Family Safety Net - Doel 3: gerandomiseerde controleproef (FSN)
Family Safety Net: ontwikkeling van een upstream-aanpak voor zelfmoordpreventie om veilige opslag van vuurwapens op het platteland en afgelegen te stimuleren
Het doel van deze klinische studie is om de Family Safety Net (FSN)-interventie te testen bij een voornamelijk inheemse bevolking van Alaska in NW Alaska. De belangrijkste vraag [en] die het probeert te beantwoorden zijn:
• Is de interventie van het Family Safety Net (FSN) haalbaar en acceptabel bij een voornamelijk inheemse bevolking van Alaska in Noordwest-Alaska?
Deelnemers aan de interventie voltooien:
- Screeningsenquête - Elk huishouden ontvangt één voucher die een volwassene mag meebrengen om de screeningsenquête in te vullen en mee te doen aan de loterij.
- Baseline-enquêtes - We zullen enquêtes houden bij personen die ten minste één wapen in huis hebben en die "ja" antwoorden op een van onze zeven vragen over geestelijke gezondheid over iemand anders in hun huis:
- Korte motiverende gesprekssessie (MI) - Deelnemers krijgen een sessie van 20 minuten, uitgevoerd door getraind onderzoekspersoneel van de Universiteit van Michigan om veilige opslag van vuurwapens in de huizen van de deelnemers aan te moedigen.
- Materiaal aanbieden om mee naar huis te nemen - Na de korte MI-sessie worden vuurwapenveiligheidsartikelen aangeboden, waaronder trekkersloten, kabelsloten en munitiekisten. Daarnaast zullen er ook artikelen beschikbaar zijn die de geestelijke gezondheid bevorderen, waaronder Careline-magneten, educatief materiaal, flyers voor aankomende ouderschapslessen, enz.
- 4 weken aan sms-berichten - Deelnemers ontvangen sms-herinneringen en aanmoediging wordt op maat gemaakt door enquêteantwoorden en wordt tussen de 2x en 4x per week afgeleverd, afhankelijk van de baseline-enquêtereacties.
- Follow-up Qualtrics-enquête van 1 maand - De follow-up-enquête bestaat uit de baseline-enquête + 9 tevredenheidsvragen over de MI-sessies, meegebrachte materialen, ontvangen sms-berichten en ondernomen acties
Onderzoekers zullen mensen uit dezelfde populatie vergelijken om te zien of de opslagpraktijken voor huishoudelijke vuurwapens veranderen vanwege de MI-interventie en toegang tot vuurwapenveiligheidsapparatuur.

- Follow-up Qualtrics-enquête van 1 maand - De follow-up-enquête bestaat uit de baseline-enquête + 9 tevredenheidsvragen over de MI-sessies, meegebrachte materialen, ontvangen sms-berichten en ondernomen acties

