Doelstellingen: De doelstellingen van dit onderzoek zijn: (1) het ontwikkelen van een intelligente klantgerichte oplossing voor kinderchirurgische zorg voor ouders van kinderen die een besnijdenis ondergaan (ICory-Circumcision) en hun kinderen; (2) om de effectiviteit van de ICory-Besnijdenis te onderzoeken op de uitkomsten van ouders (zelfeffectiviteit in kinderopvang, peri-operatieve kennis en tevredenheid in peri-operatieve zorg, behoefte aan informatie en angst) en kinderen (preoperatieve angst en postoperatieve pijn), als evenals no-show/vertraagd verschijnen voor operatiegevallen en gebruik van gezondheidsdiensten; en (3) de percepties van gebruikers (ouders, kinderen, zorgverleners) van de interventie en suggesties voor verbetering onderzoeken.
Onderzoeksopzet: Dit onderzoek bestaat uit een pretest met twee groepen en een herhaalde posttest pilot gerandomiseerde gecontroleerde trial, gevolgd door kwalitatieve procesevaluatie. In totaal worden 80 duo's van ouders en hun kinderen in de basisschoolleeftijd (7-12 jaar oud) willekeurig toegewezen aan de interventiegroep die de ICory-Circumcision plus routinezorg krijgt, waarbij de ouders de BuddyCare-applicatie ontvangen terwijl de kind krijgt de Triumf-game-applicatie en de controlegroep, die alleen de reguliere zorg van het ziekenhuis krijgt. Er zal een interviewgids worden ontwikkeld en gebruikt om de ouders en zorgverleners te interviewen die ICory-Circumcision hebben gebruikt om hun ervaringen en meningen over sterke en zwakke punten en verbeterpunten te verkennen. Ook de kinderen die de Triumf-game hebben gebruikt, worden geïnterviewd om hun ervaringen met de game-applicatie te verkennen.
Hypothese: In vergelijking met de controlegroep zullen (1) ouders in de interventiegroep die de ICory-Circumcision ondergaan een significant (a) hoger niveau van zelfredzaamheid in de kinderopvang rapporteren; perioperatieve kennis en tevredenheid (b) lagere behoefte aan informatie en preoperatieve angst. (2) De kinderen in de interventiegroep zullen een significant lager niveau van preoperatieve angst en postoperatieve pijn rapporteren. (3) De interventiegroep zal minder gevallen hebben van no-show/vertraagd verschijnen voor een operatie en minder gebruik maken van gezondheidsdiensten (bijv. minder SEH of huisartsbezoek om niet-medische redenen) binnen 2 weken na de operatie.

Kinderen
Inclusiecriteria:
- leeftijd tussen 7-12 jaar oud;
- is gepland voor een electieve besnijdenisoperatie;
- Kan Engels spreken en lezen;
- wordt vergezeld door hun ouders (moeder of vader of beide) tijdens de perioperatieve periode; En
- heeft toegang tot smartphone/tablet (eigen of huur van het project).
Uitsluitingscriteria:
- cognitieve en leerstoornissen geïdentificeerd uit het medisch dossier;
- een chronische ziekte en/of pijn waarvoor speciale medische zorg nodig was;
- angststoornis en andere stemmingsstoornissen zoals vastgesteld in hun medische dossiers;
- Gehoor- of visuele beperkingen;
- dieptewaarnemingstekorten; en/of
- in de rouwperiode in de afgelopen 6 maanden.
Ouders
Inclusiecriteria:
- vader of moeder als hoofdverzorger van het kind die voldoen aan voornoemde selectiecriteria;
- 21 jaar en ouder bent;
- Mondeling kunnen communiceren en Engels kunnen lezen; En
- toegang hebben tot smartphones/tablets (eigen of gehuurd van het project).
Uitsluitingscriteria:
- een visuele / auditieve beperking hebben;
- een cognitieve stoornis/mentale stoornis hebben, waaronder een angststoornis;
- eindstadium van ziekten hebben; en/of
- in de rouwperiode in de afgelopen 6 maanden.

Experimenteel: Interventie groep
De deelnemers aan de interventiegroep krijgen de routinezorg plus de ICory-Circumcision die speciaal voor dit onderzoek is ontwikkeld op basis van voorbereidende studies, andere literatuur en, belangrijker nog, het chirurgische pad dat momenteel wordt toegepast in het onderzoeksziekenhuis.
Het ICory-Circumcision-programma bestaat uit twee componenten: (1) de Buddy Healthcare mobiele app (BuddyCare) die een uitgebreide dagelijkse peri-operatieve gids biedt voor ouders met betrekking tot de operatie van hun kind met een interface voor zorgprofessionals om de ouders en hun behoeften van kinderen en communiceer met hen.
(2) De Triumf Health mobiele game-app die kinderen emotionele steun en afleiding biedt.

