Prospectieve evaluatie van 68Ga-FAPI PET bij galwegkankers (FAPi-2)
FAP 2: Nut van Gallium-68-Fibroblast Activatie Proteïne Remmer (FAPI) PET bij Galwegkankers: Een Prospectieve Studie
Het doel van dit prospectieve observationele onderzoek is om te evalueren of Gallium-68 Fibroblast Activatie Proteïne Remmer (FAPI) PET/CT de detectie, stadiering en beoordeling van recidief kan verbeteren bij volwassen patiënten (≥18 jaar) met vermoedelijke of bevestigde galwegkankers, waaronder galblaaskanker, cholangiocarcinoom en vermoedelijk recidief na behandeling.
De belangrijkste vraag(en) die dit onderzoek wil beantwoorden zijn:
Kan FAPI PET/CT een grotere gevoeligheid, specificiteit en diagnostische nauwkeurigheid bieden voor primaire tumoren, lymfeklierziekte en metastatische laesies in vergelijking met standaard FDG PET/CT?
Biedt FAPI PET/CT aanvullende diagnostische opbrengst die de klinische besluitvorming en stadiering kan beïnvloeden, mogelijk de behoefte aan invasieve stadieringsprocedures verminderend?
Onderzoekers zullen FAPI PET/CT vergelijken met FDG PET/CT om te zien of FAPI de detectie van metastatische of recidiverende ziekte verbetert, vooral peritoneale of levermetastasen en lymfeklierbetrokkenheid.
Deelnemers zullen:
Schriftelijke geïnformeerde toestemming verlenen.
FAPI PET/CT-beeldvorming ondergaan (baseline en/of bij vermoedelijk biochemisch of radiologisch recidief).
Kwantitatieve beeldvormingsparameters laten evalueren (SUVmax, tumor-leverratio's, metabool volume).
Vergelijking met FDG PET/CT en/of follow-up beeldvorming of histopathologie als gouden standaard kunnen ondergaan.

Galwegkankers presenteren zich vaak in een vergevorderd stadium en vereisen nauwkeurige stadiëring om behandelbeslissingen zoals chirurgie, systemische therapie of palliatieve behandeling te begeleiden. Hoewel 18F-FDG PET/CT veelvuldig wordt gebruikt in de klinische praktijk, is de diagnostische prestaties ervan beperkt bij het opsporen van kleine primaire laesies, lymfekliermetastasen, levermetastasen en peritoneale carcinomatose. Gallium-68 Fibroblast Activatie Proteïne Remmer (FAPI) PET/CT is een nieuwe beeldvormingstechnologie die zich richt op kanker-geassocieerde fibroblasten, die overvloedig aanwezig zijn in het desmoplastische stroma dat kenmerkend is voor galwegkankers. Vroeg bewijs suggereert dat FAPI PET/CT mogelijk verbeterde laesiedetecteerbaarheid en tumor-achtergrondcontrast biedt in vergelijking met FDG PET/CT.
Deze prospectieve observationele studie zal de klinische bruikbaarheid van Ga-68 FAPI PET/CT evalueren voor initiële stadiëring en vermoedelijke recidieven bij patiënten met galwegkankers. Diagnostische prestatiewaarden (gevoeligheid, specificiteit, PPV, NPV en semi-kwantitatieve PET-parameters zoals SUVmax en metabool volume) zullen worden beoordeeld en vergeleken met FDG PET/CT en/of follow-up beeldvorming of histopathologie als gouden standaard. De studie beoogt de incrementele diagnostische bijdrage van FAPI PET/CT en het potentiële effect op klinische besluitvorming te bepalen.
Bevindingen van dit onderzoek kunnen helpen bij het definiëren van de rol van FAPI PET/CT als complementaire of alternatieve beeldvormingsmodaliteit bij stadiëring en herstadiëring van galwegkankers en kunnen toekomstige prospectieve multicenterstudies ondersteunen.
