最近、1 型糖尿病 (T1D) の発生率の顕著な増加と発症年齢の早期化が注目されています。 これらの観察は、一部の子供におけるストレス体験の蔓延の増加とストレス耐性の低下に関連している可能性もあります。
人がストレスにどのように反応するかは、遺伝的要因と環境的要因の相互作用によって決まります。 発達の特定の時期における環境経験が、個人の視床下部-下垂体-副腎軸(HPA軸)ストレス反応を形作ることが示されています。 中枢神経系の初期の発達は、子供の環境との相互作用(経験依存の成熟)、特に養育者(ほとんどの場合は母親)への愛着に決定的に依存していることが知られています。 適切な介護は、子供の基礎コルチゾールレベルを低下させ、ストレスにさらされた後のHPA軸反応をより安定させることにつながります。 主な養育者との愛着形成を妨げる状況(例: 出産時の合併症、母親に影響を与える心理社会的ストレス要因、または母親の産後うつ)は、影響を受けた子供たちのストレスに対するHPA軸の反応性が高くなりました。

- 親/法的保護者が署名したインフォームド・コンセント、および研究参加前に研究参加者が署名したインフォームド・コンセント
- 患者のインフォームドコンセント(PIC)の署名前にタイプ1 DMと診断されている(T1Dグループの場合)。
- 機長の署名時の年齢が8歳から15歳まで(両端を含む)
- PIC の事前署名により治験責任医師センターによって治療された (T1D グループの場合)。
- すべての学習手順を受ける意欲がある
除外基準:
- 糖尿病の管理に影響を与えることが知られている、文書化された付随する慢性疾患(例: 腎機能の変化、治療中の活動性がん、クローン病、潰瘍性大腸炎、Mbアジソン病)
- 研究者が患者を研究プロトコールに従うことができないと判断したその他の医学的、社会的または心理的状態(例: 知的障害)。

シナクテン テスト - 1 型糖尿病患者のグループで HPA 軸の反応性を測定するために使用されます。
唾液コルチゾール測定 - シナクテン検査の前後の患者、および小児愛着面接の前後に橋本甲状腺炎および健康な対照群に対して使用されます。
