Delirium is een veelvoorkomend neuropsychiatrisch syndroom dat wordt gekenmerkt door een acute verstoring van aandacht, cognitie en bewustzijn. Het is geassocieerd met aanzienlijke morbiditeit, mortaliteit en gezondheidszorguitgaven. Recent onderzoek heeft bewijs geleverd dat de verbinding tussen hersenmetabolisme en delirium ondersteunt. Tijdens toestanden van verhoogde systemische ontsteking, zoals sepsis of trauma, ervaart de hersenen een mismatch tussen energietoevoer en -vraag, wat vaak geassocieerd is met delirium, vooral bij personen met reeds bestaande cognitieve beperking.
Bij kritisch zieke patiënten treedt mitochondriale dysfunctie op in de context van systemische ontsteking, wat bijdraagt aan verhoogde bloed-hersenbarrièrepermeabiliteit en neuro-inflammatie. Het gevolg hiervan is microgliale activatie, die de inflammatoire respons versterkt door de afgifte van pro-inflammatoire cytokines. De resulterende mitochondriale dysfunctie leidt tot verminderde oxidatieve fosforylering, verminderde adenosinetrifosfaat (ATP)-productie en verhoogde productie van reactieve zuurstofspecies. Als reactie op systemische ontsteking gaan microglia over naar een pro-inflammatoir fenotype dat wordt gekenmerkt door verhoogde aerobe glycolyse. Deze metabole herprogrammering put de beschikbaarheid van glucose voor neuronen uit en verergert het cerebrale energietekort. Opkomend bewijs suggereert dat geactiveerde microglia tijdens ontsteking concurreren met neuronen om metabolische substraten. Geactiveerde microglia vertonen metabole flexibiliteit en schakelen over naar verhoogde glycolyse om aan hun verhoogde energie- en biosynthetische behoeften te voldoen. Deze concurrentie om voedingsstoffen verergert het neuronale energietekort en verhoogt de metabole stress. De onderzoekers veronderstellen dat dit hersenenergietekort bijdraagt aan de cognitieve en neurologische symptomen die kenmerkend zijn voor delirium.
Ketonen, zoals β-hydroxybutyraat, zijn de secundaire energiebron van de hersenen wanneer glucose niet beschikbaar is. Na transport over de bloed-hersenbarrière wordt β-hydroxybutyraat gemetaboliseerd tot acetyl-CoA (acetyl co-enzym A), waardoor het direct de citroenzuurcyclus binnenkomt, de glycolytische bottleneck omzeilt, om ATP te produceren. Naast het dienen als substraat voor ATP-productie, ondersteunen ketonen mitochondriale functie, beperken ze oxidatieve stress en verminderen ze neuro-inflammatie. Ketonen bieden een tweevoudig therapeutisch voordeel in de context van voedingsstoffenconcurrentie. Ze ondersteunen niet alleen neuronale oxidatieve fosforylering door verminderde glycolyse te omzeilen, maar bevorderen ook anti-inflammatoire microgliale fenotypen, remmen ze inflammasoomactivatie en ondersteunen ze metabole herprogrammering. Dit dubbele effect vermindert verder de microgliale glucosebehoefte, waardoor de beschikbaarheid van neuronale substraten wordt verbeterd.
