Evaluación Prospectiva de 68Ga-FAPI PET en Cánceres de las Vías Biliares (FAPi-2)

El objetivo de este estudio observacional prospectivo es evaluar si la PET/TC con inhibidor de la proteína de activación de fibroblastos marcada con galio-68 (FAPI) puede mejorar la detección, estadificación y evaluación de la recurrencia en pacientes adultos (≥18 años) con sospecha o confirmación de cáncer de las vías biliares, incluyendo cáncer de vesícula biliar, colangiocarcinoma y sospecha de recurrencia tras el tratamiento.
Las principales preguntas que este estudio pretende responder son:
¿Puede la PET/TC con FAPI proporcionar mayor sensibilidad, especificidad y precisión diagnóstica para tumores primarios, enfermedad ganglionar y lesiones metastásicas en comparación con la PET/TC estándar con FDG?
¿Ofrece la PET/TC con FAPI un rendimiento diagnóstico adicional que pueda afectar la toma de decisiones clínicas y la estadificación, reduciendo potencialmente la necesidad de procedimientos invasivos de estadificación?
Los investigadores compararán la PET/TC con FAPI con la PET/TC con FDG para ver si la FAPI mejora la detección de enfermedad metastásica o recurrente, especialmente metástasis peritoneal o hepática y afectación ganglionar.
Los participantes:
Proporcionarán consentimiento informado por escrito.
Se someterán a imágenes de PET/TC con FAPI (línea base y/o en caso de sospecha de recurrencia bioquímica o radiológica).
Tendrán evaluados parámetros de imagen cuantitativos (SUVmax, relaciones tumor-hígado, volumen metabólico).
Podrán someterse a comparación con PET/TC con FDG y/o imágenes de seguimiento o histopatología como estándar de referencia.

Los cánceres del tracto biliar suelen presentarse en estadio avanzado y requieren una estadificación precisa para guiar decisiones de tratamiento, como cirugía, terapia sistémica o manejo paliativo. Aunque la PET/TC con 18F-FDG se usa ampliamente en la práctica clínica, su rendimiento diagnóstico es limitado para detectar lesiones primarias pequeñas, metástasis ganglionares, metástasis hepáticas y carcinomatosis peritoneal. La PET/TC con inhibidor del activador de fibroblastos de galio-68 (FAPI) es una nueva tecnología de imagen que se dirige a los fibroblastos asociados al cáncer, abundantes en el estroma desmoplásico característico de los cánceres del tracto biliar. La evidencia temprana sugiere que la PET/TC con FAPI puede ofrecer una mejor detección de lesiones y un mayor contraste tumor-fondo en comparación con la PET/TC con FDG.
Este estudio observacional prospectivo evaluará la utilidad clínica de la PET/TC con Ga-68 FAPI para la estadificación inicial y la sospecha de recurrencia en pacientes con cánceres del tracto biliar. Se evaluarán métricas de rendimiento diagnóstico (sensibilidad, especificidad, VPP, VPN y parámetros PET semicuantitativos como SUVmax y volumen metabólico) y se compararán con la PET/TC con FDG y/o imágenes de seguimiento o histopatología como estándar de referencia. El estudio tiene como objetivo determinar la contribución diagnóstica incremental de la PET/TC con FAPI y su impacto potencial en la toma de decisiones clínicas.
Los hallazgos de esta investigación pueden ayudar a definir el papel de la PET/TC con FAPI como modalidad de imagen complementaria o alternativa en la estadificación y reestadificación de los cánceres del tracto biliar, y pueden respaldar futuros ensayos prospectivos multicéntricos.

Los participantes elegibles incluyen individuos con enfermedad avanzada de inicio (tumores sospechosos T3/T4, afectación ganglionar N1 o afectación vascular), pacientes con cáncer incidental de vesícula biliar residual o con ganglios positivos (iGBC), y aquellos con sospecha de recurrencia posterior al tratamiento basada en marcadores bioquímicos o evaluación radiológica.

- Hombres y mujeres ≥ 18 años;
- Avanzado de inicio (sospecha de T3, T4, N1, afectación vascular)
- iGBC (residual, N1)
- Sospecha de recurrencia pos-tratamiento (bioquímica o radiológica)