De ICory-Circumcision is speciaal voor deze studie ontwikkeld op basis van voorstudies, andere literatuur en, belangrijker nog, de chirurgische route die momenteel in het onderzoeksziekenhuis wordt toegepast.
Het ICory-Circumcision-programma bestaat uit twee componenten: (1) de Buddy Healthcare mobiele app (BuddyCare) die een uitgebreide dagelijkse peri-operatieve gids biedt voor ouders met betrekking tot de operatie van hun kind met een interface voor zorgprofessionals om de ouders en hun behoeften van kinderen en communiceer met hen.
(2) De Triumf Health mobiele game-app die kinderen emotionele steun en afleiding biedt.
Het ICory-Circumcision-programma is ontworpen om door een zelfgeleide en door een zorgprofessional geleid te worden.
Deelnemers hebben toegang tot BuddyCare voor ouders en Triumf Health vanaf het moment van werving tot 2 weken na de operatie, terwijl de zorgprofessional face-to-face training krijgt over het gebruik van het BuddyCare-dashboard.
De routinematige zorg bestaat uit normaal overleg met de arts, preoperatieve voorbereiding en postoperatieve zorg.
Tijdens het eerste consult wordt het kind beoordeeld op de noodzaak van besnijdenis en legt de arts mondeling uit aan de ouders over de procedure, risico's, wat te verwachten en voorbereiding preoperatief.

Actieve vergelijker: Controlegroep
De deelnemers aan de controlegroep krijgen standaardzorg van het ziekenhuis, bestaande uit normale doktersconsultatie, preoperatieve voorbereiding en postoperatieve zorg.

De routinematige zorg bestaat uit normaal overleg met de arts, preoperatieve voorbereiding en postoperatieve zorg.
Tijdens het eerste consult wordt het kind beoordeeld op de noodzaak van besnijdenis en legt de arts mondeling uit aan de ouders over de procedure, risico's, wat te verwachten en voorbereiding preoperatief.

Preoperatieve angst en informatiebehoefte van ouders
Tijdsspanne: De preoperatieve angst en informatiebehoefte van de ouders wordt beoordeeld bij baseline (ongeveer 2 weken voor de operatie) en beoordeelt de verandering tussen baseline en op de operatiedag vóór de operatie.

De Amsterdam Preoperatieve Angst- en Informatieschaal (APAIS) (Moerman, van Dam, Muller & Oosting, 1996) wordt gebruikt om de preoperatieve angst- en informatiebehoefte van ouders te meten.
De APAIS is een vragenlijst met zes items die bestaat uit 4 vragen over de angst voor anesthesie en de procedure, terwijl 2 vragen over de behoefte aan informatie gaan.
De vragenlijst wordt gescoord op een Likert-schaal van 1 tot 5. Een score van 1 betekent 'helemaal niet' en 5 betekent 'extreem'.
De totaalscore voor de angstschaal kan variëren van 4 tot 20, terwijl de behoefte aan informatieschaal varieert van 2 tot 10.
Hoe hoger de scores, hoe hoger de angst en de informatiebehoefte van de respondenten.

De preoperatieve angst en informatiebehoefte van de ouders wordt beoordeeld bij baseline (ongeveer 2 weken voor de operatie) en beoordeelt de verandering tussen baseline en op de operatiedag vóór de operatie.

Perioperatieve kennis van ouders
Tijdsspanne: De peri-operatieve kennis van de ouders wordt beoordeeld bij baseline (ongeveer 2 weken voor de operatie) en ze beoordelen de verandering tussen baseline en post-interventie 2 weken na de operatie.

De zelfontwikkelde Ouders Perioperatieve Kennis Vragenlijst (PPKQ) wordt gebruikt om de perioperatieve kennis van ouders met betrekking tot de besnijdenis te peilen.
Het is ontwikkeld door het onderzoeksteam.
Een inhoudsvaliditeit wordt gedaan door een expertpanel bestaande uit 5-10 experts (Gilbert & Prion, 2016).
De PPKQ bevat 10 meerkeuzevragen.
Het hogere aantal correct beantwoorde vragen zou duiden op een beter begrip van het begrip van het peri-operatieve proces en dus op een betere kennis.

De peri-operatieve kennis van de ouders wordt beoordeeld bij baseline (ongeveer 2 weken voor de operatie) en ze beoordelen de verandering tussen baseline en post-interventie 2 weken na de operatie.

Zelfredzaamheid van ouders in de kinderopvang
Tijdsspanne: De zelfredzaamheid van de ouders wordt beoordeeld bij baseline (ongeveer 2 weken voor de operatie) en zal de verandering tussen baseline en post-interventie 2 weken na de operatie beoordelen.

