Sicklecell disease (SCD) bär på betydande sjuklighet till följd av sickling av röda blodkroppar (RBC) och hemolys. Stroke är en av de mest förödande följderna av SCD. Kronisk transfusionsterapi (CTT) minskar strokerisken genom att (1) tillföra normala, icke-sickle-RBC till cirkulationen, och därigenom minska andelen endogena sickle-RBC i cirkulationen, och (2) bibehålla ett högre hemoglobin (Hb), och därigenom undertrycka erytropoes av ny skära RBC. Medan effekten av CTT vid strokeprofylax är väletablerad, fortsätter nästan 45 % av barnen att få tysta eller uppenbara stroke trots CTT. Misslyckandet med CTT att förhindra strokehändelser kan vara relaterat till otillräcklig minskning av cirkulerande sickle RBC och erytropoes. Mängden cirkulerande sickle-RBC är relaterad till överlevnadskinetiken för både transfunderade RBC och endogen sickle RBC.
I en stor longitudinell analys av CTT i SCD fann forskarna stor variation i överlevnaden av donator-RBC efter transfusion, med snabbare clearance associerad med patientens immunfunktioner (historisk RBC-alloimmunisering och mjälte) och med donator-RBC glukos-6-fosfat -dehydrogenas (G6PD) brist. För att bättre förstå rollerna av patient- och donatorfaktorer i överlevnaden och elimineringen av transfunderade RBC, föreslår forskarna en mekanistisk, klinisk prövning under kroniska transfusionsepisoder hos patienter med SCD, där en liten alikvot av varje transfunderad enhet är märkt med biotinkonjugerat till RBC-ytproteiner, för att säkert identifiera och mäta in vivo-överlevnaden av donator-RBC.
Syfte 1 kommer att undersöka relationerna mellan mottagarens immunsystem (tidigare alloimmunisering, mjältvolym och markörer för funktion av retikuloendotelsystemet) på överlevnaden efter transfusion av biotinmärkt donator-RBC.
Syfte 2 kommer att undersöka sambanden mellan donator RBC G6PD nivåer och donator RBC metabolomics med in vivo överlevnad och förändringar i donator RBC senescensmarkörer. Genomförandet av dessa mål kommer att öka förståelsen för mekanismer för variationen i RBC-överlevnad under CTT, identifiera donator- och mottagareriskfaktorer för minskad RBC-överlevnad. I slutändan kommer denna kunskap att informera hanteringen av CTT för att förbättra förebyggandet av stroke vid SCD.

Inklusionskriterier:
- HbSS eller HbSβ0 talassemi SCD
- ta emot CTT i ≥3 månader före registreringen
Exklusions kriterier:
- förväntat upphörande av CTT under de närmaste ≤2 månaderna
- samtidig behandling med hydroxiurea
- automatiserad RBC-utbytesbehandling inom 3 månader före inskrivning eller förväntad inom de närmaste 3 månaderna
- fördröjd hemolytisk transfusionsreaktion under de senaste 3 månaderna
- konsumerar högdos biotin eller råa äggtillskott
- nuvarande graviditet

Experimentell: Biotin Labeled Red Blood Cells
Participants are persons with sickle cell disease (SCD) receiving a blood transfusion with biotin labeled red blood cells (RBCs).
Participants receive 2 or 3 units of transfused blood, depending on clinical care, and a portion of all units are biotin-labeled.
Samples are taken for 12 weeks after the biotinylated transfusion.
Participants continue to receive regular monthly transfusions (non-biotinylated) as part of their usual chronic transfusion therapy (CTT) during the follow-up period for this study.

On the day of transfusion, a 20 mL aliquot is sterilely withdrawn from each RBC unit, washed and labeled with sulfo-NHS-biotin for 30 minutes, washed to stop the labeling reaction, then resuspended in plasma to a hematocrit of ~60%.
The biotin-labeled RBC (BioRBC) is transfused along with the remainder of the RBC unit (unlabeled volume).
Standard blood bank and CTT protocols and minor antigen matching for SCD patients are followed.
Exact transfusion volume is determined based on pre-transfusion hemoglobin (Hb), sickle cell hemoglobin (HbS), and body weight, per clinical protocol.
Participants taking part in this optional intervention have one transfusion episode with blood using the INTERCEPT Blood System.
For this transfusion, a portion of each blood unit is biotin-labeled and one of those units has the INTERCEPT treatment.
In addition to the blood drawn for the main study, individuals participating in this optional intervention have additional tubes of peripheral venous blood drawn for evaluating treatment-emergent antibodies specific to INTERCEPT RBCs and acridine surface label monitoring.
Tests for treatment-emergent antibodies specific to INTERCEPT RBCs are performed according to procedures developed by Cerus Corporation.
This optional study activity examines the survival of transfused RBCs with and without the INTERCEPT treatment.
