Uno studio clinico di fase 3b, interventistico, adattivo, per valutare l'efficacia e la sicurezza di Tralokinumab 300 mg ogni due settimane in monoterapia rispetto al placebo in soggetti con eczema atopico della mano da moderato a grave candidati alla terapia sistemica

Lo scopo di questo studio è verificare se il trattamento con tralokinumab è sicuro ed efficace per il trattamento dell'eczema atopico della mano da moderato a grave. Questo sarà giudicato da una serie di valutazioni che valutano la gravità e l'estensione dell'eczema atopico della mano e dei suoi sintomi, nonché lo stato di salute generale e la qualità della vita.

Il processo durerà fino a 40 settimane. Ci saranno fino a 15 visite, 3 delle quali saranno condotte per telefono. La prima parte della prova è chiamata periodo di screening e durerà fino a 4 settimane. Criterio di inclusione:

  • Età 18 anni o superiore allo screening
  • Diagnosi di dermatite atopica (AD) secondo i criteri Hanifin e Rajka (1980) per l'AD
  • Storia di AD da ≥1 anno
  • Presenza di eczema atopico della mano (AHE) che persiste da più di 3 mesi o si ripresenta due o più volte negli ultimi 12 mesi, evitando qualsiasi irritante e allergene noto e rilevante
  • Coinvolgimento di AD di almeno una sede del corpo diversa dalle mani e dai polsi allo screening
  • Punteggio AHE di valutazione globale di un investigatore di 3 o 4 (da moderato a grave) allo screening e al basale
  • Un punteggio di prurito HESD (media settimanale) di ≥4 al basale
  • Soggetti che hanno una storia recente documentata (entro 12 mesi prima della visita di screening) di risposta inadeguata al trattamento dell'AHE con farmaci topici prescritti o per i quali i trattamenti topici sono altrimenti sconsigliabili dal punto di vista medico (ad esempio, a causa di importanti effetti collaterali o rischi per la sicurezza)

Criteri di esclusione:

  • I soggetti non devono partecipare allo studio se hanno sottotipi attivi di eczema della mano diversi dall'AHE che sono considerati la causa predominante dell'attuale eczema della mano, tra cui:

    • Dermatite da contatto irritante attiva
    • Dermatite allergica da contatto attiva
    • Dermatite da contatto con proteine ​​attive/orticaria da contatto
    • Eczema della mano ipercheratosico attivo
    • Eczema vescicolare attivo della mano (pompholyx)
  • Condizioni dermatologiche attive che possono confondere la diagnosi di AD o AHE o che interferirebbero con la valutazione del trattamento
