Wpływ suplementacji magnezem (Mg) i naprzemiennej ekspansji i konstrykcji szczęki (Alt-RAMEC) u pacjentów z poprzecznym niedorozwojem szczęki (TMD) (Mg + Alt-RAMEC)
Wpływ suplementacji magnezem (Mg) oraz naprzemiennego rozszerzania i zwężania szczęki (Alt-RAMEC) u pacjentów z poprzecznym niedorozwojem szczęki (TMD): randomizowane badanie kliniczne.
To randomizowane badanie kliniczne ocenia efekt łączenia suplementacji magnezem z techniką ortodontyczną zwaną Alternatywną Ekspansją i Konstrykcją Szczęki (Alt-RAMEC) przy użyciu aparatu Mikroimplantowego Wspomaganego Szybkiego Poszerzania Podniebienia (MARPE) w leczeniu dorosłych (18-30 lat) z poprzecznym niedorozwojem szczęki (TMD). TMD to stan, w którym górna szczęka jest węższa niż normalnie, co może wpływać na zgryz i rozwój twarzy.
Badanie obejmuje dwie grupy: jedna otrzymuje protokół Alt-RAMEC z samym MARPE, a druga ten sam protokół wraz z doustnymi suplementami magnezu.

Badanie porównuje dwie grupy terapeutyczne: jedna otrzymuje protokół Alt-RAMEC z samym MARPE, a druga otrzymuje Alt-RAMEC z MARPE wraz z doustną suplementacją magnezu.
Protokół Alt-RAMEC obejmuje kontrolowane cykle ekspansji i zwężenia szczęki w celu ułatwienia korekcji poprzecznych dysproporcji szczęki. Pacjenci są leczeni za pomocą specyficznych schematów aktywacji i dezaktywacji obejmujących aparat MARPE przez kilka tygodni. To badanie ocenia zmiany w strukturach zębów, wyrostka zębodołowego, szkieletu i mięśni poprzez zastosowanie zaawansowanych technik obrazowania, takich jak 3D tomografia stożkowa (CBCT) i powierzchniowa elektromiografia (sEMG). Dodatkowo, zgłaszany przez pacjentów poziom bólu i dyskomfortu będzie oceniany w trakcie leczenia ortodontycznego.
To badanie koncentruje się na obiektywnym pomiarze zmian morfologicznych i funkcjonalnych, aby określić, czy spożycie magnezu wzmacnia efekty leczenia Alt-RAMEC oraz poprawia komfort pacjenta i wyniki leczenia. Wyniki mogą dostarczyć cennych dowodów na optymalizację protokołów postępowania w poprzecznym niedorozwoju szczęki u młodych dorosłych pacjentów.

Aktywny komparator: Alt-RAMEC/MARPE Without Magnesium Supplementation
Participants receive alternate rapid maxillary expansion and constriction (Alt-RAMEC) using a micro-implant-assisted rapid palatal expansion (MARPE) appliance without magnesium supplementation.
The appliance consists of a 10-mm Maxillary Skeletal Expander II supported by four palatal miniscrews.
Activation consists of two expansion quarter-turns daily in weeks 1 and 3, one reverse quarter-turn daily in weeks 2 and 4, and one to two expansion quarter-turns daily in week 5 until the clinical expansion endpoint is reached.
Weeks 6-12 comprise passive retention and follow-up.
The appliance and activation schedule are identical to those used in the experimental arm; this arm serves as the control for evaluating the additional effects of magnesium supplementation.
