Sjokkbølgeindusert demping av forkalkede plakk kvantifisert med OCT (SCALPO)

Målet med denne observasjonelle, prospektive, multisenterstudien er å utforske lokal virkning av koronar kalsiumfragmentering utøvet av endovaskulær litotripsi ved bilder tatt med OCT, hos pasienter med koronararterieforkalkninger ansvarlige for signifikant stenose, kandidat til PCI.
Hovedspørsmålet det tar sikte på å svare på, er Shockwave Systems evne til å redusere kalsiumtettheten i området av interesse (ROI) av karveggen.

Detaljert beskrivelse
Koronararterier med forkalkede lesjoner vil gjennomgå intravaskulær litotripsi (IVL) behandling; OCT-innsamling vil bli utført før IVL-behandling, etter IVL-behandling og etter stent-implantasjon.
Alle OCT-rammer vil bli lastet opp på bildebehandlings- og analyseprogrammet ImageJ (US National Institutes of Health).
ROI vil bli valgt som en del av bildet der kalsium er lokalisert.
Bildeanalyse vil bli utført ved hjelp av frekvenshistogram av pikseltetthet i ROI pre- og post-IVL, som et mål på kalsiumtetthet.
Gjennomsnittlig pikseltetthet for ROI før IVL vil bli sammenlignet med gjennomsnittlig pikseltetthet for ROI etter IVL, som måler deltapikseltetthet.

Alle pasienter i kateteriseringslaboratoriet som gjennomgår koronar angiografi, er potensielt selektert.
Beskrivelse
Inklusjonskriterier:
- Personer med nativ koronararteriesykdom (inkludert stabil eller ustabil angina og stille iskemi) egnet for PCI
- Venstre ventrikkel ejeksjonsfraksjon (LVEF) ≥35 % som målt før innmelding
- eGFR ≥45 ml/min (Cockroft-Gault, MDRD)
- Troponin må være mindre enn eller lik øvre grense for laboratorienormalverdi innen 24 timer før prosedyren, eller hvis troponin er forhøyet, må samtidig CK være normal
- Evne til å tolerere dobbel antiplate-behandling (dvs. aspirin og klopidogrel, prasugrel eller ticagrelor) i minst 6 måneder (for pasienter som ikke bruker oral antikoagulasjon) og enkelt blodplatehemmende behandling for livet
- Mållesjonen må være en de novo koronarlesjon som ikke tidligere har blitt behandlet med noen intervensjonsprosedyre
- Enkel de novo mållesjonsstenose av LAD, RCA eller LCX (eller av deres grener) med stenose på ≥70 % og <100 % eller stenose ≥50 % og <70 % (visuelt vurdert) med tegn på iskemi via positiv stresstest, eller fraksjonert strømningsreserveverdi ≤0,80, eller iFR <0,89 eller IVUS eller OCT minimum lumenareal ≤4,0 mm²
- Referansekar må ha 2,5 mm - 4,0 mm diameter og ≤ 30 mm lengde
- Bevis på forkalkning på lesjonsstedet vurdert ved angiografi, med fluoroskopiske radioopaciteter notert uten hjertebevegelse før kontrastinjeksjon som involverer begge sider av arterieveggen på minst ett sted og total lengde av kalsium på minst 15 mm og strekker seg delvis inn i mållesjonen, eller ved IVUS eller OCT, med tilstedeværelse av ≥270 grader kalsium på minst 1 tverrsnitt
- Planlagt behandling av enkeltlesjon for hvert målkar
- Evne til å føre en 0,014" guidetråd over lesjonen
- Målfartøyet må ha en TIMI flow 3 ved baseline
Ekskluderingskriterier:
- Venstre ventrikkel ejeksjonsfraksjon (LVEF) <35 %
- Personen er gravid eller ammer
- eGFR <45 ml/min (Cockroft-Gault, MDRD) eller kronisk dialyse
- Ubehandlet pre-prosedyre hemoglobin <9 g/dL eller intensjon om å nekte blodoverføring dersom det skulle bli nødvendig
- Pasienten har et blodplateantall <100 000 celler/mm3 eller >750 000 celler/mm3
- Pasienten har en kjent allergi mot protokollpåkrevde medisiner (klopidogrel, tienopyridiner, aspirin, kontrastmidler) som ikke kan premedisineres tilstrekkelig
- Ukontrollert diabetes definert som en fastende hyperglykemi >200 mg/dL eller HbA1c større enn eller lik 10 %
- Pasienten har hatt en cerebrovaskulær ulykke (CVA) eller forbigående iskemisk angrep (TIA) i løpet av de siste 6 månedene, eller har en permanent nevrologisk defekt som kan forårsake manglende overholdelse av protokollen
- Pasienten har et aktivt magesår eller aktiv gastrointestinal (GI) blødning
- Pasienten har tidligere hatt koagulopati
- Forsøkspersonen deltar i en annen forskningsstudie som involverer et undersøkelsesmiddel (farmasøytisk, biologisk eller medisinsk utstyr) som ikke har nådd det primære endepunktet
- Personer med forventet levealder på mindre enn 1 år
- Person som nekter eller ikke er en kandidat for akuttoperasjon for koronar bypass (CABG)
- Planlagt bruk av aterektomi, skåre- eller kutteballong, eller annet undersøkelsesutstyr enn litotripsi
- Stentimplantasjon i målkaret i løpet av det siste året eller en stent innenfor 10 mm fra mållesjonen
- Bestemt eller mulig trombe (ved angiografi eller intravaskulær avbildning) i målkaret
- Angiografisk bevis på en disseksjon i målkaret ved baseline eller etter passasje av guidewire

Pasienter med signifikant forkalket koronararteriestenose kandidat til PCI
Pasienter med koronarforkalkninger ansvarlig for signifikant stenose vil gjennomgå basal OCT-ervervelse.
Etter påføring av intravaskulær litotripsi (IVL), vil en andre OCT-ervervelse gjøres.
Den gjennomsnittlige kalsiumtettheten til fartøyets ROI før IVL og etter IVL vil bli sammenlignet.
Stentimplantasjon vil bli utført etter den andre OCT-anskaffelsen.

OCT-innhenting utføres ved baseline.
Deretter vil IVL-kateteret føres over lesjonen over ledetråden.
Aterektomi eller kutting/skåring av ballonger er ikke tillatt i henhold til protokollen.
IVL-prosedyren anses som vellykket ved gjenværende stenose
