De fysiotherapeutische beoordeling van ademhalingspatroonstoornis (PHAB)
Een evaluatie van fysiotherapiebeoordeling; een mixed methods-onderzoek om de fysiotherapiebeoordeling van ademhalingspatroonstoornis te evalueren
Deze studie beoogt een klinisch hulpmiddel te evalueren, genaamd de Breathing Pattern Assessment Tool (BPAT), die wordt gebruikt door fysiotherapeuten om ademhalingspatroonstoornis (breathing pattern disorder, BPD) te beoordelen. BPD is een aandoening waarbij de ademhaling inefficiënt is of niet synchroon loopt met de behoeften van het lichaam, wat leidt tot symptomen zoals kortademigheid.
De studie zal onderzoeken of de BPAT een betrouwbare en accurate manier is om BPD te identificeren en te beoordelen, en of het kan worden gebruikt om veranderingen in de tijd te monitoren, met name na fysiotherapeutische behandeling. Het zal ook BPAT-scores vergelijken met andere veelgebruikte vragenlijsten die kortademigheid en ademhalingssymptomen meten.
Deelnemers zullen bestaan uit patiënten met BPD, patiënten met astma en gezonde personen. Patiënten met BPD zullen fysiotherapiesessies volgen als onderdeel van hun gebruikelijke zorg, waarbij hun ademhaling wordt beoordeeld met de BPAT op verschillende momenten, waaronder voor en na de behandeling. Gezonde deelnemers nemen deel aan een enkel bezoek voor beoordeling, en gegevens van astmapatiënten worden verzameld uit routinematige klinische zorg.
De studie zal ook onderzoeken of BPAT-scores veranderen na de behandeling en of deze veranderingen de verbeteringen weerspiegelen die door patiënten en clinici worden gerapporteerd.
Daarnaast wordt een klein aantal patiënten en fysiotherapeuten uitgenodigd om deel te nemen aan interviews om hun ervaringen met het beoordelingsproces en de behandeling te delen. Dit zal helpen begrijpen of de BPAT acceptabel en nuttig is in de klinische praktijk.
Over het geheel genomen beoogt deze studie te verbeteren hoe ademhalingspatroonstoornis wordt beoordeeld en gemonitord, wat kan helpen bij het begeleiden van betere behandelingen voor patiënten in de toekomst.

Deze studie evalueert de meeteigenschappen en klinische bruikbaarheid van de Breathing Pattern Assessment Tool (BPAT), een beoordeling door fysiotherapeuten die wordt gebruikt om ademhalingspatroonstoornis (APS) te identificeren. APS is een aandoening waarbij ademhaling inefficiënt of ongepast is voor de behoeften van het lichaam en wordt geassocieerd met symptomen zoals kortademigheid.
De studie heeft tot doel te bepalen of de BPAT een betrouwbare en valide methode is voor het identificeren en beoordelen van APS, en of deze kan worden gebruikt om veranderingen in de tijd te volgen, met name na fysiotherapiebehandeling. BPAT-scores worden vergeleken met gevestigde vragenlijsten die ademhalingssymptomen en kortademigheid meten. De studie zal ook beoordelen of veranderingen in BPAT-scores overeenkomen met veranderingen gerapporteerd door patiënten en clinici.
Deelnemers zijn patiënten met APS (n=100), patiënten met astma (n=25) en gezonde controles (n=25). Patiënten met APS worden geworven via poliklinieken fysiotherapie en krijgen fysiotherapie als onderdeel van de gebruikelijke zorg. Gezonde controles komen voor één beoordelingsbezoek en astmaconstrictiegegevens worden verkregen uit routinematige klinische beoordelingen.
Bij de initiële beoordeling vullen twee fysiotherapeuten onafhankelijk BPAT-scores in voor elke deelnemer. Patiënten met APS komen binnen ongeveer 4 weken voor een follow-upbeoordeling, waarbij een herhaalde BPAT-beoordeling wordt uitgevoerd. Degenen bij wie APS is vastgesteld, ondergaan daarna een reeks fysiotherapiebehandelingen als onderdeel van de gebruikelijke zorg, doorgaans bestaande uit vier sessies. BPAT en andere uitkomstmaten worden herhaald aan het einde van de behandeling (tot 6 maanden). Patiënten en clinici geven ook beoordelingen van verandering in symptomen na behandeling.
De studie evalueert de betrouwbaarheid van de BPAT tussen clinici en over de tijd, de validiteit door vergelijking met andere gangbare uitkomstmaten, en de responsiviteit op verandering na behandeling.
Daarnaast nemen een subgroep patiënten en fysiotherapeuten deel aan kwalitatieve interviews om hun ervaringen met de BPAT en het gebruik ervan in de klinische praktijk te onderzoeken. Deze interviews helpen de aanvaardbaarheid en praktische bruikbaarheid van het instrument te beoordelen.
Deze mixed-methods-studie heeft tot doel de beoordeling en monitoring van ademhalingspatroonstoornis te verbeteren en consistentere en effectievere klinische zorg te ondersteunen.

