Studie för att utvärdera effekten av Roflumilast 500 μg på exacerbationsfrekvensen hos patienter med kronisk obstruktiv lungsjukdom (KOL) som behandlats med en kombination med fast dos av långverkande beta-agonist och inhalerad kortikosteroid (LABA/ICS)
För att visa den ytterligare fördelen med roflumilast när det läggs till fastdoskombination (FDC) LABA/ICS för att minska exacerbationer hos patienter med svår till mycket svår KOL.

Beskrivning
Inklusionskriterier:
- 1. Manliga eller kvinnliga patienter som är minst 40 år gamla
- 2. Historik av KOL (enligt Global Initiative for Chronic Obstructive Lung Disease [GOLD] 2010) i minst 12 månader före screening (besök 1) i samband med kronisk produktiv hosta i 3 månader under vart och ett av 2 år i följd (med andra orsaker) av produktiv hosta utesluten). Endast patienter med kronisk bronkit kommer att inkluderas (samtidigt emfysem är tillåtet)
- 3. Forcerad utandningsvolym efter 1 sekund (FEV1)/forcerad vitalkapacitet (FVC) ratio (postbronkodilator) < 70 % vid screening (besök 1)
- 4. FEV1 (postbronkodilator) ≤ 50 % av förutspått vid screening (besök 1)
- 5. Minst två dokumenterade måttliga eller svåra KOL-exacerbationer inom 12 månader före screening (besök 1)
- 6. Patienter måste vara på FDC LABA/ICS-behandling ≥ 3 månader före screening (besök 1)
- 7. Tidigare rökare (definieras som rökavvänjning för minst 1 år sedan) eller nuvarande rökare (inklusive patienter som slutat röka under det senaste året) båda med en rökhistoria på minst 20 packår
Exklusions kriterier:
- 1. Måttlig eller svår exacerbation av KOL och/eller KOL-exacerbationer behandlade med antibiotika eller systemiska glukokortikosteroider inom 4 veckor efter screening (besök 1) (dvs patienter måste vara kliniskt stabila)
- 2. Känd alfa-1-antitrypsinbrist
- 3. Aktuell astmadiagnos (antingen kontrollerad eller okontrollerad) (Obs: Historik med astma hos barn är inte uteslutande.)
- 4. Body mass index (BMI) ≥ 45 kg/m2
- 5. Patienter med en anamnes (inom 5 år) eller aktuell diagnos av annan cancer än basal- eller skivepitelcellshudcancer
