Detaljert beskrivelse
Mål: Målet med denne studien er: (1) å utvikle en intelligent kundedrevet løsning for pediatrisk kirurgisk omsorg for foreldre til barn som gjennomgår omskjæring (ICory-Circumcision) og deres barn; (2) å undersøke effektiviteten av ICory-Omskjæring på utfall av foreldre (selveffektivitet i barnepass, perioperativ kunnskap og tilfredshet i perioperativ omsorg, behov for informasjon og angst) og barn (preoperativ angst og postoperativ smerte), som samt manglende oppmøte/ forsinket oppmøte for operasjonssaker og bruk av helsetjenester; og (3) å utforske brukernes (foreldre, barn, helsepersonell) oppfatninger av intervensjonen og forslag til forbedringer.
Studiedesign: Denne studien vil være en to-gruppe pretest og gjentatt posttest pilot randomisert kontrollert studie etterfulgt av kvalitativ prosessevaluering. Totalt 80 dyader av foreldre og deres barn i grunnskolealder (7-12 år) vil bli tilfeldig tildelt intervensjonsgruppen som vil motta ICory-Circumcision pluss rutinemessig omsorg, der foreldrene vil motta BuddyCare-søknaden mens barnet vil motta Triumf-spillapplikasjonen, og kontrollgruppen, som bare vil motta rutinemessig behandling gitt av sykehuset. En intervjuguide vil bli utviklet og brukt til å intervjue foreldrene og helsepersonell som brukte ICory-Circumcision for å utforske deres erfaringer og meninger om styrker, svakheter og forbedringsområder. Barna som har brukt Triumf-spillet vil også bli intervjuet for å utforske sine erfaringer rundt spillapplikasjonen.
Hypotese: Sammenlignet med kontrollgruppen vil (1) foreldre i intervensjonsgruppen som mottar ICory-Circumcision rapportere et betydelig (a) høyere nivå av selveffektivitet i barnepass; perioperativ kunnskap og tilfredshet (b) lavere nivå av behov for informasjon og preoperativ angst. (2) Barna i intervensjonsgruppen vil rapportere et betydelig lavere nivå av preoperativ angst og postoperativ smerte. (3) Intervensjonsgruppen vil ha færre tilfeller av manglende oppmøte/ forsinket oppmøte til operasjon, og redusert bruk av helsetjenester (f. redusert akuttmottak eller fastlegebesøk av ikke-medisinske årsaker) innen 2 uker etter operasjonen.

Barn
Inklusjonskriterier:
- alder mellom 7-12 år;
- er planlagt for en elektiv omskjæringsoperasjon;
- Kan snakke og lese på engelsk;
- er ledsaget av foreldrene deres (enten mor eller far eller begge deler) i løpet av den perioperative perioden; og
- har tilgang til smarttelefon/nettbrett (egen eller leie fra prosjektet).
Ekskluderingskriterier:
- kognitive og lærevansker identifisert fra journalen;
- en kronisk sykdom og/eller smerte som krevde spesiell medisinsk behandling;
- angstlidelse og annen stemningslidelse som identifisert fra deres medisinske journaler;
- Hørsels- eller synshemninger;
- mangel på dybdeoppfatning; og/eller
- i sorgperioden de siste 6 månedene.
Foreldre
Inklusjonskriterier:
- far eller mor som hovedomsorgsperson for barnet som oppfyller de nevnte utvalgskriteriene;
- er 21 år og eldre;
- Evne til å kommunisere muntlig og kunne lese engelsk; og
- ha tilgang til smarttelefon/nettbrett (eget eller leie fra prosjektet).
Ekskluderingskriterier:
- har nedsatt syn/hørsel;
- har noen kognitiv svikt/psykisk lidelse inkludert angstlidelse;
- har sluttstadiet av sykdommer; og/eller
- i sorgperioden de siste 6 månedene.

Eksperimentell: Intervensjonsgruppe
Deltakerne i intervensjonsgruppen vil motta den rutinemessige behandlingen pluss ICory-Circumcision som er spesielt utviklet for denne studien basert på forstudier, annen litteratur, og viktigst av alt, den kirurgiske veien som for tiden praktiseres på studiesykehuset.
ICory-Circumcision-programmet har to komponenter: (1) Buddy Healthcare-mobilappen (BuddyCare) som gir en omfattende daglig perioperativ veiledning for foreldre angående deres barns kirurgi med et grensesnitt for helsepersonell for å overvåke foreldres og deres barns behov samt kommunisere med dem.
(2) Triumf Health-mobilspillappen som gir emosjonell støtte og distraksjon til barn.

Den rutinemessige behandlingen består av vanlige konsultasjoner med legen, preoperativ forberedelse og postoperativ behandling.
Ved første konsultasjon vil barnet bli vurdert for behov for omskjæring og legen forklarer muntlig til foreldrene om prosedyre, risiko, hva man kan forvente og forberede seg preoperativt.

