Fáze 3b, intervenční, adaptivní, klinická studie k vyhodnocení účinnosti a bezpečnosti tralokinumabu 300 mg každý druhý týden v monoterapii ve srovnání s placebem u pacientů se středně těžkým až těžkým atopickým ekzémem rukou, kteří jsou kandidáty na systémovou léčbu

Účelem této studie je otestovat, zda je léčba tralokinumabem bezpečná a účinná při léčbě středně těžkého až těžkého atopického ekzému rukou. To bude posuzováno na základě řady hodnocení, která hodnotí závažnost a rozsah atopického ekzému na rukou a jeho symptomy, stejně jako celkový zdravotní stav a kvalitu života.

Zkouška bude trvat až 40 týdnů. Uskuteční se až 15 návštěv, z toho 3 telefonicky. První část zkoušky se nazývá období screeningu a bude trvat až 4 týdny. Prvních 16 týdnů po screeningu budou účastníci studie dostávat každé dva týdny buď tralokinumab nebo neúčinné injekce. Po prvních 16 týdnech budou všichni účastníci studie dostávat injekce tralokinumabu každé dva týdny po dobu 16 týdnů. Poslední částí studie je období 4 týdnů po ukončení léčebného období, kdy účastníci studie nemají lék kvůli bezpečnostnímu sledování.

Kritéria pro zařazení:

  • Věk 18 let nebo více při screeningu
  • Diagnostika atopické dermatitidy (AD) podle kritérií Hanifin a Rajka (1980) pro AD
  • Anamnéza AD po dobu ≥ 1 roku
  • Přítomnost atopického ekzému rukou (AHE), který přetrvává déle než 3 měsíce nebo se dvakrát nebo vícekrát vrátil během posledních 12 měsíců, s vyloučením jakýchkoli známých a relevantních dráždivých látek a alergenů
  • AD postižení alespoň jednoho místa těla kromě rukou a zápěstí při screeningu
  • Globální hodnocení vyšetřovatele AHE skóre 3 nebo 4 (střední až těžké) při screeningu a na začátku
  • Skóre svědění HESD (týdenní průměr) ≥4 na začátku
  • Subjekty, které mají zdokumentovanou nedávnou anamnézu (do 12 měsíců před screeningovou návštěvou) neadekvátní odpovědi na léčbu AHE topickými léky na předpis nebo pro které je topická léčba jinak z lékařského hlediska nevhodná (např.

Kritéria vyloučení:

  • Subjekty se nesmí zúčastnit studie, pokud mají aktivní podtypy ekzému na rukou jiné než AHE, které jsou považovány za převládající příčinu současného ekzému na ruce, včetně:

    • Aktivní dráždivá kontaktní dermatitida
    • Aktivní alergická kontaktní dermatitida
    • Aktivní proteinová kontaktní dermatitida/kontaktní kopřivka
    • Aktivní hyperkeratotický ekzém rukou
    • Aktivní vezikulární ekzém rukou (pompholyx)
  • Aktivní dermatologické stavy, které mohou zmást diagnózu AD nebo AHE, nebo které by narušovaly hodnocení léčby
  • Použití solárií nebo fototerapie (např. UVB, UVA1, PUVA) nebo terapie zářením Grenz na rukou nebo zápěstí během 28 dnů před výchozí hodnotou nebo použití bělících koupelí na ruce nebo zápěstí během 7 dnů před výchozí hodnotou
