SEPAGES: Acompanhamento da exposição à poluição atmosférica durante a gravidez e efeitos na saúde
A coorte casal-criança SEPAGES é coordenada por uma equipe de epidemiologia ambiental do Inserm (Instituto Nacional Francês de Saúde e Pesquisa Médica). A coorte SEPAGES é baseada na população em geral e visa cobrir várias áreas de pesquisa em saúde ambiental, incluindo desreguladores endócrinos e poluentes atmosféricos.
Os principais objetivos são quantificar o impacto da poluição durante a gravidez e nos primeiros anos de vida em três principais resultados de saúde: crescimento, saúde respiratória e neurodesenvolvimento; e para entender melhor o mecanismo biológico (como a análise genética para estudar o papel dos fatores ambientais na expressão dos genes, bem como o papel dos fatores genéticos na saúde). O objetivo é atender 500 famílias.

O SEPAGES é um novo tipo de coorte longitudinal casal-criança com exames clínicos repetidos durante a gravidez e a infância, coleta extensa de amostras biológicas repetidas dos pais e da criança, uso de novas tecnologias para avaliar a exposição à poluição do ar (GPS, dosímetro pessoal etc. ) Ao longo do estudo (durante a gravidez e nos primeiros anos de vida da criança), são realizados exames clínicos e diversos exames biológicos (urina, sangue, cabelo). A exposição a diferentes fatores ambientais é feita várias vezes durante a gravidez e nos primeiros anos de vida da criança, graças a dosímetros pessoais, GPS e acelerômetros. Durante o trabalho de parto e o parto, são colhidas várias amostras (placenta, tecidos, sangue, urina, cabelo, mecônio e leite materno). Finalmente, durante o estudo, vários questionários de avaliação são entregues aos voluntários por meio de uma plataforma de computador, a fim de ter uma melhor compreensão do ambiente em que vivem.
A coorte SEPAGES segue a regra da pesquisa biomédica. Os consentimentos informados são assinados pelo pai e pela mãe durante a visita de inclusão que ocorre no hospital. Todos os acordos éticos e de privacidade de dados foram obtidos: um parecer favorável foi emitido pelo Comité de Protection des Personnes (CPP) em 13 de novembro de 201 e pelo CCTIRS (Comité Consultatif sur le Traitement de l'Information en matière de Recherche dans le domaine de la Santé) em 09 de janeiro de 2014. O estudo foi autorizado pela Agence Nationale de Sécurité du Médicament (ANSM), em 17 de dezembro de 2013 e pelo Comité Nationale de l'Informatique et des Libertés (CNIL) em 26 de junho de 2014.
