CORT125281 är en selektiv glukokortikoidreceptor (GR) antagonist. I denna studie kommer CORT125281 att administreras oralt i kombination med enzalutamid till patienter med metastaserad kastrationsresistent prostatacancer (mCRPC) för att utvärdera kurens säkerhet, tolerabilitet, farmakokinetik, farmakodynamik och preliminär effekt. Studien består av två faser: en dosbestämningsfas och en expansionsfas. Dosbestämningsfasen är utformad för att bestämma dosbegränsande toxicitet och RD för CORT125281 plus enzalutamid hos patienter med mCRPC. När den rekommenderade doseringsregimen har bestämts kommer följande expansionskohorter att registreras och behandlas med CORT125281 plus enzalutamid vid den rekommenderade dosnivån.
Abi-resistent kohort: Patienter som har utvecklats under behandling med abirateron och som inte har fått några andra androgenreceptorblockerande behandlingar (AR)
ARant-resistent kohort: Patienter som har utvecklats under behandling med enzalutamid eller andra andra generationens AR-hämmare.
Effekten av mat på CORT125281 PK kommer att bedömas hos en del av patienterna som ingår i expansionsfasen. De två expansionskohorterna kommer att skrivas in parallellt.
I varje fas av studien kommer rutinmässiga bedömningar av säkerhet och tolerabilitet att utföras och prover kommer att samlas in för att fastställa standard PK-parametrar för CORT125281, enzalutamid och deras huvudmetaboliter. PD, livskvalitetsutvärderingar och preliminära utvärderingar av antitumöraktivitet av CORT125281 med enzalutamid kommer att utföras under hela studien.

