Atténuation induite par les ondes de choc des plaques calcifiées quantifiée avec l'OCT (SCALPO)

Le but de cette étude observationnelle, prospective, multicentrique est d'explorer l'action locale de la fragmentation calcique coronarienne exercée par la lithotripsie endovasculaire par des images acquises par OCT, chez des patients présentant des calcifications coronariennes responsables de sténoses importantes, candidats à l'ICP.
La principale question à laquelle il vise à répondre est la capacité du système Shockwave à réduire la densité de calcium dans la région d'intérêt (ROI) de la paroi vasculaire.

Intervention / Traitement
Description détaillée
Les artères coronaires présentant des lésions calcifiantes subiront un traitement de lithotritie intravasculaire (IVL) ; L'acquisition OCT sera réalisée avant le traitement IVL, après le traitement IVL et après l'implantation du stent.
Toutes les images OCT seront téléchargées sur le programme de traitement et d'analyse d'images ImageJ (US National Institutes of Health).
Le retour sur investissement sera sélectionné dans le cadre de l'image où se trouve le calcium.
L'analyse d'image sera effectuée à l'aide d'un histogramme de fréquence de densité de pixels dans le retour sur investissement avant et après IVL, comme mesure de la densité de calcium.
La densité de pixels moyenne de la ROI avant IVL sera comparée à la densité de pixels moyenne de la ROI après IVL, en mesurant la densité de pixels delta.

Tous les patients du Laboratoire de cathétérisme qui subissent une coronarographie sont potentiellement sélectionnés.
La description
Critère d'intégration:
- Sujets atteints d'une maladie coronarienne native (y compris l'angine de poitrine stable ou instable et l'ischémie silencieuse) pouvant bénéficier d'une ICP
- Fraction d'éjection ventriculaire gauche (FEVG) ≥ 35 % telle que mesurée avant l'inscription
- DFGe ≥45 ml/min (Cockroft-Gault, MDRD)
- La troponine doit être inférieure ou égale à la limite supérieure de la valeur normale du laboratoire dans les 24 heures précédant la procédure ou si la troponine est élevée, la CK concomitante doit être normale
- Capacité à tolérer une double thérapie antiplaquettaire (c.-à-d. aspirine et clopidogrel, prasugrel ou ticagrélor) pendant au moins 6 mois (pour les patients non sous anticoagulation orale) et traitement antiplaquettaire unique à vie
- La lésion cible doit être une lésion coronarienne de novo qui n'a pas été traitée auparavant par une procédure interventionnelle
- Sténose de lésion cible unique de novo de LAD, RCA ou LCX (ou de leurs branches) avec une sténose ≥70 % et <100 % ou une sténose ≥50 % et <70 % (évaluée visuellement) avec preuve d'ischémie via un test d'effort positif, ou valeur de réserve de débit fractionnaire ≤0,80, ou iFR <0,89 ou IVUS ou OCT zone de lumière minimale ≤4,0 mm²
- Le récipient de référence doit avoir un diamètre de 2,5 mm à 4,0 mm et une longueur ≤ 30 mm
- Preuve de calcification au site de la lésion évaluée par angiographie, avec des radio-opacités fluoroscopiques notées sans mouvement cardiaque avant l'injection de contraste impliquant les deux côtés de la paroi artérielle dans au moins un emplacement et une longueur totale de calcium d'au moins 15 mm et s'étendant partiellement dans la lésion cible, ou par IVUS ou OCT, avec présence de ≥270 degrés de calcium sur au moins 1 coupe transversale
- Traitement planifié d'une lésion unique pour chaque vaisseau cible
- Capacité à passer un fil guide de 0,014" à travers la lésion
- Le navire cible doit avoir un flux TIMI 3 à la ligne de base
Critère d'exclusion:
- Fraction d'éjection ventriculaire gauche (FEVG) < 35 %
- Le sujet est enceinte ou allaite
- eGFR <45 ml/min (Cockroft-Gault, MDRD) ou dialyse chronique
- Hémoglobine pré-procédurale non traitée < 9 g/dL ou intention de refuser les transfusions sanguines si cela s'avérait nécessaire
- Le patient a une numération plaquettaire <100 000 cellules/mm3 ou >750 000 cellules/mm3
- Le patient a une allergie connue aux médicaments requis par le protocole (clopidogrel, thiénopyridines, aspirine, produits de contraste) qui ne peuvent pas être correctement prémédiqués
- Diabète non contrôlé défini comme une hyperglycémie à jeun > 200 mg/dL ou HbA1c supérieure ou égale à 10 %
- Le patient a eu un accident vasculaire cérébral (AVC) ou un accident ischémique transitoire (AIT) au cours des 6 derniers mois, ou présente une anomalie neurologique permanente pouvant entraîner le non-respect du protocole
- Le patient a un ulcère peptique actif ou un saignement gastro-intestinal (GI) actif
- Le patient a des antécédents de coagulopathie
- Le sujet participe à une autre étude de recherche impliquant un agent expérimental (dispositif pharmaceutique, biologique ou médical) qui n'a pas atteint le critère d'évaluation principal
- Sujets avec une espérance de vie inférieure à 1 an
- Sujet refusant ou non un candidat à un pontage aortocoronarien en urgence (CABG)
- Utilisation planifiée d'athérectomie, d'un ballon inciseur ou coupant, ou de tout dispositif expérimental autre que la lithotripsie
- Implantation d'un stent dans le vaisseau cible au cours de la dernière année ou d'un stent à moins de 10 mm de la lésion cible
- Thrombus certain ou possible (par angiographie ou imagerie intravasculaire) dans le vaisseau cible
- Preuve angiographique d'une dissection dans le vaisseau cible au départ ou après le passage du guide

Patients présentant une sténose coronarienne calcifiante importante candidats à l'ICP
Les patients présentant des calcifications de l'artère coronaire responsables d'une sténose importante subiront une acquisition basale de l'OCT.
Après l'application de la lithotritie intravasculaire (IVL), une seconde acquisition OCT sera réalisée.
La densité calcique moyenne de la ROI du vaisseau avant IVL et après IVL sera comparée.
L'implantation du stent sera réalisée après la deuxième acquisition OCT.

L'acquisition OCT est effectuée au départ.
Ensuite, le cathéter IVL sera passé à travers la lésion sur le fil de guidage.
L'athérectomie ou les ballons de coupe/score ne sont pas autorisés par le protocole.
La procédure IVL est considérée comme réussie lorsqu'une sténose résiduelle
