bereid om geïnformeerde toestemming te geven.

Experimenteel: PAT+HEALTH-interventie
Deelnemers die aan deze arm zijn gerandomiseerd, zullen ante- en postnataal 40 tweewekelijkse PAT+HEALTH-interventies ontvangen.
Deze zullen zich in de eerste plaats richten op het bevorderen van de cardiovasculaire gezondheid voor moeder en kind door middel van levensstijl- en gedragsveranderingen, samen met aanvullende ondersteuning en voorlichting over zwangerschap en cardiovasculaire gezondheid.

Deze interventie is een aanpassing van het PAT-curriculum en de HEALTH-interventie, specifiek afgestemd op de Nigeriaanse context.

Geen tussenkomst: Controle
Deelnemers die naar de controlegroep zijn gerandomiseerd, ontvangen niet de PAT+HEALTH-interventie en krijgen alleen standaard pre- en postnatale zorg.
