Familiens sikkerhetsnett: Utvikling av en oppstrøms selvmordsforebyggende tilnærming for å oppmuntre til sikker oppbevaring av skytevåpen i landlige og avsidesliggende områder
Målet med denne kliniske studien er å teste intervensjonen Family Safety Net (FSN) i en primært Alaska-innfødt befolkning i NW Alaska. Hovedspørsmålene den tar sikte på å besvare er:
• Er Family Safety Net (FSN) intervensjon gjennomførbar og akseptabel i en primært Alaska innfødt befolkning i Nordvest-Alaska.
Intervensjonsdeltakere vil fullføre:
- Screeningundersøkelse - Hver husstand vil motta en kupong som en voksen kan ta med tilbake for å ta screeningundersøkelsen og bli med i trekningen.
- Baseline-undersøkelser – Vi vil gjennomføre undersøkelser med personer med minst én pistol i hjemmet og som svarer «ja» på ett av våre syv spørsmål om psykisk helse om noen andre i hjemmet:
- Kort motivasjonsintervjuøkt (MI) – Deltakerne vil motta en 20-minutters økt utført av opplært forskningspersonell fra University of Michigan for å oppmuntre til sikker lagring av skytevåpen i deltakernes hjem.
- Tilbyr materialer å ta med hjem - Etter den korte MI-økten vil skytevåpensikkerhetsartikler tilbys, inkludert avtrekkerlåser, kabellåser og ammunisjonsbokser. I tillegg vil elementer som fremmer mental helse også være tilgjengelig, inkludert Careline-magneter, pedagogisk materiale, flygeblader for kommende foreldrekurs, etc.
- 4-ukers tekstmeldinger – Deltakerne vil motta SMS-påminnelser, og oppmuntring vil bli skreddersydd av undersøkelsessvar og levert mellom 2x og 4X ukentlig avhengig av baseline-undersøkelsens svar.
- 1-måneders oppfølging Qualtrics-undersøkelsen - Oppfølgingsundersøkelsen består av baseline-undersøkelsen + 9 tilfredshetsspørsmål om MI-øktene, materiell hentet hjem, mottatte tekstmeldinger og handlinger som er utført
Forskere vil sammenligne personer fra samme populasjon for å se om husholdningspraksisen for oppbevaring av skytevåpen endres på grunn av MI-intervensjonen og tilgang til skytevåpensikkerhetsutstyr.

