Narzędzie diagnostyczne niepełnosprawności ED: studium wykonalności technologii informacyjnej dla zdrowia
Cel 1: Opracowanie i zatwierdzenie narzędzia Rasch Disability Diagnostic Tool (RDDT) na oddziale ratunkowym (SOR).
Hipoteza:
- Modelowanie Rascha zapewni instrument o lepszych właściwościach pomiarowych, w tym zwiększonej wiarygodności i trafności w porównaniu z obecnie dostępną miarą niepełnosprawności ED (MOD).
Cel 2: Przeprowadzenie randomizowanej kontrolowanej próby w celu oceny przydatności ED-RDDT do zmniejszenia liczby ponownych hospitalizacji u odbiorców Medicare odwiedzających miejski oddział ratunkowy z powodu choroby nieurazowej
hipotezy:
- W porównaniu ze standardowym narzędziem przesiewowym, ED RDDT zmniejszy ponowne wykorzystanie szpitala (wizyty na SOR, przyjęcia do szpitala lub zgony) w ciągu 60 dni od wypisu.
- ED RDDT obniży koszty dla Medicare, szpitali i pacjentów.

Kryteria przyjęcia:
- Pacjenci zgłaszający się na SOR dla dorosłych w szpitalu Yale New Haven (YNHH)
- 65 lat lub więcej
- Odbiorcy Medicare
- Mieszkańcy społeczności.
Kryteria wyłączenia:
Pacjenci i ich opiekunowie zostaną wykluczeni z następujących powodów:
- Nieanglojęzyczny
- cierpiących na stan uniemożliwiający przesłuchanie, tj. upośledzenie komunikacji
- nie jest w stanie podać dwóch numerów kontaktowych do dalszych działań
- z ostrą psychozą lub skłonnościami samobójczymi.
