En fas I/II observatörsblind, randomiserad, kontrollerad, multi-landsstudie för att utvärdera säkerheten, reaktogeniciteten och immunsvaret på GVGH altSonflex1-2-3-vaccinet mot S. Sonnei och S. Flexneri, serotyper 1b, 2a , och 3a, i vuxna i Europa (steg 1) följt av åldersdeeskalering från vuxna till barn och spädbarn, och dossökning hos spädbarn i Afrika (steg 2)

Syftet med den aktuella kliniska studien är att, för första gången på människor (FTIH), utvärdera säkerheten och immunogeniciteten hos altSonflex1-2-3-kandidatvaccinet mot S. sonnei och S. flexneri serotyperna 1b, 2a och 3a. Vaccinet kommer först att ges till vuxna i åldern 18 till 50 år i Europa. Därefter kommer vaccinet att administreras till en shigellos-endemisk population i Afrika, först till vuxna 18 till 50 år, sedan till barn 24 till 59 månader och slutligen till spädbarn 9 månader gamla. Spädbarn kommer också att få en tredje vaccination. Tre olika doser av vaccinet [låga (dos A), medelhöga (dos B) och höga (dos C) mängder antigen] kommer att utvärderas med hjälp av en åldersdeeskaleringsmetod (från minst sårbara vuxna till mest sårbara pediatriska populationer ). Resultaten av denna studie kommer att göra det möjligt att välja den mest lämpliga dosen för vidare utveckling av vaccin hos spädbarn 9 månader gamla, vilket är den huvudsakliga målåldergruppen för detta vaccin.

Alla deltagare:

• Deltagarnas och/eller deltagarnas förälder/er/lagligt godtagbara företrädare LAR(er), som enligt utredarens uppfattning kan och kommer att följa protokollets krav (t.ex. ifyllande av dagbokskorten, återkomst för uppföljningsbesök).

  • Skriftligt eller bevittnat/tummetryckt informerat samtycke som erhållits från deltagaren/föräldrarna/LAR till deltagaren innan någon studiespecifik procedur genomförs.
  • Friska deltagare enligt medicinsk historia, klinisk undersökning och laboratoriebedömning.

  • Deltagare negativa för humant leukocytantigen B27 (HLA-B27).

Vuxna 18 till 50 år:

  • En man eller kvinna mellan och inklusive 18 och 50 år vid tidpunkten för den första studieinterventionsadministrationen.
  • Kvinnliga deltagare av icke-fertil ålder kan inkluderas i studien. Icke-fertil ålder definieras som premenarche, nuvarande bilateral tubal ligering eller ocklusion, hysterektomi, bilateral ovariektomi eller postmenopaus.
  • Kvinnliga deltagare i fertil ålder kan registreras i studien om deltagaren:
  • har använt adekvat preventivmedel i 1 månad före studieinterventionsadministration, och
  • har ett negativt graviditetstest på dagen för studieinsatsadministration, och
  • har gått med på att fortsätta med adekvat preventivmedel under hela behandlingsperioden och i 1 månad efter avslutad studieinterventionsadministrationsserie.
  • Deltagare seronegativa för humant immunbristvirus (HIV).

Barn 24 till 59 månader:

  • En man eller kvinna mellan och inklusive 24 och 59 månaders ålder vid tidpunkten för första vaccinationen.
  • Normalt näringsmässigt Z-poäng (-2 standardavvikelse eller mer).
  • Tidigare genomförda rutinmässiga barnvaccinationer efter bästa kännedom av deltagarens förälder/lar/lar.
  • Född efter en graviditetstid på ≥37 veckor.
  • Deltagare seronegativa för HIV.

Spädbarn 9 månader gamla:

Alla deltagare:

• Känd exponering för Shigella under deltagarens livstid, bekräftad under intervju med deltagaren eller dokumenterad av patientjournaler (t.ex. historia av mikrobiologiskt bekräftad Shigella-infektion), nyligen resor* (inom 2 år) till ett land där Shigella eller annan enterisk infektioner är endemiska eller nyligen sysselsatta* (inom 3 år) som involverar Shigella-arter.

  • Uteslutning på grund av resor eller yrke gäller endast vuxna 18 till 50 år i Europa (steg 1).

    • Progressiva, instabila eller okontrollerade kliniska tillstånd.

• Administrering av immunglobuliner och/eller eventuella blodprodukter eller plasmaderivat, eller benmärgstransplantation, under perioden som börjar 3 månader före den första dosen av studievaccin eller planerad administrering under studieperioden.

Barn 24 till 59 månader och spädbarn 9 månader:

  • Akut eller kronisk kliniskt signifikant lung-, kardiovaskulär-, lever- eller njurfunktionsavvikelse, bestämt genom fysisk undersökning eller laboratoriescreeningtest.
  • Kronisk administrering (definierad som mer än 14 dagar totalt) av immunsuppressiva medel eller andra immunmodifierande läkemedel under perioden som börjar 3 månader före den första vaccindosen. Stage 2: Geometric Mean Concentrations (GMCs) of Anti-serotype Specific Shigella Lipopolysaccharide (LPS)/O-Antigen (OAg) Serum Immunoglobulin G (IgG) in Participants 9 Months of Age in Africa

Anti-serotype specific Shigella LPS/OAg serum IgG GMCs were measured by enzyme-linked immunosorbent assay (ELISA) and expressed in ELISA units per milliliter (EU/mL) of serum. Four serotypes were tested. Due to the fact, that the Per protocol set (PPS) for Stage 2 Infants - dose finding cohort had less than the 72 participants per group defined in the protocol as a minimum number of participants to ensure power of the analysis, the Stage 2 Infants Safety cohort and Dose-finding cohort were pooled for the statistical analysis as per the Statistical Analysis Plan. As per protocol, statistical analysis was performed only for the S. sonnei serotype, comparing Stage 2 Infants: Pooled groups (medium vs low dose); and Stage 2 Infants Dose-finding groups (high vs low dose). The objective of this outcome measure is to identify the preferred dose of each component of the altSonflex1-2-3 vaccine for infants 9 months of age in Africa, therefore control groups were not analyzed.

IPD för denna studie kommer att göras tillgänglig via webbplatsen för begäran om kliniska studier.

Tidsram för IPD-delning

IPD kommer att göras tillgänglig inom 6 månader efter publicering av resultaten av de primära endpoints, viktiga sekundära endpoints och säkerhetsdata för studien.

Kriterier för IPD Sharing Access

Tillgång ges efter att ett forskningsförslag har lämnats in och har fått godkännande från den oberoende granskningspanelen och efter att ett datadelningsavtal har ingåtts. Tillträde ges för en inledande period av 12 månader men en förlängning kan beviljas, när det är motiverat, med upp till ytterligare 12 månader.
