32-недельное исследование по оценке эффективности и безопасности тралокинумаба у субъектов с атопической экземой рук от умеренной до тяжелой степени, которые являются кандидатами на системную терапию (ADHAND)
Фаза 3b, интервенционное адаптивное клиническое исследование для оценки эффективности и безопасности монотерапии тралокинумабом в дозе 300 мг каждую вторую неделю по сравнению с плацебо у субъектов с атопической экземой рук от умеренной до тяжелой степени, которые являются кандидатами на системную терапию
Цель этого исследования — проверить, является ли лечение тралокинумабом безопасным и эффективным для лечения атопической экземы рук средней и тяжелой степени. Об этом будут судить по ряду оценок, которые оценивают тяжесть и степень атопической экземы рук и ее симптомов, а также общее состояние здоровья и качество жизни.
Суд продлится до 40 недель. Будет до 15 посещений, 3 из которых будут проведены по телефону. Первая часть испытания называется периодом скрининга и продлится до 4 недель. В течение первых 16 недель после скрининга участники испытаний будут получать либо тралокинумаб, либо фиктивные инъекции каждые две недели. После первых 16 недель все участники исследования будут получать инъекции тралокинумаба каждые две недели в течение 16 недель. Последняя часть испытания — это период в 4 недели после окончания периода лечения, когда участники испытания прекращают прием препарата для последующего наблюдения за безопасностью.

Описание
Критерии включения:
- Возраст 18 лет и старше на момент скрининга
- Диагностика атопического дерматита (АД) по критериям Hanifin и Rajka (1980) для АД
- История AD в течение ≥1 года
- Наличие атопической экземы рук (АЭ), которая сохраняется более 3 месяцев или рецидивирует дважды или более в течение последних 12 месяцев, при исключении любых известных и соответствующих раздражителей и аллергенов.
- Вовлечение AD по крайней мере в одном месте тела, кроме кистей и запястий при скрининге
- Общая оценка исследователя по шкале AHE 3 или 4 (от умеренной до тяжелой) при скрининге и исходном уровне
- Оценка зуда по шкале HESD (в среднем за неделю) ≥4 на исходном уровне.
- Субъекты, у которых в недавнем анамнезе (в течение 12 месяцев до визита для скрининга) имеется неадекватный ответ на лечение ОАГ местными рецептурными препаратами или для которых местное лечение нецелесообразно по другим причинам (например, из-за серьезных побочных эффектов или рисков безопасности)
Критерий исключения:
Субъекты не должны участвовать в исследовании, если у них есть активные подтипы экземы рук, отличные от AHE, которые считаются основной причиной текущей экземы рук, включая:
- Активный раздражающий контактный дерматит
- Активный аллергический контактный дерматит
- Контактный дерматит/контактная крапивница с активным белком
- Активная гиперкератотическая экзема рук
- Активная везикулярная экзема рук (помфоликс)
- Активные дерматологические состояния, которые могут спутать диагноз АД или АЭ или помешать оценке лечения.
- Использование соляриев или фототерапия (например, UVB, UVA1, PUVA) или лучевая терапия Grenz на руках или запястьях в течение 28 дней до исходного уровня или использование отбеливающих ванн на руках или запястьях в течение 7 дней до исходного уровня
- Лечение системными иммунодепрессантами/иммуномодуляторами и/или системными кортикостероидами в течение 28 дней до исходного уровня
- Лечение местными кортикостероидами, местными ингибиторами кальциневрина, местными ингибиторами фосфодиэстеразы 4 или местными ингибиторами Янус-киназы в течение 7 дней до исходного уровня
- Получение любой продаваемой биологической терапии (т. иммуноглобулин, анти-иммуноглобулин E), включая дупилумаб или исследуемые биологические агенты, за 3–6 месяцев до исходного уровня

Плацебо Компаратор: Плацебо + тралокинумаб
Плацебо вводят в течение 16 недель (двойной слепой период) перед переходом на 16-недельный открытый период, когда вводят тралокинумаб.
