Dit is een prospectieve, gerandomiseerde, gecontroleerde klinische studie met herhaalde metingen en longitudinale follow-up. Deelnemers van 14-35 jaar met vroegstadium chronische nierziekte, gedefinieerd door persisterende albuminurie ondanks behouden glomerulaire filtratie, zullen worden geworven via een institutioneel nier-screeningprogramma in Aguascalientes, Mexico. De interventie richt zich op de darm-nier-as door aanhoudende leefstijlverandering en microbiota-modulatie. De primaire mechanistische hypothese is dat het verminderen van intestinale dysbiose de microbiota-afgeleide uremische toxines en systemische ontsteking zal verminderen, mogelijk vroege markers van nierziekteprogressie zal afzwakken.
Schriftelijke geïnformeerde toestemming zal worden verkregen van deelnemers van 18 jaar of ouder. Voor deelnemers onder de 18 jaar zal schriftelijke instemming worden verkregen samen met geïnformeerde toestemming van een ouder of wettelijke voogd.

Inclusiecriteria:
- Persisterende albuminurie (ACR >30 en <300 mg/g).
- Geschatte GFR >60 mL/min/1,73 m².
- Geen identificeerbare secundaire oorzaak van nierziekte.
Exclusiecriteria:
- Hypoalbuminemie.
- Nefrotisch syndroom.
- Persisterende macroalbuminurie (ACR >300 mg/g).
- Secundaire of erfelijke nierziekte.

Experimenteel: Multimodale Interventie
Deelnemers ontvangen een gecontroleerd dieet, een gestructureerd bewegingsprogramma en een oraal symbiotisch supplement dat eenmaal daags in een vaste dosis wordt toegediend gedurende 180 dagen.

Placebo-vergelijker: Controle
Deelnemers ontvangen hetzelfde gecontroleerde dieet en gestructureerd bewegingsprogramma plus een oraal placebo dat niet van het symbiotische preparaat te onderscheiden is, eenmaal daags toegediend gedurende 180 dagen.

1. Leefstijlinterventies (dieet + beweging) 2. Hun rol in de chronische ontstekingsreactie geassocieerd met dysbiose en ophoping van intestinale uremische metabolieten zoals beschreven bij CKD zal worden gekwantificeerd met IL-1β, IL-6 en TNF-α.