Patiënten met BPD (n=100) Een gemakssteekproef van patiënten die verwezen zijn naar fysiotherapie ambulante diensten van Royal Brompton Hospitals voor BPD of onverklaarde benauwdheid, gedurende een wervingsperiode van 22 maanden, zal worden geworven.
Gezonde controlegroep (n=25) Een gemakssteekproef van gezonde controles zal worden geworven uit de relatie/belangrijke andere die de studie deelnemer begeleidt en/of ziekenhuispersoneel.
Astma controlepatiënten (n=25) Patiënten zullen worden geworven uit de Systematische Beoordeling van Astma Kliniek en er zal gebruik worden gemaakt van gegevens uit hun volledige gebruikelijke zorgbeoordeling.
Beschrijving
Inclusiecriteria (groep Ademhalingspatroonstoornis)
- Volwassenen ouder dan 18 jaar
- Poliklinische verwijzingen voor 'onverklaarbare benauwdheid' of mogelijke ademhalingspatroonstoornis
- Primaire BPD (geen andere significante respiratoire of cardiologische diagnose)
- Secundaire BPD (patiënten met een diagnose van astma)
- In staat om maximaal vier sessies poliklinische fysiotherapie-interventie af te ronden
Exclusiecriteria - Patiënten met een respiratoire diagnose waaronder COPD, ILD, bronchiëctasieën en cystische fibrose of structurele hartziekte
- Zwangerschap
- Niet in staat om geïnformeerde toestemming te geven
- Iedereen die momenteel deelneemt aan long-/cardiale revalidatie
- Iedereen die gespecialiseerde KNO-logopedie krijgt voor bovenste luchtwegsymptomen

The Breathing Pattern Assessment Tool (BPAT) is a clinician-completed assessment of breathing pattern.
Assessment components include (i) evaluation of chest/abdominal wall movement, noise of (ii) inspiratory and (iii) expiratory flow, (iv) channel of inspiration and expiration, (v) signs of air hunger (yawning, sighing and deeper breaths), (vi) RR and (vii) rhythm.
Each component is scored from 0 (normal) to 2 (severely abnormal), with a total score ranging from 0 to 14. Higher scores indicate greater deviation from normal breathing patterns.
Changes in score over time reflect changes in breathing pattern.

De Respiratie Alertheid Vragenlijst (Breathe-VQ) is een door de patiënt gerapporteerde maatstaf van 6 items die de mate van bewuste monitoring en aandachtsfocus op de ademhaling beoordeelt.
Elk item wordt beoordeeld op een Likert-schaal, met totale scores variërend van 6 tot 30.
Hogere scores wijzen op een grotere ademhalingsalertheid of hyperbewustzijn van de ademhaling, wat geassocieerd wordt met disfunctionele ademhalingspatronen.
Lagere scores duiden op minder bewuste monitoring van de ademhaling.
Veranderingen in de score in de loop van de tijd weerspiegelen veranderingen in ademhalingsgerelateerde alertheid.

The EuroQol 5-Dimension 5-Level Questionnaire (EQ-5D-5L) is a standardized measure of health-related quality of life comprising a descriptive system and a visual analogue scale (EQ VAS).
The descriptive system assesses five domains (mobility, self-care, usual activities, pain/discomfort, and anxiety/depression), each rated on five levels from 1 ("no problems") to 5 ("extreme problems"), which can be converted into a single index score.

It is assessed as the duration, in seconds, that a participant can comfortably hold their breath following a normal exhalation.
Typical values in healthy adults are approximately 20 to 40 seconds.
Higher values indicate better respiratory control and greater tolerance to carbon dioxide, while lower values indicate poorer tolerance and potential breathing pattern disorder (hyperventilation).
Changes in breath-hold time over time reflect changes in respiratory function.
There is no fixed upper limit; values are recorded in seconds.

End-Tidal Carbon Dioxide (EtCO₂, mmHg) is an objective physiological measure of the partial pressure of carbon dioxide at the end of expiration, assessed using capnography via a nasal cannula.
EtCO₂ is used as an indicator of ventilatory status.
Lower values indicate relative hypocapnia and potential hyperventilation, while higher values within the normal physiological range indicate improved ventilatory regulation.
Changes in EtCO₂ over time reflect changes in respiratory function.
Normal values are typically approximately 35-45 mmHg or 4.7 to 6.0 kPa.

The Self-Evaluation of Breathing Questionnaire (SEBQ) is a patient-reported outcome measure assessing symptoms associated with dysfunctional breathing.
It consists of 25 items, each scored on a Likert scale, with a total score ranging from 0 to 75.
Higher scores indicate worse breathing pattern disorder/dysfunctional breathing.
A total score greater than 11 has been suggested as indicative of possible dysfunctional breathing.
Changes in score over time reflect changes in symptom severity.