Deltakerne i kontrollgruppen vil motta rutinemessig behandling gitt av sykehuset som består av vanlig legekonsulent, preoperativ forberedelse og postoperativ behandling.

Ved første konsultasjon vil barnet bli vurdert for behov for omskjæring og legen forklarer muntlig til foreldrene om prosedyre, risiko, hva man kan forvente og forberede seg preoperativt.

Foreldres preoperative angst og informasjonsbehov
Tidsramme: Foreldres preoperative angst og informasjonsbehov vil bli vurdert ved baseline (ca. 2 uker før operasjon) og vil vurdere endring mellom baseline og operasjonsdagen før operasjonen.

Amsterdam Preoperative Anxiety and Information Scale (APAIS) (Moerman, van Dam, Muller & Oosting, 1996) vil bli brukt til å måle foreldres preoperative angst og informasjonsbehov.
APAIS er et spørreskjema på seks punkter som består av 4 spørsmål om frykt for anestesi og prosedyre mens 2 spørsmål handler om behovet for informasjon.
Spørreskjemaet skåres basert på en likert-skala fra 1 til 5. En poengsum på 1 ville bety 'ikke i det hele tatt' mens 5 ville bety 'ekstremt'.
Den totale poengsummen for angstskalaen kan variere fra 4 til 20 mens behovet for informasjonsskala varierer fra 2 til 10.
Jo høyere poengsum, desto høyere angst og informasjon kreves av respondentene.

Det egenutviklede Parents' Perioperative Knowledge Questionnaire (PPKQ) vil bli brukt til å vurdere foreldres perioperative kunnskap med hensyn til omskjæring.
Den ble utviklet av studieteamet.
En innholdsvaliditet vil bli gjort av et ekspertpanel bestående av 5-10 eksperter (Gilbert & Prion, 2016).
PPKQ inneholder 10 flervalgsspørsmål.
Jo høyere antall spørsmål besvart riktig ville indikere en høyere forståelse av forståelsen av den perioperative prosessen og dermed bedre kunnskap.

Et egenutviklet Parents' Self-Efficacy in Children's Perioperative Care (PSeCPC) spørreskjema vil bli brukt for å måle foreldres egeneffektivitet i barnepass gjennom hele den perioperative prosessen.
Det er 8 elementer for denne skalaen og hvert element er vurdert på fire svar: 1= ikke selvsikker i det hele tatt, 2= litt selvsikker, 3= moderat selvsikker, 4=ganske sikker og 5= ekstremt selvsikker.
Totalscore er 8-40.

Skalaen består kun av ett element og det skåres på en 6-punkts ordinær skala fra 1 for «Veldig misfornøyd» til en poengsum på 6 for «Svært fornøyd».
Den totale poengsummen kan variere fra 1 til 6.

Barns preoperative angst: Modifisert Yale Preoperative Anxiety Scale - Short Form (mYPAS-SF)
Tidsramme: Barns preoperative angst vil bli vurdert dagen rett før operasjonen.

Modifisert Yale Preoperative Anxiety Scale - Short Form (mYPAS-SF) (Jenkins, Fortier, Kaplan, Mayes & Kain, 2014) vil bli brukt for å måle preoperativ angstnivå hos barna rett før operasjonen.
Det er en 4-elements skala (aktivitet, vokalisering, emosjonell ekspressivitet, tilstand av tilsynelatende opphisselse) og den fylles ut via observasjon.
Den totale poengsummen varierer fra 23 til 100.
Høyere score betyr høyere nivåer av angst.
En poengsum på 31 og over vil indikere høy angst (Kain et al., 1997; Rema, Cavaleiro, Pina & Machado, 2016).

Tidsramme: Barns postoperative smerter vil bli målt 24 timer etter operasjonen.

Faces Pain Scale - Revised (FPS-R) (International Association for the Study of Pain [IASP], 2001) vil bli brukt for å vurdere postoperativ smerte hos barna.
FPS-R viser seks ansikter med forskjellige ansiktsuttrykk og barnet får instruksjoner om å peke på ansiktet tilsvarende mengden smerte han føler i det øyeblikket.
Skårene varierer fra 0-10 med en høyere score betyr større smerte.

En semistrukturert intervjuguide vil bli brukt til å intervjue foreldrene og barna i intervensjonsgruppen (mål 15-30) og helsepersonell som brukte ICory-Circumcision.
Foreldre og barn vil bli spurt om innholdet, aktiviteter og leveringsmetoder, og nytten av applikasjonen.
Helsepersonell vil bli spurt om styrker og svakheter ved søknaden og deres mening om søknaden fra deres perspektiv.
Dette er en kvalitativ data og dataene vil bli analysert ved tematisk analyse.
Det er derfor ingen poengsum for denne evalueringen.