De onderzoekers stellen een prospectieve, gerandomiseerde, placebo-gecontroleerde pilotstudie voor van exogene ketonestersuppletie bij 40 kritisch zieke patiënten om de veiligheid en haalbaarheid van deze nieuwe interventie te beoordelen en voorlopige gegevens te genereren over de effectiviteit ervan bij het verminderen van ICU-delirium, gemeten aan de hand van delirium- en comavrije dagen (DCFD's). Exogene ketonen zijn aangetoond om de hersenenergetica te ondersteunen en neuro-inflammatie te verminderen, waarbij ze rechtstreeks ingrijpen op pathways die betrokken zijn bij de ontwikkeling van delirium. Door de duur van delirium te verkorten of het ontstaan ervan te voorkomen, heeft dit onderzoek het potentieel om de langetermijncognitieve uitkomsten voor ICU-overlevenden te verbeteren. De onderzoekers stellen voor om volwassen patiënten in te sluiten op het moment van ICU-opname, met randomisatie naar ofwel een enterale ketonesterbehandelingsgroep of een placebo met dextrose die qua smaak, volume en calorieën overeenkomt. Het studiegeneesmiddel of placebo wordt toegediend op het moment van insluiting, binnen 24 uur na ICU-opname, en vervolgens elke zes uur gedurende maximaal 7 dagen tot ICU-ontslag of overlijden, wat het eerst plaatsvindt. Ketoentoediening wordt voortgezet na de diagnose van delirium. In overeenstemming met eerdere studies zal de initiële dosis β-hydroxybutyraat 25 g zijn; latere doses worden echter getitreerd om serum β-hydroxybutyraatspiegels tussen 1,5 en 3,5 mM te handhaven, met geprotocolleerde monitoring van vitale functies, serum-pH, glucosespiegels en bijwerkingen in het maag-darmkanaal. Delirium wordt beoordeeld met behulp van het Confusion Assessment Method for the ICU (CAM-ICU) delirium screeningsinstrument, tweemaal daags gedurende een periode van 7 dagen.
Deze pilotstudie zal de haalbaarheid, veiligheid en verdraagbaarheid van orale exogene ketonensuppletie bij kritisch zieke patiënten beoordelen. Het doel is aan te tonen dat ketoentoediening goed wordt verdragen, zonder significante veiligheidsproblemen, in overeenstemming met eerder bewijs dat orale ketonen veilig kunnen worden toegediend, zelfs bij kwetsbare patiëntenpopulaties. Succesvolle afronding van dit doel zal een veiligheidsprofiel voor ketonengebruik op de ICU vaststellen, wat essentieel is voordat deze nieuwe therapie wordt toegepast bij kritisch zieke patiënten. De onderzoekers veronderstellen dat patiënten die ketonen ontvangen meer DCFD's zullen hebben vergeleken met degenen die een placebo ontvangen. De onderzoekers zullen ook een verkennende analyse uitvoeren van het biologische effect van ketonentherapie door biomarkers geassocieerd met delirium en ketonmetabolisme te onderzoeken via seriële meting van serumspiegels van perifere inflammatoire mediatoren, metabole stresstesten, β-hydroxybutyraatspiegels en markers van centraal zenuwstelsel (CZS)-letsel.
Ketonen bieden een veelbelovende nieuwe therapeutische optie voor delirium. Door in te grijpen op de onderliggende neurometabole en neuro-inflammatoire veranderingen geassocieerd met delirium, ondersteunen ze energieproductie, verminderen ze oxidatieve stress en moduleren ze ontsteking. Patiënten met reeds bestaande cognitieve beperking, zoals personen met milde cognitieve stoornis of de ziekte van Alzheimer, vertonen een basishersenenergietekort door verminderd cerebraal glucosemetabolisme. Dit chronische energietekort verhoogt de kwetsbaarheid van het verouderende brein voor delirium. Bovendien verergeren de neurometabole gevolgen van delirium bij personen met reeds bestaande cognitieve beperking het hersenenergietekort, waardoor cognitieve achteruitgang wordt versneld. De veiligheid, verdraagbaarheid en snelle inductie van ketose na orale toediening van ketonesters, naast de bovengenoemde gunstige effecten, suggereren dat dit een therapie kan zijn die bij ICU-opname kan worden gestart als een potentiële preventieve maatregel bij patiënten met risico. Ketonen hebben het potentieel om deliriummanagement te transformeren en patiëntenzorg te verbeteren; deze klinische studie is echter nodig om de veiligheid en effectiviteit van orale ketonestersuppletie te evalueren bij het verminderen van de incidentie, ernst en duur van delirium bij kritisch zieke patiënten.

Beschrijving
Inclusiecriteria:
- Volwassen patiënten (≥18 jaar) opgenomen op de medische intensive care.
- Huidige IC-opname met verwachte IC-verblijfsduur ≥24 uur.
- Enterale toegang aanwezig, geplande plaatsing van enterale toegang, of adequate orale inname, en de mogelijkheid om binnen 24 uur na inclusie enterale dosering te ontvangen.