Een zelfontwikkelde vragenlijst over de zelfredzaamheid van ouders in de perioperatieve zorg voor kinderen (PSeCPC) zal worden gebruikt om de zelfredzaamheid van ouders in de kinderopvang gedurende het hele perioperatieve proces te meten.
Er zijn 8 items voor deze schaal en elk item wordt gescoord op vier antwoorden: 1= helemaal geen vertrouwen, 2= een beetje vertrouwen, 3= matig vertrouwen, 4=redelijk vertrouwen en 5= zeer veel vertrouwen.
De totale score is 8-40.
Een hogere score betekent meer zelfredzaamheid.

Tevredenheid van ouders over perioperatieve zorg
Tijdsspanne: De tevredenheid van de ouders over de perioperatieve zorg wordt ongeveer 2 weken na de operatie na de operatie beoordeeld.

Een 6-punts Ordinale Beschrijvende Schaal - Tevredenheid met Perioperatieve Zorg (ODS-SPC) zal worden gebruikt om de tevredenheid van ouders met ICory-Circumcision en routinematige zorg te beoordelen.
De schaal bestaat uit slechts één item en wordt gescoord op een 6-punts ordinale schaal van 1 voor "Zeer ontevreden" tot een score van 6 voor "Zeer tevreden".
De totaalscore kan variëren van 1 tot 6.
Een hogere score zou duiden op een hogere tevredenheid over de perioperatieve zorg.

De tevredenheid van de ouders over de perioperatieve zorg wordt ongeveer 2 weken na de operatie na de operatie beoordeeld.

Preoperatieve angst bij kinderen: Modified Yale Preoperative Anxiety Scale - Short Form (mYPAS-SF)
Tijdsspanne: De preoperatieve angst van kinderen wordt de dag voor de operatie beoordeeld.
|
Modified Yale Preoperative Anxiety Scale - Short Form (mYPAS-SF) (Jenkins, Fortier, Kaplan, Mayes & Kain, 2014) zal worden gebruikt om de preoperatieve angstniveaus van de kinderen vlak voor de operatie te meten.
Het is een schaal met 4 items (activiteit, vocalisatie, emotionele expressiviteit, staat van schijnbare opwinding) en wordt ingevuld via observatie.
De totale score varieert van 23 tot 100.
Hogere scores betekenen meer angst.
Een score van 31 en hoger zou wijzen op hoge angst (Kain et al., 1997; Rema, Cavaleiro, Pina & Machado, 2016).

Postoperatieve pijn bij kinderen: de Faces Pain Scale
Tijdsspanne: De postoperatieve pijn bij kinderen wordt 24 uur na de operatie gemeten.
|
De Faces Pain Scale - Revised (FPS-R) (International Association for the Study of Pain [IASP], 2001) zal worden gebruikt om de postoperatieve pijn van de kinderen te beoordelen.
De FPS-R toont zes gezichten met verschillende gezichtsuitdrukkingen en het kind krijgt instructies om naar het gezicht te wijzen dat overeenkomt met de hoeveelheid pijn die het op dat moment voelt.
De scores variëren van 0-10, waarbij een hogere score meer pijn betekent.

De postoperatieve pijn bij kinderen wordt 24 uur na de operatie gemeten.
|

Gezondheidsdiensten gebruiken
Tijdsspanne: Het gebruik van de gezondheidszorg wordt ongeveer 2 weken na de operatie beoordeeld.

Het gebruik van gezondheidsdiensten zal worden beoordeeld door middel van de Healthcare Services Utilization Questionnaire (HSUQ) om na ontslag informatie vast te leggen over het gebruik van middelen voor elke deelnemer, b.v.
spoedeisende hulp en ongepland medisch consult bij huisarts/privékliniek.
Het minimale servicegebruik zal nul zijn, maar er is geen maximaal begrensd.
De hogere waarde van het gebruik van diensten duidt op een slechter resultaat.

Het gebruik van de gezondheidszorg wordt ongeveer 2 weken na de operatie beoordeeld.
|

Proces evaluatie
Tijdsspanne: Procesevaluatie vindt ongeveer 2 weken na de operatie plaats.
|
Procesevaluatie zal worden geëvalueerd door middel van semi-gestructureerde interviews.
Er zal een semi-gestructureerde interviewgids worden gebruikt om de ouders en kinderen in de interventiegroep (gericht op 15-30 jaar) en zorgverleners die ICory-Circumcision hebben gebruikt, te interviewen.
Ouders en kinderen zullen worden gevraagd naar de inhoud, activiteiten en leveringsmethoden en het nut van de applicatie.
Zorgprofessionals worden gevraagd naar de sterke en zwakke punten van de applicatie en hun mening over de applicatie vanuit hun perspectief.
Dit zijn kwalitatieve gegevens en de gegevens zullen worden geanalyseerd door middel van thematische analyse.
Er is dus geen score voor deze evaluatie.