Het doel van deze klinische studie is om de Family Safety Net (FSN)-interventie te testen bij een voornamelijk inheemse bevolking van Alaska in NW Alaska. De belangrijkste vraag [en] die het probeert te beantwoorden zijn:
- Is de interventie van het Family Safety Net (FSN) haalbaar en acceptabel in een voornamelijk inheemse bevolking van Alaska in NW Alaska.
- Eén volwassene per huishouden binnen een gemeenschap zal een zeer korte vragenlijst invullen om te controleren of ze in aanmerking komen voor deelname aan het onderzoek. Door naar de gemeenschapslocatie te komen en een screeningenquête van 5 minuten in te vullen, doet elke deelnemer mee aan een loterij om een vat stookolie van $ 500 te ontvangen. Iedereen die met een wervingsvoucher komt, vult op de iPad een Qualtrics-enquête van 5 minuten in. De screeningsenquête probeert degenen te identificeren die voldoen aan onze geschiktheidscriteria.
- De screeningsenquête zal linken naar een toestemmingsformulier met een unieke identificatie als de persoon in aanmerking komt om deel te nemen aan het onderzoek. Onderzoeksmedewerkers bevestigen de geschiktheidscriteria, doorlopen het toestemmingsformulier op de iPad en beantwoorden de vragen van de deelnemer. Als ze ermee instemmen om deel te nemen, markeren ze hun toestemming en worden ze willekeurig toegewezen aan de vergelijkings- of interventiegroep.
- Wanneer mensen ermee instemmen om aan een van beide groepen deel te nemen, wordt hen gevraagd om zelfstandig een 10-15 minuten durende basisenquête op de iPad in te vullen.
- Nadat de deelnemers de baseline-enquête hebben voltooid, faciliteert de interventionist het 10-15 minuten durende baseline-gestructureerde interview.
Deelnemers aan de interventiegroep zullen deelnemen aan een korte motiverende gesprekssessie (MI) van 20-30 minuten, die het volgende omvat:
- inleiding tot de sessie en vaststelling van de agenda;
- het bespreken van de doelen en sterke punten van de persoon met betrekking tot het veilig houden van dierbaren door middel van veilige opslag van vuurwapens (bijv. het veiliger maken van iemands huis om de jeugd of andere familie die daar woont te beschermen) en het ondersteunen van welzijn (d.w.z. bevordering van de geestelijke gezondheid);
- het bieden van normatieve feedback over veilige opslagpraktijken voor vuurwapens binnen de gemeenschap;
- praten over de voordelen van veiligere opslagpraktijken voor vuurwapens (bijv. het verbeteren van de veiligheid van jongeren of het voorkomen van schade door jongeren);
- actieplanning om de zelfredzaamheid te vergroten om barrières te overwinnen en om de toegang tot vuurwapens te verminderen door alle huishoudelijke wapens te vergrendelen en te ontladen;
- ideeën aanreiken over het belang van het ondersteunen van anderen op 'niet-veeleisende' manieren;
- Actieplanning om te brainstormen over manieren om anderen in het huishouden op niet veeleisende manieren te ondersteunen;
- veilige opslagapparaten uitkiezen om mee naar huis te nemen als 'aanwijzingen voor actie' (bijv. Magneten, trekkersloten, kabelsloten, medicijndozen, enz.); en
- wellnessbronnen uitkiezen (bijv. Magneten voor geestelijke gezondheid, You Matter-kaarten, snoepjes om te delen, informatie over lokale bronnen, enz.);
een samenvatting van de belangrijkste inhoud van de sessie om voordelen, doelen en waarden te versterken en zelfredzaamheid te ondersteunen om veilige opslagpraktijken voor vuurwapens en welzijnsondersteuning thuis te vergemakkelijken. Afhankelijk van de interesses en doelen van de deelnemer, zal deze MI-sessie zich richten op het veilig opbergen van vuurwapens, het ondersteunen van anderen bij het geestelijk welzijn van hun huishouden of beide.
- Ontvang 4 weken lang sms-berichten die zijn afgestemd op hun antwoorden op de enquête (1x tot 4x per week, afhankelijk van de baseline-enquête en de baseline-antwoorden op gestructureerde interviews). Ze kunnen zich op elk moment afmelden.
- Een maand na de eerste sessie wordt voor zowel de vergelijkings- als de interventiegroep een link naar de deelnemers gestuurd via sms/e-mail om een vervolgenquête van 10-15 minuten (afhankelijk van de skip-logica) in te vullen. Het vervolgonderzoek bestaat uit items die verband houden met de opslag van vuurwapens en faciliterende factoren waarvan wordt verondersteld dat ze bijdragen aan dit gedrag. Het onderzoek omvat items gerichte veranderingsmechanismen, waaronder zelfeffectiviteit, en op de huidige huishoudelijke wapenopslag (# en type vuurwapen; vergrendeld / ontgrendeld, geladen / gelost en locatie van munitie voor elk) en acties die zijn ondernomen om mentaal welzijn te ondersteunen voor anderen in hun huis. Bovendien zullen degenen die de FSN hebben gedaan ook vragen beantwoorden over hun tevredenheid met de FSN (aanvaardbaarheid, culturele responsiviteit). Voor zowel vergelijkings- als interventiegroepen zal de follow-up een "momentopname" vastleggen van de huidige opslagpraktijken voor huishoudelijke vuurwapens die dag. Bovendien krijgen deelnemers de mogelijkheid om de vervolgenquête persoonlijk op een iPad met het onderzoeksteam in te vullen en worden ze van tevoren geïnformeerd over dag, tijd en locatie waar het onderzoeksteam beschikbaar zal zijn.
Onderzoekers zullen mensen uit dezelfde populatie vergelijken om te zien of de opslagpraktijken voor huishoudelijke vuurwapens veranderen vanwege de MI-interventie en toegang tot vuurwapenveiligheidsapparatuur.
Vergelijkingsgroep:
- Wanneer mensen ermee instemmen om deel te nemen en willekeurig worden toegewezen aan de vergelijkingsgroep, wordt hen gevraagd om zelfstandig op de iPad een nulmeting van 10-15 minuten in te vullen.
- Nadat de deelnemers de baseline-enquête hebben voltooid, faciliteert de interventionist het 10-15 minuten durende baseline-gestructureerde interview.
- Neem deel aan een discussie over veiligheid in huis en neem een koolmonoxidedetector, een informatieblad over koolmonoxide, een pamflet voor wapenopslag en een magneet voor geestelijke gezondheidszorg mee naar huis.

Experimenteel: FSN Lethal Means Scripted Intervention
Participants were randomized to one of two lethal-means reduction-focused FSN programs if they answer "yes" to any of the screening questions.
The scripted FSN intervention addresses the connection between a loved one's mental health and the participant's home firearm and medication storage, focusing on what the participant could do to reduce the risk of suicide for a particular loved one in their home.
Using a handout depicting a typical home, participants review where firearms, ammunition, and medications are stored, reinforcing that "10 minutes can save a life" and encouraging the display of mental health resources.
Participants receive safe firearm storage materials and #988 crisis line magnets and stickers.
They also receive 1-4 weeks of tailored automated text messages based on baseline responses (e.g., presence of children and storage practices).

Household Firearm Storage

We developed a firearm inventory to assess safety practices for up to five firearms in the home.
For each reported firearm, participants indicated whether: (1) the firearm was locked with a cable or trigger lock or stored in a locked gun safe or cabinet; (2) the firearm was unloaded; (3) ammunition was stored separately from the firearm; and (4) the ammunition was locked in an ammunition box.
Each safe storage practice endorsed was assigned one point, yielding a score of 0-4 per firearm.
Scores were then averaged across all reported firearms to create a composite safe storage score.
If a participant reported that a firearm named on the baseline was no longer in the home at follow-up, they were asked if they had moved the firearm out of the house for safety reasons.
If they removed a firearm, they were not asked the safe storage questions for that firearm, and it was not calculated in the total composite score.