- Mogelijkheid om delierbeoordelingen (CAM-ICU uitvoerbaar) te voltooien op het moment van inclusie.
Exclusiecriteria:
- Ernstige metabole acidose bij inclusie: bloedgas pH <7,20 of bicarbonaat < 8 mmol/L.
- Diabetische ketoacidose als IC-opnamediagnose of hyperketonemie door een ketoacidotische toestand.
- Hypoglykemie als IC-opnamediagnose of glucose <60 mg/dL.
- Patiënten met een voorgeschiedenis van diabetes mellitus type 1.
- Hemoglobine <7,0.
- Nierfalen waarvoor dialyse nodig is, gestart of verwacht bij inclusie (CRRT of iHD).
- Fulminant leverfalen of AST/ALT > 5× ULN of totaal bilirubine > 3 mg/dL.
- Refractaire shock (gedefinieerd als norepinefrine dosis ≥20 µg/min of gebruik van een tweede vasopressormiddel).
- Zwangerschap (positieve urine/serum hCG bij screening of bekende zwangerschap).
- Ongecontroleerde ileus of gastro-intestinale aandoening, zoals een bovenste gastro-intestinale bloeding, die enterale dosering verhindert.
- Gebruik van SGLT2-remmers in de afgelopen 7 dagen.
- Gebruik van ADH/ALDH-remmers (bijv. fomepizol, disulfiram) in de afgelopen 7 dagen of gepland gebruik.
- Ernstige dementie of neurodegeneratieve ziekte, gedefinieerd als beperking die de patiënt verhindert zelfstandig te leven bij baseline of IQCODE >4,5, gemeten via een gekwalificeerde vertegenwoordiger van de patiënt. Deze exclusie geldt ook voor psychische aandoeningen die langdurige institutionalisatie vereisen, verworven of aangeboren verstandelijke beperking, ernstige neuromusculaire aandoeningen, ziekte van Parkinson en ziekte van Huntington. Het sluit ook patiënten uit met ernstige tekorten door structurele hersenziekten zoals beroerte, intracraniële bloeding, schedeltrauma, maligniteit, anoxisch hersenletsel of cerebraal oedeem.
- Benzodiazepineafhankelijkheid of alcoholafhankelijkheid gebaseerd op het besluit van het medisch team om een specifiek behandelplan met benzodiazepines (als continue infusies of intermitterende intraveneuze bolussen) voor deze afhankelijkheid in te stellen.
- Actieve epileptische aanvallen tijdens deze IC-opname die behandeld worden met intraveneuze benzodiazepines.
- Verwachte overlijden binnen 24 uur na inclusie of gebrek aan toewijding aan agressieve behandeling door familie/medisch team (bijv. waarschijnlijk levensondersteunende maatregelen binnen 24 uur na screening te staken).
- Opname op de IC alleen voor postoperatieve monitoring of frequente neurologische beoordelingen.

Delirium severity will be assessed using the Confusion Assessment Method for the Intensive Care Unit-7 (CAM-ICU-7) delirium severity scale.
The CAM-ICU-7 score ranges from 0 to 7, with higher scores indicating more severe delirium.
A composite CAM-ICU-7 delirium severity score will be calculated for each participant by summing all available CAM-ICU-7 scores from enrollment through study day 7.
The primary outcome will compare the mean composite CAM-ICU-7 delirium severity score between treatment arms.
A clinically meaningful treatment effect is defined as a 25% lower mean composite CAM-ICU-7 score in the intervention arm compared with the control arm.

A prespecified safety or tolerability event is defined as any of the following occurring from enrollment through study day 7: acid-base abnormality (blood gas pH <7.20 or serum bicarbonate <8 mmol/L), off-target hyperketonemia (peak serum β-hydroxybutyrate >3.5 mmol/L despite dose reduction), hypoglycemia (<60 mg/dL), renal or hepatic safety signal (new dialysis initiation; aspartate aminotransferase (AST) or alanine aminotransferase (ALT) >5× upper limit of normal, or total bilirubin >3 mg/dL without alternative explanation), or hemodynamic instability temporally related to dosing.
Gastrointestinal symptoms (nausea, vomiting, diarrhea, cramping) will be reported separately as tolerability adverse events.
