Wpływ przeciwności psychobiologicznych na dzieci i młodzież z cukrzycą typu 1

W ostatnim czasie obserwuje się wyraźny wzrost zachorowalności i wcześniejszego wieku zachorowania na cukrzycę typu 1 (T1D). Obserwacje te można również wiązać ze zwiększonym występowaniem stresujących doświadczeń i obniżoną tolerancją na stres u niektórych dzieci.
Sposób, w jaki dana osoba reaguje na stres, zależy od wzajemnego oddziaływania czynników genetycznych i środowiskowych. Wykazano, że doświadczenia środowiskowe w określonych okresach rozwoju kształtują reakcję stresową osi podwzgórze-przysadka-nadnercza (oś HPA). Wiadomo, że wczesny rozwój ośrodkowego układu nerwowego jest krytycznie zależny od interakcji dziecka z otoczeniem (dojrzewanie zależne od doświadczenia), a zwłaszcza od przywiązania do opiekunów (najczęściej matki). Właściwa opieka wiąże się z niższym podstawowym poziomem kortyzolu u dzieci i bardziej stabilną odpowiedzią osi HPA po ekspozycji na stres. Wszelkie okoliczności, które zaburzały tworzenie się przywiązania do głównego opiekuna (np. powikłania porodowe, stresory psychospołeczne wpływające na matkę czy jej depresja poporodowa) skutkowały wyższą reaktywnością osi HPA na stres u dzieci dotkniętych chorobą.
Wykazano również, że zmiany w poszczególnych genach wpływają na podatność danej osoby na stres i ryzyko rozwoju psychopatologii wywołanej stresem.

Kryteria przyjęcia:
- Świadoma zgoda podpisana przez rodzica/opiekuna prawnego i świadoma zgoda podpisana przez uczestnika badania przed przystąpieniem do badania
- Zdiagnozowano cukrzycę typu 1 przed podpisaniem świadomej zgody pacjenta (PIC) (dla grupy T1D).
- Wiek od 8 do 15 lat (włącznie) w momencie podpisania PIC
- Leczone przez ośrodek badacza wcześniejszy podpis PIC (dla grupy T1D).
- Gotowość do poddania się wszelkim procedurom studiów
Kryteria wyłączenia:
- Każda udokumentowana współistniejąca choroba przewlekła, o której wiadomo, że wpływa na kontrolę cukrzycy (np. zmieniona czynność nerek, aktywny nowotwór w trakcie leczenia, choroba Leśniowskiego-Crohna, wrzodziejące zapalenie jelita grubego, Mb choroba Addisona)
- Wszelkie inne uwarunkowania medyczne, społeczne lub psychologiczne, które w opinii badacza uniemożliwiają pacjentowi przestrzeganie protokołu badania (np. upośledzenie intelektualne).

Dzieci z cukrzycą typu 1 w wieku od 8 do 15 lat. Zostaną zastosowane następujące interwencje/ekspozycje: Behawioralne: ustalenia psychologiczne, genetyczne: określenie podatności genetycznej, zabieg/operacja: test uwalniania kortyzolu, zabieg/operacja: pomiar kortyzolu w ślinie. |
W celu ustalenia tła psychologicznego zostanie przeprowadzony ustrukturyzowany wywiad i kwestionariusze:
DNA zostanie wyizolowane z próbek krwi pacjentów i utworzy wymazy z policzków osób kontrolnych.
Zbadane zostaną geny o spodziewanym wpływie na reaktywność stresową i zaburzenia psychiczne.
Test Synacthen - będzie stosowany w grupie pacjentów z cukrzycą typu 1 do pomiaru reaktywności osi HPA
Pomiar kortyzolu w ślinie - będzie stosowany u pacjentów przed i po teście Synacthen oraz z Hashimoto i grupą kontrolną osób zdrowych przed i po Wywiadu Przywiązania