Viktiga inkluderingskriterier:
- Kunna förstå syftet och riskerna med studien; villig och kapabel att följa schemalagda besök, behandlingsplaner, laboratorietester och andra studieutvärderingar och procedurer och ge skriftligt informerat samtycke
- Män ≥18 år vid tidpunkten för undertecknandet av samtycke
- Histologiskt bekräftat adenokarcinom i prostata med metastaserande sjukdom
- Dosbestämningsfas Segment 1 och expansionsfas: Progressiv sjukdom enligt definition av PSA eller avbildning efter senaste tidigare behandling. PSA ≥1 ng/ml, om en bekräftad ökning av PSA är den enda indikationen på progression. Progression av PSA kräver att PSA stiger över ett tidigare referensvärde med minst 2 mätningar med ≥1 veckas mellanrum. PSA-mätningar kan samlas in under eller efter den senaste tidigare behandlingen.
Dosbestämningsfas Segment 2: Får för närvarande enzalutamid med stigande PSA enligt följande:
- Stigande PSA: 25 % ökning över nadir och ett absolut värde på >1 ng/ml med minst 2 mätningar erhållna med ≥1 veckas mellanrum. PSA-mätningar kan samlas in under eller efter den senaste tidigare behandlingen.
- Patienterna måste ha fått enzalutamid i minst 12 veckor och ha stabila doser av enzalutamid ≥80 mg dagligen i minst 4 veckor före cykel 1 dag 1. Patienterna kommer att fortsätta med enzalutamid utan avbrott under screeningperioden (ingen uttvättningsperiod ). Detta kommer att vara startdosen för enzalutamid för kombination med CORT125281 med början på cykel 1 dag 1.
- M0 sjukdom är tillåten
Expansionsfas: Patienterna måste ha utvecklats medan de fått en androgeninriktad behandling enligt följande:
- Abi-resistent kohort: Patienterna måste ha utvecklats under behandling med abirateron.
- ARant-resistent kohort: Patienterna måste ha utvecklats under behandling med enzalutamid eller andra generationens AR-blockerande terapier. Patienter som fortskrider med enzalutamid omedelbart innan de registreras i denna studie måste ha stabila doser av enzalutamid. Dessa patienter kommer att fortsätta med enzalutamid utan avbrott under screeningsperioden (ingen uttvättningsperiod krävs).
- Baslinje tumörbedömning utförd inom 28 dagar före den första dosen av studiebehandlingen (CORT125281 och/eller enzalutamid i studien, beroende på vilket som är tidigast)
- Tidigare kirurgisk eller kemisk kastrering med serumtestosteron <1,7 nmol/L (50 ng/dL). Om kastreringsmetoden är användning av en luteiniserande hormonfrisättande hormon (LHRH)-analog, måste det finnas en plan för att upprätthålla effektiv LHRH-analogbehandling under hela försöket
- Samtycke till att ha alla protokoll som krävs för farmakodynamiska biomarkörprover, inklusive förbehandlingen och på behandlingsparade tumörbiopsier (obligatoriskt för en undergrupp av patienter).
- Samtycke till att tillhandahålla obligatoriskt farmakogenomiskt blodprov (endast dosbestämningssegment 1)
- Eastern Cooperative Oncology Group (ECOG) prestandastatus på 0 eller 1
- Adekvat baslinjeorganfunktion inom 14 dagar före den första dosen av studiebehandlingen (enzalutamid i studien och/eller CORT125281, beroende på vilket som är tidigast)
- Patienter som får systemiska kortikosteroider i mer än 2 veckor inom 3 månader från studiestart eller med kliniska tecken på binjurebarksvikt måste ha bevis på adekvat binjurefunktion baserat på morgonplasmakortisolkoncentration eller ACTH (cosyntropin) stimuleringstest
- Om en patient har sexuellt umgänge med en kvinna i fertil ålder måste kondom med spermiedödande medel och annan form av preventivmedel användas under och i 100 dagar efter den sista dosen av studiebehandlingen (CORT125281 eller enzalutamid, beroende på vilket som är senast). Kondom krävs under och i 100 dagar efter avslutad behandling med enzalutamid om en patient är engagerad i sexuell aktivitet med en gravid kvinna. Patienterna måste också gå med på att undvika spermiedonation under studien och i minst 100 dagar efter den slutliga behandlingen.
Huvudsakliga uteslutningskriterier:
- Fick kemoterapi, icke-palliativ strålbehandling, immunterapi eller någon undersökning av cancerterapier inom 21 dagar före den första dosen av CORT125281, eller behandling med sådana terapier planeras under protokollbehandling. Samtidig anticancerbehandling är inte tillåten under inledningsperioden för enzalutamid under dosbestämningsfas segment 1
- Mer än två tidigare cytotoxiska kemoterapiregimer för behandling av mCRPC
Dosbestämningsfas och expansionsfas kommer att exkludera patienter för följande:
- Dosbestämningsfas (endast segment 1)
- Framsteg under behandling med enzalutamid före cykel 1 Dag -28 (gäller endast patienter som får enzalutamid-inledning) eller
- Fick tidigare 2:a generationens anti-androgen och kräver akut sjukdomssvar eller stabilisering
Abi-resistent kohort i expansionsfasen:
- Fick tidigare behandling med enzalutamid, eller
- Fick tidigare 2:a generationens anti-androgen och kräver akut sjukdomssvar eller stabilisering
- Expansionsfas ARant-resistent kohort: Kräver akut sjukdomsrespons eller stabilisering
- Pågående eller förväntad behandling med hormonbehandling (annan än LHRH-analog), inklusive valfri dos av megestrolacetat (Megace), finasterid (Proscar), dutasterid (Avodart) eller fått abirateron inom 28 dagar före den första dosen av CORT125281
- Kontraindikation eller försiktighetsåtgärd för enzalutamid
- Parenkymala hjärnmetastaser
- Varje kliniskt signifikant okontrollerat tillstånd som kan öka risken för studiepatienten eller som utredaren anser utsätter patienten för en oacceptabel risk
- Fick växtbaserade produkter eller alternativa terapier som kan sänka PSA-nivåerna eller som kan ha hormonell anti-prostatacanceraktivitet (t.ex. sågpalmetto, PC-SPES, PC-HOPE, johannesört, selentillskott, druvkärneextrakt, etc.) inom 28 dagar efter påbörjad studiebehandling eller planerar att påbörja behandling med dessa produkter/alternativa terapier under hela studiens varaktighet
- Fick systemiska glukokortikoider inom 21 dagar före den första dosen av CORT125281, eller behov av kroniska eller ofta använda systemiska eller inhalerade glukokortikoider för medicinska tillstånd (t.ex. reumatoid artrit, immunsuppression efter organtransplantation). Korta kurser (<5 dagar) av icke-cancerrelaterade skäl är tillåtna om det är kliniskt nödvändigt (såsom profylax för CT).
- Samtidig behandling med starka hämmare eller inducerare av CYP3A4 eller CYP2C8 eller med känsliga substrat av CYP3A4, CYP2C9 eller CYP2C19

Patients will receive lead-in enzalutamide monotherapy once daily for 28 days.

Experimentell: Dose Expansion - Abi-Resistant Cohort Food Effect (Open-label)
Subcohort (first 10 patients enrolled into Cohort A).
Patients enrolled into this subcohort will receive a single dose of exicorilant at Cycle 1 Day -7 and a single dose of exicorilant at Cycle 1 Day 1 30 minutes after a standard breakfast to assess the effect of food on pharmacokinetic (PK)parameters.
Patients will then begin exicorilant in combination with enzalutamide on Cycle 1 Day 2 and continue in 28-day dosing cycles.
