この観察的、前向き、多施設研究の目的は、PCIの候補となる重大な狭窄の原因となる冠動脈石灰化を有する患者において、OCTで取得した画像による血管内結石破砕術によって引き起こされる冠動脈カルシウム断片化の局所的作用を調査することである。
この研究が答えようとしている主な質問は、血管壁の関心領域 (ROI) のカルシウム密度を低減するショックウェーブ システムの能力です。
調査の概要
詳細な説明
石灰化病変のある冠動脈には血管内結石破砕術(IVL)治療が行われます。 OCT 取得は、IVL 治療前、IVL 治療後、およびステント移植後に実行されます。
すべての OCT フレームは、画像処理および分析プログラム ImageJ (米国国立衛生研究所) にアップロードされます。
ROI は、カルシウムが存在する画像の一部として選択されます。
画像解析は、カルシウム密度の尺度として、IVL 前後の ROI のピクセル密度の周波数ヒストグラムを使用して実行されます。
IVL 前の ROI の平均ピクセル密度が IVL 後の ROI の平均ピクセル密度と比較され、デルタ ピクセル密度が測定されます。

説明
包含基準:
- PCIに適した先天性冠動脈疾患(安定または不安定狭心症および無症候性虚血を含む)を患っている患者
- 登録前に測定された左心室駆出率(LVEF)≧35%
- eGFR ≥45 ml/分 (Cockroft-Gault、MDRD)
- トロポニンは、処置前の 24 時間以内に検査室の正常値の上限以下である必要があります。または、トロポニンが上昇している場合、付随する CK は正常でなければなりません
- 二重抗血小板療法に耐えられる能力(すなわち、 アスピリンとクロピドグレル、プラスグレル、またはチカグレロル)を少なくとも6か月間(経口抗凝固療法を受けていない患者の場合)、および生涯にわたって単回抗血小板療法を行う
- 標的病変は、これまでにいかなる介入処置も受けていない新規冠状病変でなければなりません
- 70%以上100%未満の狭窄を伴うLAD、RCAまたはLCX(またはそれらの分枝)の単一のde novo標的病変狭窄、または陽性ストレス試験による虚血の証拠を伴う狭窄50%以上70%未満(視覚的に評価)、または部分流量予備値 ≤0.80 または iFR <0.89 または IVUS または OCT の最小内腔面積 ≤4.0 mm²
- 基準容器の直径は 2.5 mm ~ 4.0 mm、長さは 30 mm 以下でなければなりません
- 血管造影によって評価された病変部位における石灰化の証拠。造影剤注入前に心臓の動きはなく、少なくとも1か所の動脈壁の両側に関与し、カルシウムの全長が少なくとも15 mmで、部分的に内部にまで広がっている、造影剤注入前の心臓の動きがない状態で、透視検査による放射線不透過性が認められる。標的病変、またはIVUSまたはOCTにより、少なくとも1つの断面に270度以上のカルシウムが存在する
- あらゆる標的血管に対する単一病変の計画的治療
- 0.014インチのガイドワイヤーを病変に通過させる能力
- ターゲット血管はベースラインで TIMI 流量 3 を持っている必要があります
除外基準:
- 左心室駆出率 (LVEF) <35%
- 対象者は妊娠中または授乳中です
- eGFR <45 ml/分(Cockroft-Gault、MDRD)または慢性透析
- 未治療の術前ヘモグロビン<9 g/dL、または輸血が必要になった場合に拒否する意向がある
- 患者の血小板数が <100,000 細胞/mm3 または >750,000 細胞/mm3 である
- 患者はプロトコルに必要な薬剤(クロピドグレル、チエノピリジン、アスピリン、造影剤)に対して既知のアレルギーを持っており、適切に前投薬することができない
- コントロールされていない糖尿病は、空腹時高血糖>200mg/dLまたはHbA1cが10%以上と定義されます。
- 患者は過去6か月以内に脳血管障害(CVA)または一過性脳虚血発作(TIA)を起こしたことがある、またはプロトコール不遵守の原因となる可能性のある永続的な神経学的欠陥を抱えている
- 患者は活動性の消化性潰瘍または活動性の胃腸(GI)出血を患っている
- 患者には凝固障害の病歴がある
- 被験者は主要評価項目に達していない治験薬(医薬品、生物製剤、または医療機器)を含む別の研究に参加している。
- 余命が1年未満の対象者
- 被験者が緊急冠動脈バイパス移植術(CABG)手術を拒否しているか、その候補者ではない
- アテローム切除術、スコアリングまたはカッティングバルーン、または結石破砕術以外の治験器具の計画的使用
- 過去1年以内に対象血管にステントが植え込まれている、または対象病変から10mm以内にステントが植え込まれている

PCIの候補となる重度の石灰化冠動脈狭窄を有する患者
重大な狭窄の原因となる冠動脈石灰化を有する患者は、基礎 OCT 取得を受けます。
血管内結石破砕術 (IVL) の適用後、2 回目の OCT 取得が行われます。
IVL 前と IVL 後の血管の ROI の平均カルシウム密度が比較されます。
ステント移植は、2 回目の OCT 取得後に実行されます。

OCT 取得はベースラインで実行されます。
次に、IVL カテーテルがガイドワイヤー上で病変を通過します。
アテローム切除術またはバルーンの切断/スコアリングは、プロトコールに従って許可されていません。
IVL 処置は、狭窄が残存している場合に成功したとみなされます。