Målet med denne kliniske studien er å teste intervensjonen Family Safety Net (FSN) i en primært Alaska-innfødt befolkning i NW Alaska. Hovedspørsmålene den tar sikte på å besvare er:
- Er Family Safety Net (FSN) intervensjon gjennomførbar og akseptabel i en primært Alaska innfødt befolkning i NW Alaska.
- En voksen per husstand i et fellesskap vil fylle ut et veldig kort spørreskjema for å sjekke om de er kvalifisert til å delta i studien. Ved å komme til lokalsamfunnet og fylle ut en 5-minutters screeningundersøkelse, vil hver deltaker bli med i en loddtrekning for å motta en trommel på 500 dollar med fyringsolje. Alle som kommer med en rekrutteringskupong vil ta en 5-minutters Qualtrics-undersøkelse på iPad. Screeningsundersøkelsen søker å identifisere de som oppfyller våre kvalifikasjonskriterier.
- Screeningsundersøkelsen vil lenke til et samtykkeskjema med en unik identifikator dersom personen er kvalifisert til å delta i studien. Forskningsmedarbeidere vil bekrefte kvalifikasjonskriterier og gå gjennom samtykkeskjemaet på iPad og svare på deltakerens spørsmål. Hvis de godtar å delta, vil de markere sitt samtykke og bli tilfeldig tildelt sammenlignings- eller intervensjonsgruppen.
- Når folk godtar å delta i en av gruppene, vil de bli bedt om å fullføre en 10-15-minutters grunnlinjeundersøkelse uavhengig på iPad.
- Etter at deltakerne har fullført grunnlinjeundersøkelsen, vil intervensjonisten legge til rette for det 10-15 minutter lange baseline-strukturerte intervjuet.
For intervensjonsgruppedeltakere vil de delta i en kort 20-30-minutters motiverende intervjuøkt (MI) som inkluderer:
- introduksjon til sesjonen og sette dagsorden;
- diskutere personens mål og styrker knyttet til å holde sine kjære trygge gjennom sikker oppbevaring av skytevåpen (f. gjøre hjemmet tryggere for å beskytte ungdommen eller annen familie som bor der) og støtte velvære (dvs. fremme av psykisk helse);
- å tilby normative tilbakemeldinger om sikker oppbevaring av skytevåpen i samfunnet;
- snakke om fordelene med tryggere våpenoppbevaring (f.eks. forbedre ungdomssikkerheten eller unngå ungdomsskade);
- handlingsplanlegging for å øke selveffektiviteten for å overvinne barrierer og bevege seg mot å redusere tilgangen til skytevåpen ved å låse og losse alle husholdningsvåpen;
- å tilby ideer om viktigheten av å støtte andre på "ikke-krevende" måter;
- Handlingsplanlegging for å brainstorme måter å støtte andre i husholdningen på på ikke-krevende måter;
- velge ut trygge lagringsenheter for å ta med hjem som "signaler for handling" (f.eks. magneter, utløserlåser, kabellåser, medisinbokser osv.); og
- velge ut velværeressurser (f.eks. magneter for mental helse, You Matter-kort, søtsaker å dele, lokal ressursinformasjon, etc.);
oppsummering av nøkkelinnholdet i økten for å styrke fordeler, mål og verdier og støtte selveffektivitet for å legge til rette for trygg skytevåpenoppbevaring og velværestøtte hjemme. Avhengig av deltakerens interesser og mål, vil denne MI-økten fokusere på sikker oppbevaring av skytevåpen, støtte andre i husholdningens mentale velvære eller begge deler.
- Motta 4-ukers tekstmeldinger skreddersydd etter undersøkelsessvarene deres (1x til 4x ukentlig avhengig av baseline-undersøkelsen og baseline-strukturerte intervjusvar). De kan velge bort når som helst.
- En måned etter den første økten, for både sammenlignings- og intervensjonsgrupper, vil en lenke bli sendt til deltakerne via tekst/e-post for å fullføre en 10-15-minutters (avhengig av hopplogikk) oppfølgingsundersøkelse. Sammenligningsgruppe:

Eksperimentell: FSN Lethal Means Scripted Intervention
Participants were randomized to one of two lethal-means reduction-focused FSN programs if they answer "yes" to any of the screening questions.
The scripted FSN intervention addresses the connection between a loved one's mental health and the participant's home firearm and medication storage, focusing on what the participant could do to reduce the risk of suicide for a particular loved one in their home.
Using a handout depicting a typical home, participants review where firearms, ammunition, and medications are stored, reinforcing that "10 minutes can save a life" and encouraging the display of mental health resources.
Participants receive safe firearm storage materials and #988 crisis line magnets and stickers.
They also receive 1-4 weeks of tailored automated text messages based on baseline responses (e.g., presence of children and storage practices).

Eksperimentell: Experimental: FSN Lethal Means MI Counseling
Participants will be screened into one of two lethal-means reduction-focused FSN programs if they answer "yes" to one or more of the screening questions.
The MI FSN intervention addresses the connection between a loved one's mental health and the participant's home firearm and medication storage, focusing on what the participant could do to reduce the risk of suicide for a particular loved one in their home.
The MI session asked broad open-ended questions about what that person might do to increase environmental safety in their home.
At the end of the session, participants were offered firearm safe storage resources to take home, as well as magnets and stickers with #988, the national crisis line.

Household Firearm Storage

We developed a firearm inventory to assess safety practices for up to five firearms in the home.
For each reported firearm, participants indicated whether: (1) the firearm was locked with a cable or trigger lock or stored in a locked gun safe or cabinet; (2) the firearm was unloaded; (3) ammunition was stored separately from the firearm; and (4) the ammunition was locked in an ammunition box.
Each safe storage practice endorsed was assigned one point, yielding a score of 0-4 per firearm.
Scores were then averaged across all reported firearms to create a composite safe storage score.
If a participant reported that a firearm named on the baseline was no longer in the home at follow-up, they were asked if they had moved the firearm out of the house for safety reasons.
If they removed a firearm, they were not asked the safe storage questions for that firearm, and it was not calculated in the total composite score.
