Werkzaamheid en veiligheid van vaginale radiofrequentie voor vulvovaginale atrofie bij borstkankerpatiënten (RF-Vaginale)
RF-Vaginale: gerandomiseerde fase III-studie die de werkzaamheid en veiligheid van vaginale radiofrequentie evalueert bij de behandeling van vulvovaginale atrofie en droogheid bij patiënten die worden behandeld voor borstkanker
Deze fase III, gerandomiseerde, evenwichtige, parallelle groep, multicenter-studie heeft als doel het effect van vaginale radiofrequentie op de symptomen van vaginale droogheid na zes maanden te evalueren. De studie vergelijkt vaginale radiofrequentiebehandeling met niet-hormonale hydratatietherapie bij een populatie van patiënten die adjuvante hormonale therapie ondergaan met aromataseremmers, met of zonder LHRH-agonisten, voor borstkanker.
Patiënten worden willekeurig toegewezen aan een van de twee parallelle behandelingsgroepen tot de beoordeling van zes maanden:
De eerste groep krijgt de referentiebehandeling, die bestaat uit lokale hydratatie met behulp van een op hyaluronzuur gebaseerde behandeling die drie keer per week is toegepast. Bovendien kunnen onderzoekers ondersteunende maatregelen opnemen naar eigen goeddunken, zoals fysiotherapie of vaginale dilatoren.
De tweede groep krijgt dezelfde referentiebehandeling in combinatie met vaginale radiofrequentietherapie. Deze interventie bestaat uit drie sessies, elk op afstand 4 tot 6 weken uit elkaar.

Deze studie wil zien of vaginale radiofrequentietherapie kan helpen de vaginale droogheid te verminderen bij vrouwen die hormoontherapie voor borstkanker krijgen.
De deelnemers zijn verdeeld in twee groepen:
Eén groep zal drie keer per week een niet-hormonale vaginale vochtinbrengende crème (hyaluronzuur) gebruiken. Indien nodig kunnen ze ook andere ondersteunende behandelingen gebruiken zoals fysiotherapie of vaginale dilatoren.
De andere groep ontvangt dezelfde moisturizerbehandeling, maar zal ook drie sessies van vaginale radiofrequentietherapie hebben, met elke sessie op een afstand van 4 tot 6 weken uit elkaar.
Na zes maanden zullen onderzoekers de twee groepen vergelijken om te zien of vaginale radiofrequentie extra voordelen biedt bij het verlichten van vaginale droogheid.

Beschrijving
Inclusiecriteria:
- Patiënt ouder dan 40 jaar
- Het presenteren van het uitschakelen van vaginale droogsymptomen ondanks goed geavanceerde lokale hydraterende behandeling, bevestigd door een vaginale droogheidsscore ≥3
- Patiënt die adjuvante behandeling ondergaat voor borstkanker
- Patiënt behandeld met aromataseremmers +/- LHRH-agonist gedurende minimaal 3 maanden met een geplande resterende duur van ten minste 12 maanden
- Patiënt aangesloten bij een organisatie voor sociale zekerheid
- Patiënt heeft de geïnformeerde toestemming voor de huidige studie ondertekend
Uitsluitingscriteria:
Contra -indicaties gerelateerd aan de radiofrequentietechniek:
- Lopende urineweginfectie
- Pacemaker
- Spiraaltje intra -uterine apparaat
- Zwangerschap
- Mucocutane ziekte met vulvovaginale lokalisatie: infectie, pijnlijke, scleratrofisch korstmos, psoriasis, eczeem, enz.
- Neurodegeneratieve ziekte met perineale betrokkenheid
- Huidige of recente behandeling (in de afgelopen 12 maanden) met andere fysieke methoden: vaginale laser, fotobiomodulatie, PRP-injectie, hyaluronzuurinjectie of lipo-modellering.
- Zwangere vrouwen of borstvoeding geven
- Patiënten onder voogdij of trusteeship.

Experimenteel: Vaginale radiofrequentiebehandeling
De evaluator zal blind zijn voor de interventie (verschillende onderzoeker voor vaginale radiofrequentie -sessies en klinische evaluatie -overleg tot 6 maanden, M0, M3 en M6). De verblindende zal worden opgeheven na de evaluatie van 6 maanden, met de mogelijkheid van radiofrequente behandeling voor patiënten in de controlegroep die niet voldoende zijn opgelucht. |
Voor patiënten gerandomiseerd naar de experimentele groep: 3 sessies van elk 20 minuten, 4 tot 6 weken uit elkaar (M0, M1, M2) idealiter, met +/- 1 onderhoudssessie tussen 8 en 12 maanden na de eerste sessie (M8-12).
Referentiebehandeling die lokale hydratatie combineert (op hyaluronzuur gebaseerde behandeling, 3 keer per week), met de mogelijkheid om bijbehorende maatregelen te gebruiken naar discretie van de onderzoeker (fysiotherapie, vaginale dilatoren, enz.).

Type bijwerkingen: gevoel van warmte, roodheid, erytheem, overgevoeligheid, dyspareunia, andere. Tijd van het begin: tijdens de sessie, onmiddellijk na of meer dan 24 uur na de sessie. Intensiteit: beoordeeld volgens het National Cancer Institute Common Terminology Criteria voor bijwerkingen (NCI-CTCAE V5.0), met scores variërend van graad 1 (mild) tot graad 5 (overlijden). Hogere scores duiden op een grotere ernst van bijwerkingen. Gevolgen voor de behandeling: moet de sessie onderbreken, de radiofrequente temperatuur verminderen, enz. |
