Kinshasa long echografie -benadering validatie (K-LUS)
Validatie van de Kinshasa-long-echografie-benadering voor het vernauwing van differentiële diagnose bij kinderen die zijn opgenomen met acute ademhalingssymptomen in instellingen met lage hulpbronnen
Studie -team zal een prospectief, observationeel onderzoek uitvoeren op twee locaties in de Democratische Republiek Congo (DRC) en Bangladesh bij kinderen van 3 maanden tot 14 jaar, opgenomen in het ziekenhuis met acute ademhalingssymptomen. De Kinshasa Lung Ultrasound (K-LUS) -benadering integreert bestaande WHO-klinische richtlijnen, long-ultrasone diagnostische nauwkeurigheidsbewijs en pediatrische echografie-richtlijnen. De aanpak werd gebouwd met behulp van een gemodificeerde Delphi -techniek en integreert zes LUS -profielen, twee klinische geschiedeniskenmerken (timing van aanvang, trauma) en één klinisch onderzoeksfunctie (koorts) om een van de 10 klinische diagnose te suggereren. Nadat de eerste diagnose is vastgesteld door de behandelend arts, zal een onderzoeksassistent een LUS-onderzoek uitvoeren en de K-LUS-aanpak toepassen. Vergelijking tussen de K-Lus afgeleide diagnose en de klinische diagnose zal worden uitgevoerd. Na het ontslaan van de patiënt zal een panel ook de meest waarschijnlijke diagnose stellen volgens alle informatie die beschikbaar is tijdens het verblijf van de patiënt.

Inclusiecriteria:
- Kinderen tussen 3 maanden en 14 jaar;
- Toegelaten op de afdeling spoedeisende hulp of wijk met hoest of moeite met ademhaling;
Uitsluitingscriteria:
- Verwacht kort verblijf op de afdeling spoedeisende hulp (<6H)
- Noodoverdracht naar andere faciliteiten
- Weigering van geïnformeerde toestemming door het bijwonen van ouder of verzorger, indien van toepassing
- Long -echografie niet haalbaar (bijv. Niet-beschikbaarheid van een getrainde arts)

Een echografie van elke afzonderlijke zone wordt op de machine opgeslagen voor beoordeling en score en kwaliteitscontroledoeleinden. Alle longregio's worden gescoord met behulp van de LUS -beluchtingsscore (11,23).
